4 Ageing In Place. (A Service Guide for Healthy Ageing) A Note from SingHealth An ageing population will pose increased health, economic and social challenges to Singapore. There has been a rapid expansion of health and community services for the older persons, both by the government and voluntary welfare organisations, to meet their health and social needs. Many older persons are very healthy and lead independent lives. However, there are also others who require health and support services. Various home-based community services are available to assist the older persons and their caregivers to help ensure appropriate care at home. In SingHealth, we believe that everyone should be empowered to manage his or her own care. True empowerment stems from ready access to information. With this ready access, one is supported to make critical and informed decisions about their health and lifestyle. This is paramount for the older persons and their caregivers. This guide aims to create awareness among healthcare providers in the community on the types of services available for the older persons, so that they can better advise those under their care. A joint effort with the National Council of Social Service and Integrated Care Services, this is a SingHealth initiative to bring together key stakeholders in aged care, harnessing the strengths of each partner in creating an effective community healthcare model for the people in Singapore. 7 The Healthy & Active Seniors Staying Healthy and Active Everyone has a role to play in the care of the older persons who is an integral part of our society. A wide range of eldercare support services and programmes is available for them to stay physically and mentally active as well as socially engaged. These services and programmes are developed to promote ageing in place, where the family concept is preserved as the best social insurance for its members, thereby allowing the older persons to age gracefully in familiar surroundings whilst maintaining their self-worth and dignity. 8 The Healthy & Active Seniors Neighbourhood Links Neighbourhood links are physical nodes in the community where the older persons can obtain information on social services, volunteer their services, interact and form mutual help groups. They also provide a broad array of programmes to promote inter-generational interaction. 13 The Healthy & Active Seniors Sheltered Homes The Sheltered Home is a residential facility catering to the needs of the ambulant senior citizens who are without next-of-kin or for certain reasons, unable to stay with family members. These Homes, located within HDB neighbourhoods, provide support services to assist and enable seniors to maintain their independence within the home and community. Social, recreational and keep-fit activities such as exercise, handicraft and karaoke are organised for the residents. Admission criteria: 1. 16 Home Introduction Many older people prefer to live in a familiar environment with their family members and friends. However, in the event that the older person and / or the family cannot manage to completely care or support the needs of the older person, home-based or community-based services may be required to help the older person to continue staying at home, while providing some form of respite and support for their caregivers. 17 Home Befriending Services Befriending Services aim to promote the general emotional and psycho-sociological wellbeing of the lonely and vulnerable elderly living in the community. The programme recruits and trains volunteers (including healthy seniors) to befriend and provide support to the isolated and vulnerable seniors who are homebound, have no family support and at risk due to physical, health, social and / or psycho-emotional reasons. The volunteers visit these seniors on a regular basis and help them in simple household chores, running of errands and ad hoc escorts for medical appointments. Where possible, the volunteers also accompany these seniors for group outings or to participate in social and recreational activities. 18 Home Day Care Centres (Social Day Care Centres) Social Day Care Centres provide daytime supervision and care to frail seniors while their family members are at work or require respite from their caregiving roles. The programmes conducted by the Centres include maintenance exercise as well as social and recreational activities such as handicraft, singing and karaoke. These programmes help to maximise the physical functions of the seniors while offering opportunities for them to socialize and engage in meaningful activities. 22 Home Day Rehabilitation Centres The Day Rehabilitation Centres provide rehabilitation services such as physiotherapy and occupational therapy to the elderly who suffer from stroke, heart condition, fractures and other conditions that impair functional abilities. This service aims to restore, improve and / or prevent any deterioration of functional abilities of the older person through customized programmes of exercise and training. In addition, the therapists will also counsel and train caregivers to care for the patient. Typically, the clients at these centres suffer from musculoskeletal conditions, stroke, fractures, and disabilities. Hence, it is important that the older persons be assessed by a doctor to certify that he is suitable for a rehabilitation programme. If an older person requires day rehabilitation services, he can approach his doctor from the hospital, community hospital, or polyclinic that knows of his condition to make the referral. Some day rehabilitation centres can be paid through Medisave. To receive subsidies for this service, means testing is required. 