NURSING HOME SEARCH To search for a nursing home, either click the link called Nursing Homes (including short-term rehabilitation) or use the red menu bar at the top for Nursing Homes. At any time during your search, if you want to switch the type of home to review, just choose a different facility type using the top menu bar. Other facility types include: Assisted Living (RCFs). Licensed as Residential Care Facilities, these residences generally provide meals, housekeeping, laundry, transportation, and social activities. They also offer personal care, such as assistance with eating, bathing, grooming and personal hygiene. Some nursing care may also be provided. Supportive Living Facilities. These homes, also referred to as Adult Care Facilities, provide room, board and personal care for individuals who need assistance with daily living. Residential Treatment Facilities. These room and board to persons with mental illness. They all require a referral from a hospital, mental health agency or practitioner

After choosing to search for Nursing Homes, the site will display a page titled Nursing Homes, including short-term rehabilitation. The page has a great deal of information to help you in your search tips on selecting a nursing home, how they re licensed, how to pay for nursing home care and who to call for help in selecting or to report a problem. To begin your search, click the red button called Click here to search for Nursing Homes. TIP: If you frequently search for nursing homes, this might be a good page to bookmark to return in the future.

Facility Search: The facility search page will allow you to search for a facility by Geography and by the services available in the facility. The Type of facility will already be marked for nursing homes. You may wish to search by county, by the city, or by the distance to your zip code. We recommend you choose one search type at a time not all three to ensure you get sufficient responses to review. If you re in a rural area, start with a county search; in a large city, you may want to start with 5 to 10 miles from your zip code. To search by zip code, simply enter your 5 digit zip code into this box, then drag the marker to the left or right to increase or decrease the distance for your search. If you have a few nursing homes in mind already, you may wish to enter the names here in the Keyword Search box, separated by a comma. Use few of the words in the name for instance, type Happy Hills for Happy Hills Nursing and Rehabilitation Center.

The right hand side lists services that the facilities may or may not offer. If a facility has not logged onto the site and entered services, no results will appear if you search by these criteria. Try to choose as few as will help guide your search. Keep in mind, these are services the facility has indicated they offer; verify any information with a home before making decisions about long term care. After you ve performed your search, you ll get a results list of facilities. Here you ll see the facilities that meet your search criteria along with their city, county and the overall scores they received on their latest Family and Resident Satisfaction Surveys. To get the results of their latest inspection by the Ohio Department of Health, click on the small plus sign to the left of the facility s name to expand the selection. More inspection information is available if you select three facilities to compare (see below).

Facility Profile: For a facility profile, you can click on the faciliy s name (in red): The facility profile will list their address, contact information, and any policies, affiliations or services the facility staff have submitted to the site. Profile pages can be easily printed and used for comparison later off line.

Detailed Comparison: Backing up to the search results page, you can choose up to three facilities to compare at a more detailed level. To do so, check the box under the Select to Compare column: Additional navigation options are across the top: Satisfaction Surveys: The first tab Satisfaction Surveys will display numerically, and in graphs, how your three chosen facilities performed on the latest Resident and Family Satisfaction Survey conducted by the Ohio Department of Aging.

To get a detailed report of each facility s performance, select View Report. To see detailed comparisons of facilities scores on individual questions, click Click here for more Survey Question Comparisons. Inspections: The second tab Inspections will display how the three facilities performed on their last inspection by the Ohio Department of Health survey team. To view a facility s survey history and the details of each survey, click the date of the Most Recent Annual Survey.

From the facility s survey history, you may view details of survey citations by again clicking the date of the survey for which you want to view more information. The detailed information will show the deficiencies, whether or not they indicated substandard care or immediate jeopardy and whether, on a survey team revisit, the deficiencies had been corrected.

The remaining tabs, Location & Affiliations, Special Care Services, Services to Non-Residents, Policies, Beds & Rates and Staff will display other information, much of it provided by the facilities themselves to aid in your search.
