1 PATIENT CARE SERVICES POLICIES AND PROCEDURES TITLE: Precepted Nursing Student Policy NC EFFECTIVE DATE: December, 2011 PREVIOUS REVISIONS: APPROVAL SIGNATURE: Jennifer Jackson, RN, MSN, CNRN Interim Chief Nursing Officer Vice President, Patient Care Services REVIEWED/REVISED by: Patient Care Services Policy and Procedure Steering Committee RESPONSIBILITY: Patient Care Services Staff PURPOSE To direct the use of clinical facilities in order to ensure both optimum patient care and a nursing care environment conducive to learning for a precepted experience. AREAS AFFECTED All patient care areas and all schools of nursing that utilize University Hospital (UH) clinical facilities. POLICY I. Nursing, as a part of Patient Care Services, supports the University Hospital mission of service, research and education. The primary focus of Patient Care Services is to ensure the continuity of comprehensive quality nursing care in varied settings under the direction of professional nurses. The provision of learning opportunities for nursing students is considered an important obligation of Patient Care Services and a professional responsibility for all registered nurses. II. Guidelines for all Inpatient/Outpatient services: A. The Clinical Education Supervisor coordinates all precepted experiences for undergraduate and graduate nursing students. Faculty at a School of Nursing must contact Clinical Education Supervisor in order to request preceptor placement. At no time should a faculty, students, or UH associates contact staff to coordinate preceptor placement. Assignments for students are discussed and confirmed with the appropriate faculty, manager/educator and preceptor. 1. Preference in assignment of precepted experiences is given to students enrolled at the University of Cincinnati nursing programs. All other requests for student precepted experiences will be considered at the discretion of the PAGE 1 of 3
2 Clinical Education Supervisor. 2. Changes to student preceptors must be approved in advanced by the Clinical Education Supervisor. 3. The college/school will make available on request documentation of compliance with Clinical Education Agreement requirements. 4. Faculty and/or student will provide course objectives to Clinical Education Supervisor upon request. 5. For observational experiences, refer to Observation of Nursing Care policy. B. Students are required to adhere to the policies of University Hospital, the Patient Care Services Department and the particular area where they are assigned. Student practice is expected to remain within the scope of the clinical course as outlined in the syllabus for that course. Special attention must be paid to the UH Chain of Command policy. Any student found to be unsafe in the clinical area or to violate hospital policy will be asked to leave and the student s faculty will be notified. The student will not be able to return until the student can demonstrate to UH that they no longer present a threat to the safety of patients, staff or liability to the institution. Following an evaluation by the Clinical Education Supervisor and at the recommendation of the manager and/or Nursing Administration the learning experience may be terminated. C. Student Prerequisites for access to clinical areas 1. Before providing any patient care, students will provide proof of understanding standard precautions, transmission-based protocol and safety policies. Students must pass UH Safety Training Annual Review (STAR) manual test and the National Patient Safety Goals test with a grade of 100%. Students must read the Precepted Nursing Student policy and sign the Student Performance Agreement. These documents must be turned into the Clinical Education Supervisor prior to the student clinical experience. 2. Licensure: It is the responsibility of the affiliating school, college or clinical program to verify proof of licensure for any nursing faculty or licensed students needing precepted experiences. 3. The college/school will verify all students are current in Basic Life Support and Advanced Cardiac Life Support when required. 4. Professional behavior on the part of student is expected at all times while in the hospital. D. Clinical Supervision of precepted students 1. The student must be supervised by the assigned preceptor at all times. Faculty responsible for the student must be immediately available (e.g. by cell phone) while students are completing their clinical experience. a) Students will not be permitted to give patient care without preceptor supervision. b) Students who are also associates of UC Health may only function in a student capacity when at UH during academic clinical experiences. Students may not access patient information needed for their student role (e.g. glucometers, HealthBridge, Lastword, Access Anywhere etc.) while in their UC Health associate role. E. Identification Badges/Parking 1. Schools of Nursing must provide the Clinical Education Supervisor all student information two weeks prior to the start of the clinical experience. Page 2 of 5
