TIU Note Templates: Overview for Pharmacy
Learning Objectives Define key terms associated with templates List resources for future reference List some advantages and disadvantages of templates Identify key parameters for obtaining access to template editing functions Be able to navigate RPMS/EHR notes and editing functions Understand the 2 extremes in template design Recall some examples used in pharmacy practice Build a simple template using attached script 2
Colleagues List-serves The IHS web page National guidelines Links Resources mplates ftp://ftp.ihs.gov/pubs/ehr/templates/ 3
Templates: The Jargon (or how to talk nerdy ) TIU = Text Integration Utility Building blocks Data Objects Items that may be embedded into a template to help populate patient information automatically Template Fields Used to provide structure and input options May choose from list or create new ones with the correct access 4
Templates: The Jargon (or how to talk nerdy ) My Templates A list of templates defined by the user, for that user Access may be given for editing Shared Templates A list of templates available to all users Usually very limited access 5
Advantages of Templates Consistency Completeness Clinical guidance Clean (Maybe) 6
Disadvantages Clutter (busy, extraneous information) Carelessness (lack of thought, rote) Clicks (nested, complicated) Cut n Paste 7
Parameters: Giving and Getting Access 8
Parameters for Templates TIU PERSONAL TEMPLATE ACCESS Who can make personal templates. The choices are: 1 READ ONLY 2 NO ACCESS 0 FULL ACCESS If the system level is full access, all users can make personal templates 9
Parameters for Templates TIU TEMPLATE ACCESS BY CLASS Same as other parameter, but by user class. Choices: 0 FULL ACCESS 1 READ ONLY 2 NO ACCESS 10
Parameters for Templates TIU TEMPLATE PERSONAL OBJECTS This parameter controls which objects a user can insert into a personal template. 11
Parameters for Templates TIU FIELD EDITOR CLASSES Who can create template fields. At System level, this should be set for Clinical Coordinators. 12
Template Creation Most templates will be created using the graphical interface. Users can also create their own templates. Shared templates can be created by anyone in the Clinical Coordinator user class. Templates can also be imported from other sources. 13

Template Fields 21
Template Fields 22
Template Fields Streamline data entry portions of note and help provide some decision support Edit Box Combo Box Button Check Box Radio Button Date Number Word Processing Text Hyperlink 23
Edit Box Fields A phrase of text which, by default, will drop into note, and that can be edited (overwritten or deleted) 24
Also called Drop-Down Default value chosen Combo Box Fields 25
Button Fields Allow selection of one item by cycling through each item as the button is pressed. If the Default field is blank, the button will initially appear without any text 26
Check Box Fields Creates a check box for each item More than one item may be selected Default item will initially be checked If multiple items are checked, the text will be separated by commas 27
Radio Button Fields Creates a radio button for each item One item may be selected The default item will initially be checked Can be unchecked, leaving no selection 28
Separate Lines Property Check Box and Radio Button fields have a Separate Lines property, which will display each check box or radio button on a separate line of the dialog. Has no affect on generated text 29
Separate Lines Property (cont.) Before Separate Lines After Separate Lines 30
Date Fields Date Fields have six different date types Default will evaluate to the current date/time 31
Number Fields Number fields allow numeric values between a specified range. They always have a default value. Minimum and Maximum Values can range from 9999 to
Word Processing Fields Word processing fields allow an unlimited amount of text to be entered (scroll bars will appear if needed) 33
Hyperlink Fields Hyperlinks fields are text-only fields that allow linking to Web pages. Hyperlink text is either the Default (if specified) or the actual URL address. Can be display only. 34
Text Fields Text fields provide a mechanism for embedding predefined (noneditable) text into the dialog Can be display only 35
Dialog Templates Portions of templates can be grouped for better organization/flow 36
Just a few thoughts. Getting the desired structure and content Fairly easy Getting a fast, refined and user friendly template Not so easy Trial and error; refinement with time Two extremes of design click driven the blank form 37
Local Approval Each site may have protocols and policies that govern TIU templates Electronic equivalent of the forms committee Access to edit personal templates Everyone? Limited? Basic content Approval of templates to be shared 38
Examples Open to each site to show live versions A few screen-shots for general ideas 39
Example of a Click-Driven May nest templates within a template Allows the user to choose what is needed; provides options Good for repetitive, canned type responses and work-flows Can be cumbersome for patient encounters; stuck in the box 40
Template Example: Initial View 41
Template Example: Each Click Expands Options 42
Template Example: Each Click Expands Options 43
Template Example: Each Click Expands Options 44
Template Example: Each Click Expands Options 45
Template Example: Each Click Expands Options 46
Final Result Once Out of the Box Only what you click will be here 47
Example of a Template as a Form No options up front, need to cut out what you don t want; add other items Easier good typists; may be more patient friendly Provides structure and consistency No box to get stuck in Free to navigate around E.H.R 48
Template as a Form 49
Think about: Fine-Tuning If you always want the information in the note Try to not make it a click box, or worse multiple clicks to get the information Potential to be left out Tedious Not patient friendly Vs. optional information Ideal for click boxes 50
Example: Too Many Clicks Not only do you have to click once, but TWICE to get INR information into an anticoag note! Very tedious and non-patient friendly. 51
