Program Coordinator: Brain Injury Day Program The Opportunity Centre 2003 to 2004 Developed and implemented a comprehensive adult day program d The Opportunity Centre and facilitate community re-entry for individuals who had sustained severe-moderate brain injuries in partnership with the Brain Injury Association of Waterloo Wellington. The Opportunity Centre Day Program grew from 10 regular participants to well over 100 brain injured adults attending regularly within a given month. The day program model developed is considered a template for brain injury province wide. Hired, trained, and supervised Life Skills Trainers, Brain Injury Outreach Workers and Attendant Care Workers for the Brain Injury Day Program and Brain Injury Outreach Program. Assessed clients for intake in order to identify support needs for attendance at The Opportunity Centre.
5 Jean Taylor 5 Oversaw the development, funding, and implementation of activities and events including music programming, martial arts, wood burning, crafts, gardening, pottery, and caregiver support. Life Skills Trainer Brain Injury Residential Program 1993 to 1995 Developed and assessed skills based retraining programs to facilitate community re-entry for individuals who had sustained severe-moderate brain injuries in a supported independent living environment. Planned and implemented programs combining strategies and graduated support, designed to address the brain injured individual's memory, attention, communication, physical, social and emotional issues within the context of his/her home, school, paid/unpaid work, and community settings such as banks, stores, and leisure events. Facilitated communication between involved professionals, lawyers, physicians, therapists, educators, vocational supervisors, service providers and family with the individual. Co-ordinated annual case conferences and maintained communications. Provided reports to reflect functional status, outcomes and goals. Oversaw the community volunteer aspects of the Brain Injury Program. Educated and trained attendant care staff, volunteers and family members regarding the effect of brain injuries and applied strategies.
