The Examinations Board may appoint experts from outside the institution to be examiners. Examiners shall provide the Examinations Board with information upon request. The Examinations Board applies a fixed procedure for the appointment of examiners. 7 End-of-term exams Article 7.1 Exam dates and times 1. This Article supplements the provisions of Article 3.3, Section A of the 2013-2014 Teaching and Examination Regulations for the Bachelor's programme in Medical Informatics and the Master's programme in Medical Informatics. 2. Written end-of-term exams will be administered under the direction of the Examinations Board on dates and at times determined at least 30 days before the start of the module concerned. 3. The time and location of written end-of-term exams may be changed up to five working days in advance. The most up-to-date times and locations of end-of-term exams are posted on www.rooster.uva.nl. 4. Oral or individual written end-of-term exams are administered at a time determined by the examiner(s). 5. When scheduling the dates and times of end-of-term exams, every effort is made to ensure they do not overlap. 6. Dates and times will only be changed in the event of exceptional circumstances. 7. Examiners/coordinators are not authorised to offer students additional opportunities to take endof-term exams. 8. The Examinations Board will not offer additional end-of-term exam opportunities to individual students, even in the case of timetable conflicts, last end-of-term exams, the 'Bachelor-before- Master' rule and so on. Article 7.2 End-of-term exam registration and deregistration 1. Students will only be permitted to take an end-of-term exam if they have registered for the exam in the proper and timely manner prescribed and are registered in the University's Student Information System (SIS). 2. Contrary to the provisions of Article 3.2, paragraph 1, Section A of the 2013-2014 Teaching and Examination Regulations for the Bachelor's programme in Medical Informatics and the Master's programme in Medical Informatics, students must register themselves in SIS during the set registration periods for all end-of-term exams and partial tests. 3. The registration period for end-of-term exams runs from the day the course starts up to five working days before the exam. The registration deadline is specified in the test timetables on Blackboard. 4. The registration period for retakes is stated in the test timetables on Blackboard. 5. Students who fail to meet the entry requirements for an end-of-term exam are not permitted to register. 6. Registration is not definite until the end-of-term exam appears under the 'Deregister' tab in SIS. Students will also receive an email to confirm this, which they must take care to save as it is their proof of registration. If the end-of-term exam does not appear under Deregister tab in SIS, the Rules and Guidelines for the Medical Informatics Examinations Board 4/11

student must contact the Programme Administration before end of the registration period (onderwijsadministratie@amc.uva.nl). 7. It is not possible to register for an end-of-term exam during the exam or retroactively. 8. Any student who attends, takes and hands in an end-of-term exam without having registered will not have their exam marked unless the Examinations Board retroactively declares their exam to be valid. 9. Students can deregister for end-of-term exams by sending an email to the Programme Administration (onderwijsadministratie@amc.uva.nl) from any time after the course has started until 12:00 on the working day before the exam. 10. If, between 12:00 on the working day before the end-of-term exam and the time at which the exam is to start, a situation arises whereby a student cannot take the end-of-term exam, the student must deregister in the manner described in paragraph 9 and notify the module coordinator by telephone. 11. Deregistration becomes definite upon receipt of a confirmation email from the Programme Administration. Students must take care to save this email confirmation. 12. Students who fail to deregister for an end-of-term exam on time and do not attend the exam are nevertheless considered to have taken part and will receive a mark of NA (attempted but Not in Attendance). This will be counted as a valid end-of-term exam opportunity. 13. In very exceptional cases, students may invoke the hardship clause. In that case, the student must report to the study adviser no later than five working days after the end-of-term exam. Article 7.3 Rules of order for tests/end-of-term exams Prior to the test/end-of-term exam 1. Students must be at the location on time and will be admitted to the test/end-of-term exam location five to ten minutes before it is scheduled to start. 2. Students are not permitted to wear sunglasses, a head covering (such as a hat or cap), burka and/or other items that fully or partially cover the face, as this prevents the proper assessment of cheating. Wearing a headscarf is permitted as it does not fully or partially cover their face. Late arrival at a test/end-of-term exam 3. If circumstances lead a student to arrive late, they may join the test/end-of-term exam only when the invigilator signals that they may do so. The student should wait at the door until signalled and the invigilator will let them in. Students admitted after the test/end-of-term exam has started will not be allowed any additional time to complete it. Students who arrive after the time at which late arrivals are permitted to join will not be allowed to take the test/end-of-term exam. Admission to the test/end-of-term exam location 4. All conversation must cease and everyone is to keep silent from the moment of entering the test/end-of-term exam location. 5. Coats and bags are to be placed to one side of the table. Mobile/communication devices must be fully switched off and stored in students' coats or bags. Carrying a mobile/communication device on one's person (e.g. in clothing) will be treated as fraud. Students are not allowed to retrieve anything from their bags or coats during an test/end-of-term exam without first asking permission from an invigilator. 6. Students should find a suitable seat, following any instructions from the invigilator, and sit down. Students are not permitted to start a test/test/end-of-term exam until the invigilator has completed the introduction and signalled that students may start. During the test/test/end-of-term exam 7. When taking a test/test/end-of-term exam, students must provide proof of their identity based on their valid proof of enrolment (student ID card) and a valid identification document with a recognisable passport-type photo. Students must place these documents on the table so they are visible. Upon the invigilator's request, students must place their signatures on the end-of-term exam form. Students who cannot provide a proof of identity may be refused admission to the endof-term exam. Rules and Guidelines for the Medical Informatics Examinations Board 5/11

