1 Baccalaureate Degree Program Application for Admission & Readmission RN-BSN Track Please read the application carefully and fill it in completely. Incomplete applications will not be accepted. Admission Requirements Admission into the RN-BSN program is selective. All applications are carefully reviewed by the School of Nursing s Student Affairs Committee. Minimum admission qualifications: Applicants must be admitted to the University of South Carolina Upstate there is a separate application process for this available on the website. Completion of all pre-requisite courses (lower general education studies) with a grade of C or better by the end of the spring term is required. Applicants must have a diploma or associate degree in nursing and an active, unencumbered license. Criminal Record Background Check (CRBC) All applicants for admission must complete a CRBC; which must come back clear. Students applying for readmission must complete a CRBC if there has been an interruption in your coursework (you missed one or more semesters). The form for the CRBC will be mailed to applicants when an admission decision is made. Submit Completed School of Nursing Application to: Mary Black School of Nursing 225 S. 2 Application Checklist Your application to the Mary Black School of Nursing RN-BSN Program will be considered incomplete and will NOT be sent to Committee unless the following items have been completed: The application was turned in by the stated deadline. All items on the application are completely filled out. You have applied to, or have been readmitted to, USC Upstate. You do not have to wait to be accepted to USC Upstate to apply to the School of Nursing. o Official transcripts were sent to and received by the Admissions Department of USC Upstate with ALL grades earned. This can take several weeks; please do not delay sending in transcripts. Don t forget that you must send in grades of any courses you are currently taking as soon as the semester is over. It is not necessary to send transcripts from any USC campus. o Students will have to have been fully accepted to USC Upstate for their application to be considered. Official transcripts were sent to and received by the School of Nursing with ALL grades earned. Ideally, these transcripts will accompany your application. The SON cannot accept electronic transcripts. They must be handed to us directly or mailed (UNOPENED) to: The Mary Black School of Nursing, 225 Pleasantburg Dr., Greenville, SC Don t forget that you must send in grades of any courses you are currently taking as soon as the semester is over. Again, it is not necessary to send transcripts from any USC campus. o Some schools insist on sending electronic transcripts. In that case, you should send an electronic transcript to the Admissions Office AND hand-deliver or mail a second copy to the School of Nursing. 3 Frequently Asked Questions 1. How do I submit my RN to BSN application? a. Please print off this application, complete the form, and mail to the following address: Mary Black School of Nursing 225 S. Pleasantburg Drive Greenville, SC Do I have to send my transcripts to both the School of Nursing and the University? a. Yes, we require that you send separate official copies of your transcript to both the School of Nursing and the University. 3. Do all of my prerequisite courses have to be completed before the program starts? a. Yes. 4. I have been accepted to USC Upstate does that mean I am automatically accepted into the RN to BSN program? a. No, acceptance into the University does not guarantee admission into the RN to BSN program. You must satisfy all of the prerequisite course requirements, possess an active license, and be admitted to the University in order to be considered for admission. 5. Where do I send my citizenship verification and immunization forms? a. The School of Nursing does not require the citizenship verification or immunization form. Those are University requirements. You will need to contact those respective departments in reference to those forms and their submission. They can be reached at Did you receive my transcripts? a. Due to the large volume of applicants we are unable to individually respond to each request checking for transcripts. Because your transcripts are sent both to the University and our department the easiest way to confirm receipt is by checking your University admission status. That application will note receipt of all transcripts. 7. When do I have to complete the Criminal Record Background Check? a. Information on the Criminal Record Background Check will be sent to you in May once admission decisions have been extended. The background check must be clear.