27 Home Dementia Day Care Centres Dementia Day Care Centres provide a structured day care programme to accommodate older person who suffers from dementia. This service will relieve the care person or family member from their care giving commitments by providing custodial care to the elderly person in the day. These centres also provide training for the older persons and their caregivers. Firstly, it trains the older persons to care for themselves as much as possible and secondly, it provides skills to caregivers on how to cope with the demands of caring for a dementia patient. Those who attend Dementia Day Care Centres should be able to either walk independently or with the use of walking aid. They must be diagnosed to have dementia by a doctor and they should not display unmanageable behaviors that are violent and / or disruptive. If an older person with dementia requires this service, a doctor from the hospital, community hospital, polyclinic or clinic that is aware of his condition will be able to make the referral to the appropriate service provider. To receive subsidies for this service, means testing is required. 32 Home Home Medical Service Home Medical Service provides ongoing medical care to the homebound older person through regular home visits. This service, usually comprising of a doctor, nurse and social worker, provides holistic care to the patient and family in terms of medical treatment, nursing care, psychosocial input, health education and training. The team also provides ongoing support to the family by coordinating services for the older person and family to ensure that he remains in the home as long as appropriate. Some of the services that the team will link and coordinate are meals delivery, home help, befriender and financial assistance. Frail and / or bedridden older person who have multiple medical or nursing needs with permanently disability and / or unable to leave the flat (for e.g., not on lift landing) will be suitable for this service. 38 Home Hospice Day and Home Care Hospice is a specialised form of care designed to provide palliative care, physical and emotional comfort to patients and their families when a terminal illness no longer responds to treatments. The objective of hospice care is to prepare patients and their families to attain a degree of mental and spiritual acceptance of death that is meaningful. There are 2 types of hospice services available for seniors who wish to stay at home: Hospice Day Care Hospice Day Care Centre operates during weekdays. It provides support to persons in the early stages of terminal illness and / or those who are still able to cope with traveling. The programme includes rehabilitation and social / recreational activities to promote physical and emotional well-being of the older persons. Hospice Home Care This service reaches the homebound elderly who require medical and nursing care to control symptoms and manage pain. These visiting healthcare professionals also monitor the older persons condition and provide necessary training and support to the family. A need to engage institutional service providers such as nursing homes and community hospitals is sometimes necessary. 42 Care in the Community (Intermediate & Long-term Care - Residential Facilities) Community Hospitals A Community Hospital is an intermediate care facility between an acute hospital and community services. It provides continuation of care from acute hospitals. The objective of community hospital is to promote functional independence of the older persons. Hence, it caters to those who require longer inpatient stay for rehabilitation, nursing and respite care after treatment in the acute hospitals. Typically, community hospitals cater to patients who require: Rehabilitation This group of patients suffers from stroke, fracture and / or other medical conditions. They have good potential to improve in their ability to care for themselves. Their medical conditions are relatively stable. Patient and family are prepared to participate in the programme. Sub-acute care This caters to patients who do not require the level of care provided in hospital but are not well enough to be managed at home. They are usually discharged for continued management at these sub-acute facilities with clearly defined care plans. Respite care Lastly, respite care is a form of short-term custodial care for patients who may have complex care needs but may not require medical care. It also helps caregivers, who after a prolonged period of caregiving require some short-term relief. In order to be admitted to a community hospital, the patient must qualify for the Community Hospital s admission criteria. The referral has to be made by the doctor, who may be from hospitals or clinics, and is aware of the patient s conditions. The referring doctor will help to fill up the Community Hospital Referral Form and the patient and his family will be informed of the outcome by the respective Community Hospital. 