8. All items required for the test/end-of-term exam (in any case including a HB pencil, pen, eraser and, where relevant, a calculator) must be placed on the table in advance. Pencil cases may not be placed on the table. 9. Non-graphical calculators may only be used if the end-of-term exam cover sheet explicitly states that this is permitted. The use of graphical calculators or calculators with additional memory is not permitted. Students must present their calculator to the invigilator for inspection upon request. 10. During a test/end-of-term exam, students are not permitted to place any syllabi, textbooks, references or notes on the table unless it has been explicitly agreed and stated that these items are permitted. In the latter case, the invigilator may inspect the material. The invigilators can provide students with scrap paper if required. 11. During a test/end-of-term exam, students are only permitted to ask questions regarding the meaning of a test/end-of-term exam question. 12. Students are not permitted to use the toilet during the first hour and the final half hour of an endof-term exam. Between these times, the use of toilet facilities will be restricted. If a student wishes to use the toilet facilities, they should raise their hand and wait for permission from the invigilator. Only one student will be permitted to go the toilet at a time. No mobile/communication devices may be taken along or food or beverages purchased while going to the toilet. Students are not permitted to use the toilet facilities during tests/end-of-term exams lasting one hour or less. 13. If a student leaves the exam hall without permission and/or during the period in which use of the toilet facilities prohibited, they will not be permitted to finish the test/end-of-term exam. Students must hand in their work and leave the exam hall. 14. Students may not leave the exam hall until their attendance has been registered. In the case of end-of-term exams, students are not permitted to leave the exam hall during the first 45 minutes after the exam has started. 15. If eating/drinking is permitted in the test/end-of-term exam room, students may consume food (snacks) and drink they have brought themselves (within limits), provided they do so quietly and at the discretion of the invigilator. The invigilator may confiscate food and drink if it causes a disturbance. 16. Foreign students are permitted to use a dictionary and are allowed more time (an additional ten minutes per hour) to take tests/end-of-term exams. This only applies if they have submitted a written statement from the study adviser to the Programme Administration at least two weeks before the test/end-of-term exam. The dictionary must not be an encyclopaedic dictionary and it must be inspected by an invigilator in the exam hall. Also see Article 7.6 of these Rules and Guidelines. 17. Students with disabilities may be allowed additional facilities during regular final end-of-term exams (longer test/end-of-term exam time, larger font, wider line spacing, etc.), provided they submit a written statement from the student counsellor to the Programme Administration at least two weeks before the first end-of-term exam of the academic year concerned. Also see Article 7.5 of these Rules and Guidelines. 18. At the end of an end-of-term exam, students will be asked to complete an evaluation form. Coordinators and the programme and evaluation committee urgently request that students complete this evaluation form. 19. The above rules of order may vary depending on the type of test/end-of-term exam. The invigilator will orally announce these rules at the start of the test/end-of-term exam. 20. The instructions of invigilators must always be followed. 21. The Examinations Board emphasises that the fraud regulations referred to in Article 3.11, paragraph 1, Section A of the 2013-2014 Teaching and Examination Regulations for the Bachelor's programme in Medical Informatics and the Master's programme in Medical Informatics apply during the administration of tests/end-of-term exams. 22. Non-observance of the rules of order will result in students being excluded from the test/end-ofterm exam. Article 7.4 Invigilation during end-of-term exams Rules and Guidelines for the Medical Informatics Examinations Board 6/11

