3 Main Referral Hubs Managed by a CAHP and SSVF Matcher 1. HSN-Supportive Services for Veteran Families (SSVF) (HMIS ID: #648) 2. CAHP - Permanent Supportive Housing (PSH) (HMIS ID: #812) 3. CAHP - Navigator Match (HMIS ID: #811) ALL AGENCIES MUST COMPLETE VI-SPDAT and provide contact information for client before making a referral.

1. HSN-Supportive Services for Veteran Families (SSVF) (HMIS ID: #648) Refer: Any veteran clients (before making the referral the client must have contact information and a VI-SPDAT Completed ) Once referred to the prioritization list the SSVF Matcher will make an appropriate referral Clients scoring 5+ and answer yes to the CH question will be referred for VA programs Clients scoring 0-9 will be referred to SSVF provider.

2. CAHP - Permanent Supportive Housing (PSH) (812) Refer clients that meet all the following: Score 10+ on the VI-SPDAT Are chronically homeless and are document ready. Document Ready Checklist Letter of Disability Letter of Chronic Homelessness Valid ID SSN

3. CAHP - Navigator Match (811) Refer clients that: Score 10+ on the VI-SPDAT Are chronically homeless Do not have any contact with outreach or CoC Agencies with capacity

HOW TO COMPLETE REFERRALS IN HMIS TO MAIN HUBS

Go to client Click on service transactions

Click on Add Referral

A) Select appropriate need B) Once highlighted click on > Add Terms

Once the need is selected all providers in HMIS that provide a service related to that need will populate in the search results. You will select one of the 3 main hubs managed by a CAHP and SSVF Matcher 1. HSN-Supportive Services for Veteran Families (SSVF) ( 2. CAHP - Permanent Supportive Housing (PSH) 3. CAHP - Navigator Match ALL AGENCIES MUST COMPLETE VI-SPDAT and provide contact information for client before making a referral. You select the referral provider by clicking the green plus sign.

Once select the provider will populate in the selected provider area

Attaching a VI-SPDAT score to Referral You select the VI- SPDAT score by clicking the green plus sign. Click on search It is essential that a VI-SPDAT score is attached to the referral since this is the base of our prioritization list. Referrals will not be accepted if they do not have an attached score

Click on Save ALL
