Discuss in detail about scientific management theory. 5. Case study. MANAGING THE OUTPATIENT CLINIC You are the manager of an outpatient clinic in a large hospital. The hospital s management has always been searching for ways of increasing efficiency. They have recently installed a new computerized record keeping and billing system, but to the surprise of everyone, including yourself, the expected increase in efficiency was not realised. In fact, efficiency and effectiveness have both fallen off, (i.e., lost records, late billings, incorrect charge of) and the extent of employee turnover has risen. You do not believe there is anything wrong with the system. You have had reports from other hospitals who are using it and they confirm this opinion. You have also had representatives from the firm that built and installed the system and machines, go over them and they report that they are operating at peak efficiency.
3 Among your immediate subordinates (three first line supervisors, each in charge of a shift, and your administrative assistant), the drop in production has been variously attributed to poor training of the operators, lack of an adequate system of financial incentives and poor morale. Clearly, this is an issue about which there is no time to lose in rectification. The hospital management would like to know within a week what steps you plan to take to tackle the situation. You share your manager s concern with the falling productivity and know that your subordinates are also concerned. The problem is to decide what steps to take to rectify the situation. Low costs, combined with excellent facilities, have provided the perfect formula for India to become a major player in the $2.8 trillion worldwide healthcare industry. Today the cost of healthcare in India is only one-tenth the cost in the US in cases of major surgeries. With cost of providing healthcare low, and with global standards available, the world cannot compete with US The cost of a heart surgery at Apollo is $2,500 as compared to $ 30,000 in the US. Bone marrow transplants cost $ 50,000 in India as against $ 4,00,000 in US. India s attraction as a low cost, high-quality centre for healthcare may be new but the signs are visible. In , Apollo Hospitals conducted heart surgeries on 91 patients
6 from Tanzania. In Chennai, Apollo treats around 30 Sri Lankans a day. As many as 40 patients from Muscat are registered in Apollo hospitals and clinics in Delhi, Chennai and Hyderabad Healthcare not only brings in direct purchase of health care, but it also encourages a lot of expenditure, on travel, lodging and boarding. Which, translated, means foreign exchange And India has enough Embassies and Tourism Development Corporations to spread the message. However government polices, as they stand, are not conducive to more private participation in healthcare, when compared to neighbouring countries Sri Lanka, offers a 100% customs duty exemption on all equipment for the hospital, medical or otherwise. And if the project cost were to exceed Rs. 100 crore, the hospital would be eligible for a 12-year tax holiday However such elaborate incentives are not necessary, though some basic incentives need to be provided. The government does not provide any money to this sector, and it should not, it should act as a facilitator Lot of foreign investors are keen on investing here, but are shying due to the lack of tax and other incentives. The city of Denverput the opening of Denver International Airport, the nations first major airport to open since Dalls Fort Worth Airport in 1974, on hold for months as the nations largest airfield struggled to resolve problems with a 20 mile - long computerized baggage -handling system that had mangled its cargo. The airport, which replaced Denver s Stapleton airport, opened behind schedule by many months and each day of delay added another $ 500,000 to the cost of the $ 3 billion project for Denver tax payers.
9 The PC controlled baggage system is outside the purview the airport s IS department, because the entire airport has a decentralized computing environment. Each functional unit of the airport, such as security, neating, ventilation facilities management, and the individual airlines, has its on computer system, operated by sub contractors in most cases. The central Fiber Distributed Data Interface (FDDI) network itself is operated by the airport s IS department, which also supports PC LANS and work stations on the network and maintains the central servers. The airport choose a client - server environment that supports specialized functions such as security and flight management. The automated baggage system is based on PCs, proprietary applications software, and a multitasking operating system thought to be IBM s OS/2. The systems are intended to speed turn a round time by routing bags automatically among airlines. Laser scanners route the baggage carts on underground tracks, directing each piece of baggage by reading its bar - coded label. There is only one piece of luggage per cart; as many as 60 carts per second traverse a single track. The automatic baggage - handling system, which was once touted as the world s best, has become a classic case of project - failure; power fluctuations, mechanical glitenes, and software problems are cited as some of the primary reasons for this debacle. Another reason cited by experts is that Denver has pushed the technology to its limit. The project was too ambitions it involved a number of leading - edge technologies, and integrating these technologies has been a major challenge and a primary course of system failure. Although the baggage system is similar to the company installed in San Francisco s United Airlines terminal, it is 10 times larger, with PC consoles in nine control rooms tracking bags throughout the airport on some 4,000 automated carts. Recently, United Airlines was hired as the systems integrator and given the responsibility of solving the problem. However, negotiations between the city of Denver and BAE automated systems, which built the computerized baggage system, must be completed before the project can begin. Denver is attempting to recover more than $ 80
10 million for BAE failure to deliver a working system. BAE has demanded $ 40 million in payment for additional personnel and equipment it used during the delays. BAE devies that computer problems prevented the baggage system from working; the firm alleges that last - minute changes to the original plans, made by the city caused the delay. Experts at united believe tht the sheer size and complexity of the system is the reason for its catastrophic failure. United wants to simplify the system and reduce the workload ofthe individual computers that track luggage on the 4,000 one - bag carts. Denver International s baggage system was modeled on an integrated system used in Frankfurt. But the Frankfurt system uses conveyors to carry bags instead of the 4,000 PC driven automated carts used at Denver International. United is suggesting additional tracks and fewer carts per track. It s just like adding another lane onto a highway, says a spokes person. (a) (b) Identify some of the problems with the automatic baggage system. Taking into consideration above developments with regard to hospital planning and establishment, if you are appointed as an architect for planning the establishment of hospital in the city area, design a plan for the establishment by highlighting the factors which you consider to be the most important.