Tests/end-of-term exams administered within the Bachelor's and Master's programmes in Medical Informatics are subject to an invigilation protocol laid down by the Examinations Board. Article 7.5 Additional facilities for students with disabilities 1. Students with disabilities will be given the opportunity to take tests/end-of-term exams in a manner adapted to their specific disability as much as possible. 2. Dyslexia is also treated as a disability. Procedure 3. In order to qualify for these additional facilities, students must submit a request to this effect to the study adviser at the start of the academic year. The student will then be referred to the UvA student counsellor. 4. The student counsellor will assess the situation and, in the case of an established disability, issue a statement for a certain period of time (usually one or two academic years) granting one or more facilities. 5. The student must present this statement from the student counsellor at the student desk at least two weeks before the first end-of-term exam in order to be able to use the facilities granted for all regular corresponding end-of-term exams in the academic year concerned. 6. If a student wishes to retake an end-of-term exam, they must send an email to the Programme Administration at least one week before the exam retake date requesting the use of the previously granted facility/facilities for the retake. 7. After the period of validity of this statement has expired, students must request a new one from the student counsellor. Article 7.6 Use of dictionaries by foreign students 1. This Article applies to students enrolled in the Bachelor's programme in Medical Informatics. 2. When taking tests/end-of-term exams, foreign students in the first year of the Bachelor's programme in Medical Informatics are entitled to the following: a. the use of a dictionary; b. ten additional minutes per hour, if required. 3. To quality for the above, the student must: a. contact the study advisor at least one month before the end-of-term exam. The study adviser will issue a statement specifying the dictionary and whether the student has permission for an extension of the test/end-of-term exam time; b. present this statement at the student desk at least two weeks before the end-of-term exam so that the appropriate arrangements can be made in time; c. present the dictionary to an invigilator in the test/end-of-term exam hall for inspection. Article 7.7 Objections to test/end-of-term exam questions 1. After a test/end-of-term exam, students can object to or query the test/end-of-term exam questions and answer models by submitting a written description of their objection/query to the examiner/course coordinator. 2. Such written objections/queries must include the submitter's name and student ID number and provide a numbered list of comments and/or queries per question. 3. Objections must be received no later than five working days after the test/end-of-term exam was administered. 4. The coordinator/examiner will respond to objections within two weeks or in any case before the test/end-of-term exam marks are announced. Article 7.8 Assessment 1. An end-of-term exam represents an investigation by the examiner into students' knowledge, insight and skills, together with an assessment of the results of that investigation. 2. Written end-of-term exams are assessed on the basis of standards that are determined and recorded in advance, which may be adjusted pursuant to the correction if necessary. Rules and Guidelines for the Medical Informatics Examinations Board 7/11

3. In the event that more than one examiner is involved in the assessment of an end-of-term exam, the Examinations Board will ensure that all examiners use the same standards for the assessment. 4. The assessment method must be such that students can understand how their test/end-of-term result was determined. 5. The most recent marks determine the final mark. 6. In the case of practical exercises where several students contribute to a single group project, the course coordinator will determine the method by which each individual student is to be assessed. 7. Bachelor's theses are assessed with the aid of an assessment form and based on a weighting of the marks for each sub-component. The marks for these sub-components are determined by different people. The final mark is determined by the Bachelor's work placement and thesis coordinator. 8. Master's theses are assessed by the Scientific Research Project Examination Committee on the basis of an assessment form. The final mark is determined by the committee chair. Article 7.9 Inspection regulations After the assessment of an end-of-term exam, students have the right to inspect their assessed work. This right is laid down in Article 3.8, Section A of the 2013-2014 Teaching and Examination Regulations for the Bachelor's programme in Medical Informatics and the Master's programme in Medical Informatics. Article 7.10 Reassessment of end-of-term exam questions 1. Students have the right to a reassessment of end-of-term exam questions. This right is laid down in Article 3.5, paragraph 4, Section A of the 2013-2014 Teaching and Examination Regulations for the Bachelor's programme in Medical Informatics and the Master's programme in Medical Informatics. 2. Students can submit a written request for a reassessment to the relevant course coordinator/examiner within four weeks after the day on which the results are announced. 3. Such requests must include the name and student ID number of the student as well as a description of the end-of-term exam and the specific exam questions of which reassessment is requested. For each question, the student must provide arguments (e.g. relating to the content) substantiating their request. 4. The coordinator must respond to this request as soon as possible and in any case within three weeks after receipt of the reassessment request. 5. The coordinator/examiner will pass the request on to the lecturer who assessed the student's answers to the questions. 6. Based on this request, the lecturer will once more assess the student's answers and inform the coordinator/examiner whether the reassessment has resulted in an adjustment of the number of points, providing a substantiation if possible. 7. After the reassessment, the coordinator/examiner informs the student of whether the reassessment has resulted in an adjustment of the number of points awarded for the question concerned, if possible substantiated with the lecturer's arguments regarding the number of points awarded. 8. If a student disagrees with the first reassessment, they may ask the coordinator/examiner to have a second reassessment carried out by a different lecturer. 9. The Examinations Board is responsible for supervising that procedures for the reassessment of end-of-term exams are carried out correctly. 10. The Examinations Board will not concern itself with the specifics of the student's and/or assessor's argumentation. Article 7.11 Quality assurance for testing The procedures applied by the Examinations Board to assure the quality of testing are set out in Appendix 7 of the AMC-UvA's Medicine and Medical Informatics Education Quality Assurance Handbook. 8 Final exams Rules and Guidelines for the Medical Informatics Examinations Board 8/11

Article 8.1 Final exam dates 1. The Examinations Board will automatically proceed to award a diploma as soon as a student has satisfied the requirements. 2. The University has 12 fixed final exam dates per year. 3. The degree certificate will bear the date of the last working day of the month following the last successfully completed end-of-term exam. 4. Bachelor's degree ceremonies are held at least twice a year. 5. Master's degree ceremonies are held at least twice a year. 6. Students must register to take part in the graduation ceremony using the designated form on Blackboard. 7. Additional information about graduation can be found at student.uva.nl. (http://student.uva.nl/mi). Article 8.2 Degree classifications 1. The Examinations Board has different rules for the allocation of the designation of cum laude to different groups of students. These different groups are: a. Students who enrolled in the Bachelor's programme in Medical Informatics or in the Master's programme in Medical Informatics after 1 September 2011 (see paragraph 2 of this Article). b.
