1 SUPPLIER PREQUALIFICATION QUESTIONNAIRE FORM BRIEF This Supplier Pre Qualification Questionnaire is provided for the following project types of ALARGAN PROJECTS: 1) Supply of Construction Materials 2) Supply of Structural Materials 3) Supply of Finishing Materials 4) Services that use directly or indirectly in ALARGAN PROJECTS Company Projects. 5) Special Materials as per design criteria requirements. 6) Others In an effort to develop (Name of Project), we are taking steps to invite and communicate with Suppliers to participate in our program by developing information guidance and soliciting information for evaluating and establish Supplier s qualifications. ALARGAN PROJECTS requires interested bidders to establish proof of their competency and responsibility prior to submitting bids and has prepared a prequalification questionnaire as supplemental information for evaluating Suppliers. In order to become prequalified, the Supplier Pre Qualification Questionnaire Form, together with all of the required information as requested, must be submitted for review and succeed. 2 CHECK LIST You must be able to answer Yes to all of the below statements before you complete and submit your Pre Qualification Questionnaire and Financial Statements for review: I am interested in bidding on above mentioned ALARGAN PROJECTS. I am appropriately Registered and licensed in Saudi Arabia. I have either Compiled, Reviewed or Audited financial statements, dated within the last twelve (12) months. GENERAL INFORMATION AND INSTRUCTIONS IMPORTANT: PLEASE READ AND FOLLOW ALL INFORMATION & INSTRUCTIONS AND COMPLETE CAREFULLY. Suppliers bidding for ALARGAN PROJECTS, must be prequalified. In order to become prequalified, the Pre Qualification Questionnaire and required Financial Statements must be submitted for review and succeed. Once prequalified, Suppliers will retain that status for one (1) year following notification of qualification. A Supplier s status as a prequalified contractor may be automatically revoked for misrepresentation or falsification in the information submitted in the prequalification statement. Submittal of this questionnaire specifically authorizes ALARGAN PROJECTS to investigate or cause investigation of any and all statements made in this questionnaire and to use the information obtained in such investigation for reviewing and scoring the responses. All consult must answer all questions, fill in all blanks and provide the required references. If a particular question does not apply, the response must state Not Applicable (NA). False statements and /or omissions will disqualify Supplier from prequalifying, and other infractions as stated in this questionnaire may lead to suspension and subsequent termination of existing prequalification status and contracts with the ALARGAN PROJECTS. The Supplier must verify accuracy of answers in this questionnaire by affixing the signature of a person authorized to sign on behalf of the Supplier. 3 THE PROCESS OF PRE QUALIFICATION 1. Supplier will complete and submit the Pre qualification Questionnaire form, along with appropriate information / statements / references. 2. ALARGAN PROJECTS staff will verify the information submitted in the questionnaire and statements. 3. ALARGAN PROJECTS staff will use standardized rating criteria to evaluate and score the questionnaire and will determine the Supplier s maximum bidding capacity. 4. Once pre qualified, Supplier will have one year eligibility to bid on ALARGAN PROJECTS Company various projects. 5. To continue eligibility bidding status annually, renewal of prequalified status is required. PRE QUALIFICATION REQUIREMENTS QUESTIONNAIRE Supplier must provide and meet approval on the following items: 1. Complete and submit the most current revision of the ALARGAN PROJECTS pre qualification questionnaire. 2. Have a current and active Saudi Arabia Supplier s license. 3. Meet General Qualification requirements. 4. Maintain pre qualification requirements. The questionnaire will be thoroughly evaluated by ALARGAN PROJECTS, taking into consideration the following factors: 1. Experience 2. Quality and workmanship 3. Delivery Time 4. Timeliness of past performance 5. Financial / Bonding resources to determine category and maximum project range 6. Licensing and Classifications 7. Trustworthiness, reliability and responsibility 8. Compliance with ALARGAN PROJECTS requirements (including technical, administrative, managerial and engineering experience). Above mentioned factors will be the basis of rating Suppliers with respect to the size and scope of contracts for which Suppliers are qualified to bid. All references supplied with this pre qualification request MUST be equal to the size and type of the projects that the Supplier wishes to bid on for the ALARGAN PROJECTS Project s and must reflect the type and size project that has previously been bonded by the Supplier. 4 ALARGAN PROJECT Company reserves the right to check other sources available. Omission of requested information can result in automatic disqualification. ALARGAN PROJECTS will apply a uniform system of rating Suppliers on the basis of adequately experienced, equipped and financed to meet their contractual obligations at the completion of previous each works projects. It is the responsibility of the Suppliers to ensure that they provide sufficient information about their firm and their Sub Suppliers to meet the prequalification requirements including experience and other things deemed necessary to determine their capabilities and receive a fair evaluation. Supplier MUST provide a reviewed or audited financial statement prepared by a licensed accountant. PLEASE BE ADVISED that in some instances where ALARGAN PROJECTS determines that there are substantial performance and/or other critical issues with a project, such as technicality and duration, a PRELIMINARY EVALUATION may be conducted during the course of bidding that project PRIOR to its awarding of such contract, and from that evaluation appropriate action will be taken. RENEWAL OF PRE QUALIFICATION Prequalification must be renewed yearly. It is the responsibility of each prequalified Supplier to submit a Prequalification Renewal Questionnaire at least thirty (30) days prior to expiration of the one year period so that prequalification status does not lapse. If the Renewal Questionnaire is not received at least 30 days prior to the expiration date of the existing prequalification, then the entire regular Prequalification Questionnaire must be submitted. This is an official document that is used to determine the qualifications of Suppliers to bid with the projects of ALARGAN PROJECTS Company. Any alternations of this document or fail to provide answers to all questions in complete will make your questionnaire invalid and it will not be accepted by the office of evaluation.