44 Care in the Community (Intermediate & Long-term Care - Residential Facilities) Nursing Homes The older person can be admitted into a Nursing Home if he needs daily nursing care and / or assistance in activities of daily living (such as toileting, walking, grooming and bathing) and if he has no caregiver at home. The older person must be dependent, wheelchair bound or bed bound. In addition, he must be medically assessed to be suffering from physical or mental dysfunction requiring high level of nursing care. The Nursing Homes in Singapore may be run by private commercial operators or Voluntary Welfare Organizations (VWOs). VWOs Nursing Homes The common objective of the VWOs nursing homes is to assist the low income, destitute and poor. One group receives funding from the government and another group does not receive any sort of funding. If one requires subsidies, he or his caregiver may consult the Doctor and Social Worker from the hospitals, national centers, community hospitals, and polyclinics that provide care for the older person. If the older person is not known to any hospitals, national centers or polyclinics, he may approach the nearest Family Service Centers (FSC) or Community Development Councils (CDC). Please refer to for the nearest CDC. For application to government-funded nursing homes, the following components would have to be completed by the older person s doctor and social worker: a) Particulars of Older Person and Social Report (by Social Worker) - includes personal details and social history of the older person and the family b) Medical Report (by Doctor) - information on the applicant s medical condition and its management plan c) Functional Status and Nursing Needs (by Nurse or Doctor) - an assessment tool known as the Resident Assessment Form (RAF), this is used to assess the level of nursing care required by the older person, and thereby the costs for the applicant s stay in the Nursing Home. d) Means Testing If the applicant requires subsidies for this service, means testing is required. Upon completion of these components, it will be directed to Integrated Care Services (ICS), which is the central agency responsible for the placement of applicants to all governmentfunded nursing homes. Applications will then be channelled to an appropriate nursing home according to medical and nursing needs, religious affiliations, dietary preferences and geographical location. C.04
45 Care in the Community (Intermediate & Long-term Care - Residential Facilities) Private Nursing Homes If one does not require any financial assistance from government for admission to a Nursing Home, one may consider a private Nursing Home. To apply for admission, bring along a medical report that is completed by the doctor who is aware of the older person s conditions. It is also recommended to make a visit to the Nursing Home to understand its philosophy, medical and nursing services, religious affiliations, dietary preferences and geographical location before making the decision. Discharge Planning When the patient s medical condition has improved to a level where the nursing care needs is considered minimal, and/or that patient only requires assistance in Activities of Daily Living (ALDs) such as bathing, toileting, feeding and mobility, the Nursing Home would discuss discharge planning with the family. Families should be prepared for this possibility, as Nursing Homes are not for long-term placements. Respite Care Services Respite care is a form of short-term custodial care for patients who exhibit challenging behaviours that may or may not require medical care. It also helps caregivers, who after a prolonged period of caregiving require some short-term respite. Respite care services are available in private and some VWO nursing homes. However, one would need to pay the full rate. 53 Care in the Community (Intermediate & Long-term Care - Residential Facilities) Hospice Inpatient Service Hospice is a specialized form of care designed to provide palliative care, physical and emotional comfort to patients and their families when a terminal illness no longer responds to treatments. The objective of hospice care is to prepare patients and their families to attain a degree of mental and spiritual acceptance of death that is meaningful. When the older persons condition becomes too difficult to care for at home, and the prognosis for him is less than 3 months, they can be admitted to an inpatient hospice. Inpatient hospice also provides short-term admission service for relief of distressing symptoms and / or during family crisis. If the patient who has cancer or some terminal illness, he can be referred by doctors from hospitals, health institutions, community hospitals, polyclinics and clinics that they visit. With more people living to old age and developing chronic impairments associated with advanced age, more families will need to provide care for their senior relatives at home. A caregiver is someone who has to take care of another person who is unable to live independently because of a mental or physical impairment. 