17 THREE PROJECTS ONE GOAL SNAP CCAP IHA Using data to strengthen the safety net The members of the CCAP collaborative envision the proposed project as a bridge between multiple data initiatives the Safety Net Analytics Program (SNAP) from the Center for Care Innovations, and the Strengthening Performance Measurement in the Safety Net project from Integrated Healthcare Association (IHA). Through SNAP, health centers can learn to use and manage data, which can be shared with regional networks for validation, aggregation and analysis. The data can then be shared with IHA and other partners for greater statewide discussions regarding analytics, performance improvement and payment reform. SNAP ACTIVITIES CCAP ACTIVITIES IHA ACTIVITIES Clinic level Assess clinic capacity in regard to analytics Coach clinics on using and optimizing analytics tools Coach clinics on using data for performance improvement Regional Network level Validate and aggregate data Build capacity through learning sessions for clinics and networks Share data across networks Focus on clinical, operational and financial improvement goals Statewide Build IHA knowledge base related to safety net and Medi Cal Work with state initiatives on incorporation of safety net solutions Expand performance measurement and improvement activities
