Cntract f engagement This cntract f engagement is between FSB 4 Financial Limited (the adviser) and (the client). Purpse This cntract establishes the relatinship between the adviser and the client relating t nging service. The client has engaged the adviser t prvide the services that have been ticked: Insurance services T review and prvide advice n current insurances and the requirement fr additinal insurance prducts when requested t d s by the client. Prvide a 12 mnth review n existing insurances and whether these meet the client s gals. T speedily prcess any insurance claim based upn the infrmatin prvided by the client. T educate the client n insurances by the prvisin f selected articles. These will be delivered via email t the clients last stated email address. T retain cmprehensive file ntes n all discussins with the client. The abve services will relate t the prducts ticked. Medical insurance Life insurance Trauma insurance Ttal and permanent disablement insurance Incme prtectin insurance Business insurance General insurances Other Investment services (nn-kiwisaver) T act in the best interests f the client t meet the agreed investment bjectives and risk prfile. T meet with the client n a 12 mnth basis and prvide a prtfli valuatin reprt relating t hw the prtfli is meeting the client s bjectives. T keep the client infrmed abut investment pprtunities by frwarding relevant infrmatin t the client s last stated email addresses. T prvide an annual prtfli valuatin reprt and ecnmic summary. This will be sent t the client s last stated email address. T retain cmprehensive dcumentatin relating t the client s investments and bjectives. Page 1 f 5

The abve services will relate t the fllwing ticked slutins: Investment services using Aegis platfrm. The services will be actined in accrdance with the Aegis client agreement. Cnslidated tax reprts fr custdial assets will be prvided annually. The adviser will nt buy r sell securities withut the signed cnsent f the client. Investment services nt n a wrap platfrm: - There will be n cnslidated taxatin reprting. - The adviser will nt buy r sell securities withut the signed cnsent f the client. Mrtgage services - T ffer the client the pprtunity t review their mrtgages n a 12 mnth basis and prvide recmmendatins as t the suitability f the current mrtgages. Asset prtectin and estate planning services - As part f the investment r insurance review, the adviser will prvide (subject t btaining client infrmatin) (select as applicable) class service r persnalised service relating t asset prtectin and estate planning services. - Refer the client t a suitable specialist ffering asset prtectin and estate planning services. Tax services As part f the investment r insurance review, the adviser will prvide (subject t btaining client infrmatin) (select as applicable) class service r persnalised service relating t taxatin services. Refer the client t a suitable tax specialist. N advice/limited advice services - T undertake transactins n behalf f the client in accrdance with the client s written instructins and nly after the client has signed acknwledging the advantages f receiving a full persnalised service. Investment services (KiwiSaver) Planning Services Lifestage Plan - T prvide a reprt every 12 mnths that includes fund value and perfrmance summary fr the client s KiwiSaver prduct. This will be frwarded t the client s last stated email address. - T ffer the client, fr a fee f inclusive f GST, the pprtunity t meet and discuss issues assciated with KiwiSaver. - Upfrnt fee f $ + GST - Onging fee cmmencing immediately after plan implementatin f $20.00 + GST per frtnight. Adviser bligatins T maintain accurate recrds. T send the agreed upn reprts and infrmatin t the client s last knwn email address r t an alternative address agreed by the adviser and client. T use best endeavurs t ensure that the infrmatin and advice prvided is crrect. T advise the client f any change t adviser Registratin/Authrisatin. T prvide all advice in writing. T prvide a current disclsure statement at the time any financial advice is prvided. This will either be prvided in persn at a meeting r emailed t the client s last stated email address. T prvide the services in a timely manner. Client s bligatins T practively infrm the adviser f any material changes that may have ccurred since the last meeting. These include, but are nt limited t: Increased r decreased yur mrtgage(s)? Changed hmes? Entered int a new relatinship? Separated frm yur partner r spuse? Had mre children? Had children turn 21? Established a family trust? Changed trustees f yur trust? Changed ccupatins? Had a pay increase r decrease? Accumulated a sum f mney that enables yu t nw push yur incme prtectin wait perid ut t a lnger date, thereby lwering yur premium? Stpped smking? Had a change in health that may trigger a remval f an exclusin r lading n yur insurance? Retired r stpped wrk? Transferred the wnership f yur hme r ther insured assets t a family trust? Changed frm salaried emplyment t being self-emplyed? Changed yur gals? Started r stpped ther insurance plicies including any grup schemes and medical insurance? Been hspitalised? Page 3 f 5

Truthfully cmplete all applicatin frms. Ntify the adviser in writing if yur risk prfile may have changed. The client cnfirms that all funds used fr investment r the payment f insurance premiums r fees have been btained by lawful means. Pay all agreed upn fees at the agreed upn time. Review at least annually yur tax categries and PIR and infrm the adviser in writing f any change. Failure t prvide the crrect PIR culd result in the client paying the tp tax rate. Adviser remuneratin and cnflicts f interest Prviding the abve services, the adviser will be remunerated in accrdance with the adviser s current Secndary Disclsure Statement. Any ptential cnflicts f interest relating t the prvisin f the abve services are cntained in the adviser s Secndary Disclsure Statement. In the event f a dispute The adviser Disclsure Statement utlines the prcedure t be fllwed if the client has a dispute r wishes t make a cmplaint abut the adviser service. Terminatin This agreement will remain in place until it is terminated by either party giving the ther at least ne mnth s written ntice. Page 4 f 5

Client acknwledgement 1. I/We appint the financial adviser t implement the services ticked abve. 2. I/We have been supplied with: a) The adviser Primary and Secndary Disclsure Statements, current as at tday s date. b) Sufficient infrmatin relating t the abve mentined services, t enable me/us t make an infrmed decisin relating t these services. 3. I/We acknwledge that all investments have risk and the value f my/ur prjected return culd vary t that stated. I/We understand that there is n guarantee that sme r all f the capital and interest will be returned when I/we need it. 4. That all the infrmatin I/we have supplied is true and crrect and nthing that is deemed material has been mitted. 5. That my/ur circumstances culd change ver time and it is my /ur respnsibility t cntact the stated adviser when changes ccur that culd alter the applicability f the advice given in the SA. 6. That any limitatin in the scpe f advice has been limited by me/us and nt by the adviser. I/we acknwledge the advantages f receiving a full persnalised service rather than a limited service. 7. My/Our cntact details can be added t the adviser s database s I/we can receive regular updates, newsletters and infrmatin n additinal services prvided by the adviser. 8. I/We have the right t access and t crrect my/ur persnal infrmatin that is held n file. 9. Permissin is granted fr my/ur files t be reviewed by the Financial Regulatr r the adviser s external cmpliance persnnel, as part f the adviser s requirements t meet the regulatins and maintain industry best practice. 10. Permissin is granted fr staff and ther advisers within FSB4 Financial Ltd t access my/ur details and file t prvide the necessary advice in the event my/ur designated adviser is nt available. 11. I/We will prvide the adviser with any change f address (including email) plus any change in my/ur Prescribed Investr Rate (PIR). 12. The client indemnifies the adviser against any liability incurred by the adviser while acting prperly under the terms f this cntract.