57 Support for Seniors & Caregivers Community Case Management Services Community Case Management Service (CCMS) is a service provided by professional case managers who are either social workers or nurses. Community Case Managers are trained to assess the needs of the older person and to find the appropriate services to support them and their caregivers. The main objectives of CCMS are to: Help the older person to remain active in the community Reduce the stress of the caregiver and Prevent the older person from premature institutionalization If an older person (60 years and above) has multiple medical, physical and social needs and require assistance from community support, s/he will probably benefit from CCMS. 66 Assistance Schemes for Seniors Means Testing Means testing is compulsory for Singapore Citizens or Permanent Residents who require government subsidies from the government. Means Testing is a method to calculate the government subsidies that an older person will get if he requires some form of intermediate and long-term care services. It takes into consideration the following: Gross income (before CPF deductions) of the older person, his spouse and his children Siblings who live with him Ownership of major assets such as private property of older person and spouse Older Person s rental income, insurance claims, savings (e.g. fixed deposit, hard cash) and other sources of income (e.g. shares, dividends) Applying for Subsidy If the older person requires government subsidies, he can approach the staff from the hospital, nursing/ sheltered home, community hospital, hospice, day care centres (rehabilitation, social and dementia), home help service or community service provider. The following steps will be required for means testing: Step 1 - Prepare the necessary documents for submission Applicant s identity card All documents indicating all sources of income including wages and rentals (before CPF deduction) of applicant, spouse, children and / or siblings staying at the same address (bullet 2 & 3 are the same) Other relevant documents such as letter of adoption, retrenchment letters and Medisave Statements Step 2 Submission of Documents The family can submit these documents to the staff who is conducting the means testing for the applicant. The staff may be a medical social worker from restructured hospitals and national centres, administrator from a nursing home, community hospital, day care centres and other community service providers. Step 3 - Notification of the Outcome of Means Testing Once all the documents are collected, the subsidy level can then be calculated. If the older person qualifies for subsidies, the service provider will receive the subsidy directly from the government. This amount should be reflected in the bill that is given to the applicant. 67 Assistance Schemes for Seniors Eldershield ElderShield is an opt-out scheme for all Medisave account holders who are Singapore Citizens or Permanent Residents. Those who reach age 40 and who have Medisave accounts will be automatically covered under ElderShield if they do not opt out. 68 Assistance Schemes for Seniors Interim Disability Assistance Programme for the Elderly (IDAPE) IDAPE is a government assistance scheme to provide financial help to needy Singapore citizens who become disabled and need some cash to cope with expenses. IDAPE is for Singapore citizens who are not eligible for ElderShield because of their age or pre-existing disabilities. IDAPE provides $100 or $150 a month, up to 60 months, per life time. To make a claim under IDAPE, one must satisfy these criteria: A Singapore citizen aged 70 and above as at 30 September 2002 Or Unable to perform 3 or more of the 6 Activities of Daily Living or ADLs (washing, feeding, dressing, toileting, mobility and transferring). 69 Assistance Schemes for Seniors Primary Care Partnership Scheme The Primary Care Partnership Scheme (PCPS) was launched to help needy elderly Singaporeans to receive medical and dental services from private general practitioners at affordable prices. To qualify for the PCPS, the applicant must be either: A Singapore citizen who is 65 years old and above with a per capita monthly income of $700 and below and holds a Community Medical Benefits Card (CMBC) issued by a Community Development Council (CDC) or A person who is currently under the Public Assistance (PA) scheme, regardless of age. 114 Glossary Befrienders are volunteers who provide regular emotional support to vulnerable seniors, who are 60 years and above, have limited or no family support and at risk of social isolation due to physical, health, psycho-emotional or sociological reasons. Chronic Sick Facilities are specialized nursing homes that provide care to the older persons with complex medical conditions that require a greater level of care and supervision. Community Case Management Services are provided by professional case managers who assess the needs of the older persons, provide support to caregivers and coordinate an array of services to meet the needs of the older persons and their families. Community Hospitals are intermediate care facilities between the acute hospital and community-based healthcare facilities, providing continuation of care during the period of recovery from a health-related problem. Patients who require such care are usually those with stroke, lower limb amputations or hip fractures. Counseling Services help the older persons and their families resolve personal and family challenges. Day Care Centres provide supportive care to seniors who need supervision in a supportive environment when their family members are at work or require respite from caring. They also provide programmes which help the elderly maximize their