1 Before completing this form, you must read the VET FEE-HELP information booklet, available at You must: complete this form if you are requesting for all or part of your VET tuition fees for your VET course(s) of study; ensure that you complete each question that is relevant to you, including ticking the relevant boxes. Your form will not be valid unless each question is completed correctly; and return the completed form to the student administration/enrolments office at your VET provider (provider) on or before the census date for the first unit(s) of study for which you are requesting VET FEE HELP assistance. You will be provided with a copy of this form for your records. IMPTANT: you will only be entitled to for a particular VET unit of study if you remain eligible and have sufficient FEE-HELP balance to cover your tuition fees; it is your responsibility to be aware of your FEE HELP balance and you must advise your provider if you do not have enough FEE-HELP balance to cover your VET tuition fees; and your provider may set an earlier date for the submission of the form check with your provider /13

Your provider is authorised under the Higher Education Support Act 2003 (the Act) to collect your TFN. You will not be able to obtain for the amount of your VET tuition fees that remain unpaid on the census date unless you give your provider, on or before the census date, either your TFN or the certificate from the Australian Taxation Office (ATO), stating that you have applied for a TFN. te: as TFNs are usually provided within 28 days of application, it is essential that you apply early to ensure you have your TFN on or before the census date. For more information on requesting a TFN visit or phone between 8.00am and 6.00pm (EST), Monday to Friday. 3 10. Your obligations: I have read the VET FEE-HELP information booklet and I am aware of my obligations if I receive. I understand, over my lifetime, the amount of VET FEE-HELP assistance (and FEE-HELP assistance) that I access must not exceed the FEE-HELP limit. I understand that it is my responsibility to be aware of my FEE-HELP balance and to advise my provider/s (and FEE-HELP providers if applicable) if I am approaching the FEE-HELP limit and may not have enough FEE-HELP balance to cover my tuition fees. In this situation, I will advise each relevant provider of how much VET FEE-HELP (or FEE-HELP) assistance I wish to access for my units of study. I understand the statements in item 11 relating to repayments of VET FEE-HELP debts (and refunds under special circumstances). I understand that under the Act, the term Commonwealth supported student refers to a student who is enrolled in a Commonwealth supported place a subsidised higher education place (visit for more information). A student accessing VET FEE-HELP (or FEE-HELP) is not a Commonwealth supported student.
