Hayley Hemming (HH) - Heart of England Mencap 1. Introductions EC introduced herself as a Lead Commissioning Manager, chairing the meeting. All attendees introduced themselves and signed the attendance register. 2. Minutes of meeting held on 19 th January 2015 Minutes of the previous meeting were agreed. 3. Provider feedback from Pre Meet One member raised concerns around The Young Adults Team not feeding information through to Providers, about how many people are coming through the system who will require support/placements. Elaine suggested Jonathan Monk; the YAT Manager is invited to the next meeting to speak. Action: WW to invite Jonathan Monk to next Provider Forum. Page 4

With upcoming Care Act could really do with some detailed information on how this will affect services in Worcestershire. Elaine confirmed that Melanie is currently organizing 2 sessions for April, WW will confirm date ASAP. Action: WW to bring details of future training courses to next meeting. EC mentioned that there were 3 or 4 sessions carried out late last year, Providers would like to know who attended these previous Care Act training sessions as no one from LD seems to have been invited. MT to investigate and confirm. Action: WW to establish details of who attended the previous course. Elaine also confirmed to the group that there is other training available to Providers; we will circulate the link to this. Action: WW to investigate and bring details to next meeting A member raised concern over the new website and the authorization levels 1-3, especially for day opportunities, seems to be some confusion over what we were told at event and what is happening on the system. The group discussed, this could cause major issues especially for smaller organizations. EC will seek a response from Charles Huntington and Contracts Team on this and contact the forum via to confirm. Action: EC and WW to speak with Charles Huntington and Contracts and bring a response to next meeting. A member asked if the group could be provided with a flow diagram of the referral process and how people move through the system? Elaine explained that ways of working will be changing quite quickly from April. Elaine suggested inviting a WCC rep to the group to speak about how Social Workers will work going forwards. The group agreed this would be beneficial. WW will invite rep, possibly Liz Staples. Action: WW to invite Liz Staples or a suitable representative to the next meeting. 4. CLDN nurses presentation on service Saffron Box delivered a presentation on the CLDN team Saffron ran through the wide range of referrals the teams can receive. If anyone needs to send something to the Nurses team to look into then they can use the referral form, this will be sent round with the minutes. Responses are usually quite quick and are prioritized by urgency. If anyone needs any help or advice with anything then the team welcomes contact via phone or referral form. Elaine updated the group on what commissioning is doing to improve annual health checks Page 5

LD Strategy update Elaine presented the group with the final copy of the LD strategy, which has been shared with the group several times whilst in draft. This has been sent to be printed and will be uploaded to the website and sent to this group. Action: WW to upload a copy of the LD strategy to the forum webpage and send as an to the group. Commissioners within The ICU are leading on different sections of the strategy. Elaine informed the group that WCC are expecting a 50% increase in requests for assessments from April. A market position statement will be the next thing for Commissioners to work on producing. 6. Update on Day Services Elaine informed the group that the proposals for Worcester and Malvern are going through today, these now have a 5 day call in period before they can be passed. Cllr Sheila Blagg and Cllr Marcus Hart have been given delegated responsibility from Cabinet to make the decisions. A design group will be setup for the new room at The CUBE in Malvern. Elaine wanted to make Providers aware of likely changes to transport and to prepare for probable changes. Going forward WCC providing transport as it does now looks unlikely with offers being expensive and unstainable. The money instead will be put into people's personal budgets for them to purchase from a chosen provider. Graham made the group aware that from his experience providing transport can get complicated and to talk to insurance companies. 7. Supported Living update Elaine updated the group on supported living, WCC have a target to move 200 service users into Supported Living. There will be a tender on the e-portal soon for small pots of capital money to convert existing buildings or build new ones to create more supported living in Worcestershire, please keep an eye on the portal if you are interested. There is a team of 3 Social Workers within ICU working on moving Older People into Extra Care. There are also 4 Social Workers in The YAT looking at supported living as an option for people. Elaine informed the group that WCC have had their first parent and child placement, this was for an elderly mother and her daughter with LD who she cares for, they have moved into Meadow Court which is an extra care facility, this is a great scenario where both can get the appropriate care whilst still being independent. Provider feedback from Partnership Board Elaine informed the group that David Broadbent has resigned from The Learning Disability Partnership Board as a rep for the Providers. Graham confirmed that he was still a rep and that there was space to have 3 Provider reps on the group. EC asked the group to come up and agree a new rep and to let Will know who these are; she recommended that Providers from different sectors are represented to give a broader knowledge base. 9. Any other Business Elaine noted that LD banding rates for care are currently being worked on; this should tell people what they would expect to pay for care, with and without nursing etc. The final document will be shared with the group, WW to circulate. Action: WW to circulate LD banding rates document to group. It was agreed that the minutes for the Partnership Board and its sub groups would be uploaded to the website for this group to see, along with a list of who are the Provider reps on each group.