physical functions and offer opportunities for them to socialize and engage in meaningful activities so that they can lead physically, socially and mentally active lives. Day Rehabilitation Centres provide centre-based rehabilitation services during the day, such as physiotherapy and occupational therapy, and cater to the older persons who suffer from medical conditions (such as stroke, fractures and lower limb amputations) that impair their functional status. The centres aim to restore, improve and / or prevent any deterioration of functional abilities of the older persons through exercise and training programmes. Dementia Day Care Centres provide day care programmes for the older persons who suffer from dementia. The centres teach and train these older persons and their caregivers, so that the person with dementia can care for himself as much as possible. Eldershield is a severe disability insurance scheme, designed to provide Singaporeans with basic financial protection against expenses required in the event of severe disabilities. Home Care Suppliers either sell or rent equipment for the care of the older persons at home, such as wheelchairs, walking aids, hospital beds, safety railings etc. Home Help Services provide a range of services to assist the older persons living in their own homes. These include meal delivery, housekeeping, transport and escort service to hospitals / clinics, personal hygiene and laundry services. G.01
115 Glossary Home Medical Services provide medical care to the homebound older persons through home visits at their own homes. Medical care includes comprehensive assessments, development of management problems, both acute and chronic, as well as referrals for specialist treatment where appropriate. Home Nursing Services provide nursing care for homebound or bedridden older persons who require nursing procedures. The home nurse will visit the patient at home to provide nursing care, such as wound dressing, injections, change of feeding tubes and urinary catheters, checking of blood pressure and blood sugar levels. Home Therapy provides home-based rehabilitative services (physiotherapy, occupational therapy or speech therapy) to clients who have rehabilitation potential but may have difficulty gaining access to centre-based services due to mobility limitations. Hospice Service is a specialized form of care designed to provide palliative care, physical and emotional comfort to patients and their families when a terminal illness no longer responds to treatment. Types of services available include home care, day care and inpatient care. Integrated Care Services is the centralised agency responsible for the handling of referrals to intermediate and long-term care facilities such as Nursing Homes and Chronic Sick Facilities. It also helps to integrate services to ensure a seamless flow of care from the hospitals to Community Hospitals, Day Rehabilitation Centres and / or Nursing Homes. Interim Disability Assistance Programme for the Elderly (IDAPE) is a government assistance scheme to provide financial help to needy Singapore citizens who become disabled and need some cash to cope with expenses. This is for Singapore citizens who are not eligible for ElderShield because of their age or pre-existing disabilities. Means Testing is a method to calculate the government subsidies that an older person will get if he requires some form of intermediate and long-term care services. Neighbourhood Links serve as neighbourhood focal points to engage senior residents and others in volunteer activities, promote inter-generational activities, and assist the residents in obtaining appropriate services to meet their needs. Nursing Homes provide care for persons with medical conditions requiring long-term daily nursing care. Their residents are without any caregivers to look after them at home or the caregiver is unable to provide the nursing care required. Some of these nursing homes provide long-term residential care for persons with dementia when the caregiver is unable to cope with the challenges posed by the illness. Respite Care offers the caregiver a short and temporary break from their caregiving duties. This may range from a few days to one or two weeks. Senior Activity Centres provide a support network for older residents in one-room rental HDB flats and offer a range of services including information and referral services, maintaining a register of older persons, forming mutual help groups and promoting social interactions. 116 Glossary Senior Citizens Clubs organize cultural, social, recreational, sports activities, as well as talks and other ad hoc events to encourage the older persons to lead active, healthy and meaningful lives. They also help enhance family relationships and promote intergenerational interaction between the elderly and members from other age groups. Sheltered Homes are residential arrangement catered to the needs of the ambulant elderly who are without next-of-kin or for certain reasons, unable to stay with family members. These Homes, located within HDB neighbourhoods, provide home-like living arrangements as well as support services to assist and enable seniors to maintain their independence within the community. Support Groups bring caregivers together to meet and provide mutual support. Caregivers are also trained to care for the older persons at home.
