Critical Illness can be selected through my Aviva flex online or over the phone during the annual flex enrolment window. New starters at Aviva have an opportunity to select when they join Aviva. flex window or in connection with certain life events (details found at Critical Illness cover can only be purchased as an optional benefit to Term Assurance or Mortgage Life Insurance products (it can t be purchased as a stand alone product). You can purchase integrated Critical Illness cover on your Term Assurance policy (meaning if you claim on your Critical Illness cover, the policy comes to an end). If you purchase Critical Illness cover on you Mortgage Life Insurance product, it can be integrated cover (policy comes to an end if you claim on your Critical Illness cover) or independent cover (if you claim on your Critical Illness cover, the life cover will continue) or both in which case, a claim for CI would mean two pay outs and the policy would end. It can be applied for at anytime of the year over the phone via the my Aviva one stop shop. Funded monthly through salary sacrifice reduction to your gross pay. Payable monthly or annually by direct debit. As a group scheme, the cost of the cover to you is based on a collective scheme rate rather than an individually underwritten rate. The premium is determined by your age and the level of cover you select regardless of your individual claims experience and depending on your circumstance may be cheaper than individually underwritten cover. The monthly salary reduction is made before tax and national insurance (NI) are deducted. You save NI on the cost of the cover. However as the benefit is taxable through the P11D process ultimately there is no tax saving. Depending on your circumstances, being individually underwritten may be cheaper than the group scheme. Receive up to 30% employee discount (minimum premium applies) available for all current Aviva employees. This discount will apply for the full term of your policy. Price matching is available on any like for like quote on any mortgage life insurance or term assurance (both with optional critical illness cover) that you receive elsewhere. You can select cover in units of 25,000 up to a maximum of 5 x salary or 250,000, whichever is lower. 38 conditions are currently covered by this product; please see my Aviva flex site for more info. The maximum amount of Critical Illness cover available is 2,000,000, but there is no minimum amount of cover it is just subject to a minimum premium. 57 conditions are currently covered by this product; please see my Aviva products site for more info. You can cover your partner the cover available is from 25,000 up to a maximum of 250,000 (in 25,000 units), however their level of cover cannot exceed your own level of cover. Critical Illness for your children is included in the premium up to 25% of your own cover subject to a maximum limit of 20,000 per child. Joint cover is available for you and your partner. Your children are automatically covered by the critical illness benefit. We will pay the lower of 25,000 or 50% of the critical illness amount per child. The policy also provides a child death benefit which pays out 5,000 if your child was to unfortunately die. Critical Illness through flex is a Group scheme, so there is no individual underwriting. Depending on your personal circumstances, you may need to be individually underwritten. The cost of your monthly payment is determined by your age and the level of cover you select. at the beginning of the new scheme year. Premiums will increase at this point if you or your partner move into a new age band. and level of cover you select. Your monthly payment is guaranteed to stay the same throughout your policy unless you choose reviewable premiums where your premium amount can go up or down during the term of your policy. Price matching is available on any like for like quote on any mortgage life insurance or term assurance (both with optional critical illness cover) that you receive elsewhere. You can select cover up to a maximum of 250,000 in units of 50,000. For decreasing or level Life Insurance you can select any level of cover up to maximum of 500,000. For Term Assurance and Mortgage Life Insurance the life cover is unlimited. Terminal Illness Benefit (which is different to the critical illness option) is included with several of our life insurance policies. This means we ll pay you the sum insured if you re diagnosed with a terminal illness and are not expected to live more than 12 months. If we do pay out after you make a claim for terminal illness, your plan will end. Can I cover my partner? You can include cover for you partner: up to a maximum of 250,000 in units of 50,000. However, this cannot be of a higher value than your own cover. Your partner will need to complete a paper health declaration to obtain cover. Joint cover is available for you and your partner. Life insurance through flex is a Group scheme so there is no automatic individual underwriting, however you are required to meet our actively at work conditions. You will be required to satisfactorily complete a short online health declaration if you want cover in excess of 100,000. Your partner will need to satisfactorily complete a short paper health declaration to obtain their cover. We do not impose any standard pre-existing condition exclusions. There are 3 levels of private medical benefit cover available through my Aviva flex: Comprehensive Plus Comprehensive Essential An excess of 150 per claim applies for all levels of cover. Full details can be found on the flex site This is a flexible product giving you the ability to choose from a range of options; more details can be found at the my Aviva one stop shop on Aviva World. Self and partner, single parent and family cover available. Includes all children up to age 21, and up to age 24 if they are in full time education. Families can benefit from an offer of paying for the eldest member under the age of 20 and all other members under 20 are free. (This will only apply if there is at least one member age 20 or over on the policy). Dependants are able to stay on the policy as long as you want, with no cut off ages. Private medical benefit through flex is a Group scheme, so there is no individual underwriting. Pre-existing health conditions are covered under the Comprehensive Plus level of cover, but excluded under Comprehensive and Essential. This is an individual policy, therefore will be underwritten based on your personal circumstances. The cost of your monthly payment is determined by the level of cover you select and whether you include family dependents in your cover. You can find out the cost to you on the flex system. This cost is guaranteed for the scheme year (1st July to 30th June). Premiums are based on your personal circumstances and level of cover selected. Your individual policy will include a no claim discount which will increase or decrease each year, depending on claims made. Your monthly payment will stay the same during the 12 months of the policy. Any changes to your monthly/annual payment will only take effect at your renewal date. Cover will cease if you leave employment at Aviva, although preferential continuation terms are available up to 45 days after you leave the scheme. Cover will be unaffected by your employment status with Aviva. You will no longer be eligible for the employee discount, which will stop at the next annual renewal.

Travel cover can be selected through my Aviva flex online or over the phone during the annual flex window. There are 4 levels of annual multi-trip cover available; Worldwide cover with winter sports Worldwide cover excluding winter sports European only including winter sports European only excluding winter sports Standard terms and cover applies - no additional bespoke cover options available. Medical exclusions apply. Both single trip and annual multi trip cover is available. Standard terms apply. Bespoke cover options are also available; e.g. baggage or Airspace Closure. Travel insurance through my Aviva flex is a Group scheme, so there is no individual underwriting. Individual product therefore cover will be individually underwritten. The cost to you is determined by the level of cover you select and whether you include family members in your cover. You can find out the cost to you on the flex system. This cost is guaranteed for the scheme year (1st July 2014 to 30th June 2015) but may increase at the beginning of the new scheme year. and type of cover you select.