3 Schools of Nursing should contact the Clinical Education Supervisor for required student information. 2. Students must wear a University Hospital student identification badge in addition to their school badge at all times when in the hospital. 3. Parking is ONLY available in specific designated areas. Parking in undesignated areas may result in the placement of a parking immobilizer on illegally parked vehicles. The school must contact the Clinical Education Supervisor for updated information as it may change at the hospital s discretion. 4. Student badges and parking passes should be returned to the Clinical Education Supervisor at the end of the clinical experience. F. Patient Information 1. Students are expected to follow HIPAA regulations with regard to all patient information. UH will terminate clinical experiences for those involved in incidents involving failure to follow HIPAA regulations. G. Student Expectations 1. The student is expected to notify their preceptor if they will be absent for a clinical experience. 2. Students must be in school uniform (including their University Hospital AND school/college ID badge) or white uniform and comply with UH dress code when providing/supervising patient care. The students are expected to review the dress code policy (including tattoos and piercings) prior to the clinical experience 3. If the student is working in an area where hand off of care report is utilized it is the expectation that the student be present at scheduled patient report times. I. Patient Care Activities 1. The preceptor is responsible for all student activities according to hospital policy. All student documentation must be cosigned by the preceptor. The preceptor must follow chain of command (per hospital policy) and the OBN laws and rules in addition to the hospital policies when any question regarding patient care arises. (e.g. clarification of a physician order, any conflicting or confusing information regarding patient medications or treatments, etc.) 2. The manager or their designee has ultimate responsibility for nursing care quality at the department level. She/he or designee is expected to be available to the students to address patient care issues. 3. The nurse assigned to the patient is ultimately responsible for the care of patient. 4. Precepted Student Responsibility a) Nursing students are encouraged to provide input into patient's Plan of Care. However, any proposed major revisions of the plan of care made by a student must be verbally communicated to and approved by the preceptor prior to documentation of changes to the plan of care and implementation of the revisions. b) Students will give report to the preceptor including patient care provided and patient status before leaving the department for any reason. 4 c) Students are expected to document (including signature and initials) all activities regarding their patient care under the guidance of the preceptor. Included with their signature must be the name of their school of nursing. d) Students who take a patient off the unit for unscheduled activities (such as walks) must follow UH policy and clear this activity with the assigned staff nurse. Students may accompany a patient to X-ray, Physical Therapy, and other scheduled activities per policy. 5. Errors or incidents involving patients or visitors must be reported immediately to the manager or designee and assigned nurse. After assuring patient safety, the student will work with the manager or designee to file an incident report. Prior to the leaving for the day the error or incident should be reported to the Clinical Education Supervisor. 6. Faculty or students injured during a clinical experience at UH should seek medical assistance from the Center for Emergency Care (policy # III-208). J. Student Property 1. The hospital is not responsible for loss or damage to student property. a) Students are advised to bring as few valuables/property as possible and only enough money for breaks and transportation. b) Students are instructed not to leave valuables/belongings within view in their cars if parking on UH property. K. Personal Electronic Device Usage 1. Personal electronic devices i.e. personal cell phones, PDAs, IPods, etc., must be turned off during clinical hours except during breaks and scheduled meal times. Accessories identified for use with personal electronic devices, i.e. hands free devices, ear phones, etc., shall not be worn or used during clinical hours. 2. Use of personal electronic devices is limited to break areas, as identified by department, and the outside perimeters of the hospital. Personal electronic devices should not be used while transiting the corridors of the hospital or in public waiting areas of the hospital. 3. At no time is personal cell phone use, including text messaging, permitted by students in view of patients. 4. At no time are students permitted to use personal electronic devices for the purposes of taking pictures of /video taping patients. L. No Smoking Environment: University Hospital Medical Center is a smoke-free environment. Students/Faculty are expected to follow/support the UH Tobacco Free Campus Policy.
