4 SENIOR HOUSING Assisted Living Assisted Living Residences or Assisted Living Facilities (ALFs) are housing facilities for seniors needing assistance but don t require the level of care provided by a nursing home. ALFs provide help in the monitoring of resident activities to help to ensure the health, safety, and well-being of residents. Nursing Home PAGE 6 PAGE 10 Nursing Homes or Skilled Nursing Facilities are a place of residence for people who require constant nursing care and have significant deficiencies with activities of daily living. Nursing aides and skilled nurses are usually available 24 hours a day. Memory Care PAGE 22 Memory Care is a long-term care option for patients who have been diagnosed with Alzheimer s or other memory-related impairments. Memory Care environments are designed for persons with a level of impairment that makes it unsafe for them to continue living at home. Memory care allows a person experiencing memory loss to maintain a level of independence, but relies on the safety and security of living in a facility with a trained staff. Short-Term Rehab. PAGE 14 Short-Term Rehabilitation provides crucial rehabilitation services following surgery, illness or injury. The primary goal is to get patients back on track and into their daily routines as quickly as possible. Short-term programs specialize in treating patients who have experienced cardiovascular, neurological or orthopedic disorders such as strokes, hip fractures or other debilitating medical conditions. Residential Care PAGE 18 Home Health Care and Personal Care helps seniors live independently for as long as possible given the limits of their medical condition. It covers a wide range of services and often delays the need to move into a long-term care facility. Home Health Care is most often medically oriented, whereas Personal Care includes homemaking, companionship and other non-medical assistance. 4 5

5 ASSISTED LIVING Assisted Living Facilities (ALFs) are viewed as the best of both worlds, because residents have as much independence as they want with the knowledge that personal care and support services are available. Assisted Living communities provide residents assistance with basic ADLs (activities of daily living) such as bathing, grooming, dressing, and more. These communities differ from nursing homes in they don t offer complex medical services. Some criteria to consider when choosing an ALF are: budget, location, level of care needed and what level the facility is licensed to provide. Assisted Living is typically paid for through private funds, but there are exceptions. Some long-term care insurance policies cover licensed Assisted Living. If the perspective resident was a war veteran, or is the spouse of a war veteran, he or she might qualify for Veteran s Benefits that can help pay for assisted living. Medicaid funds and waivers are sometimes available to help with assisted living costs. Our advisors can help determine eligibility and narrow the list of facilities that best meet your situation. 7 NURSING HOME NURSING HOME Nursing homes offer the most extensive care outside a hospital. These homes offer custodial care and skilled care. Custodial care includes assistance with bathing, getting dressed, and eating. Skilled care includes medical monitoring and treatments provided by registered nurses or specially-trained physical, occupational and respiratory professionals. About half of all nursing home residents pay nursing home costs out of their own savings. The average annual cost of nursing home services was about $60,000, and the average stay was two years. Under certain limited conditions, Medicare will pay some nursing home costs for beneficiaries who require skilled nursing or rehabilitation services. To be covered, you must receive the services from a Medicarecertified skilled nursing home after a qualifying hospital stay of three days or more. For people with limited income and assets, Medicaid is a state and federal program that will pay most nursing home costs. To determine if you are eligible, check your State s requirements. After personal savings and other resources are spent, many people who stay in nursing homes for long periods eventually become eligible for Medicaid. 9 SHORT-TERM REHABILITATION Short-term rehabilitation facilities provide therapy for individuals recovering from a surgery, illness or accident. Generally, those needing short-term, in-patient rehabilitation may remain involved in their program at a facility for as little as a couple of days to as many as several weeks. Short-term rehabilitation programs help patients achieve their maximum functional capacity and get back to their homes and community in the shortest time possible. To achieve this goal, patients receive physical, occupational and speech therapy from compassionate and highly skilled therapists. Therapists are part of a team that includes physicians, nurses, social workers and nutritionists, who work with the patient and family members to develop an individualized care plan. 13 MEMORY CARE MEMORY CARE VOCABULARY Although many are able to care for someone suffering from Alzheimer s or other forms of dementia at home, it may come to a point in the disease process where home care is unmanageable. With the variety of Home Health Services that are available, placement may be delayed; but often, placement is warranted. Assisted Living Facilities and Nursing Homes can provide housing for seniors with Alzheimer s and Dementia but the question now becomes what level of care does my loved one require? Sometimes there is a fine line that separates an appropriate assisted living resident from a resident who would be bet- memory care: facility designed for individuals with a level of cognitive impairment that makes it unsafe for them to remain at home, and may require residence in a secured area. 14 MONEY & LEGAL Medicare Medicare is our country s health insurance program for people 65 years or older. The program helps with the cost of health care, but does not cover all the medical expenses or the cost of most long-term care. Medicaid PAGE 26 PAGE 28 Medicaid is a health and long-term care coverage program that is jointly financed by states and the federal government. Each state establishes and administers its own Medicaid program. The New Choices Waiver program is designed to help people who are living in nursing homes because of financial necessity to move out and live in an Assisted Living environment. Reverse Mortgages PAGE A Reverse Mortgage is a financial tool designed to be a form of financial relief for homeowners 62 and older. It allows seniors to stay in their home, eliminate their current mortgage payment, and access their equity. It is important to talk to a professional to discover whether a Reverse Mortgage is right for your situation. Social Security PAGE 44 Social Security retirement benefits may start as early as age 62, or as late as age 70. Monthly benefit amounts depend on the age you begin receiving it. As you plan for your financial future, take the time to better understand your Social Security benefits. Quality of care is affected by how benefits are selected and received. Unless Medicare recipients have supplemental insurance, the deductible and coinsurances must be paid. Medicare does not cover all health care services, and rarely does it pay 100 percent of the cost. Health care services not covered by Medicare include but are not limited to: most care received outside of the United States, cosmetic surgery, most dental care, hearing aids, personal care/custodial care, housekeeping services, most nonemergency transportation, and vision care. 16 MEDICAID Medicaid is a jointly funded, federal-state health care program for financially eligible persons. Medicaid provides care for acute medical needs, rehabilitation, and long-term care (at home, in nursing homes, and some programs for Assisted Living). There are also numerous community-based programs including adult day care and assistance with local transportation. At one time, Medicaid programs allowed beneficiaries to qualify if they gave away their house and all their money not anymore. There is a 5-year lookback law, and if asset transfers are done incorrectly, Uncle Sam will find your money and make you pay. For a low-income senior to qualify for Medicaid, consult a Medicaid professional to understand the acceptable ways to spend-down assets. Some assets may be gifted within limits to a beneficiary, but these assets will no longer be in your control. Spousal poverty protection laws have been passed to allow the spouse of a senior, who needs long-term nursing home care, to maintain up to 50% of the couple s assets. 18 VA BENEFITS Largely unknown, the Veterans Administration offers Aid and Attendance as part of an Improved Pension benefit. This pension benefits Veterans and surviving spouses who might require the regular attendance of another person to assist in eating, bathing, dressing, undressing, medication dosing, etc. It also includes individuals who are blind, and patients who are incapacitated mentally or physically so long as they are in a nursing home, require Assisted Care, or are in an Assisted Living facility. This important benefit is often overlooked by those who qualify and need additional monies to care for loved ones. This is a Pension Benefit, and is not dependent upon service-related injuries for compensation. Aid and Attendance can assist in paying for in-home care, Nursing Home care, or care given in an Assisted Living Facility. Veterans are eligible for up to *$1,732 per month, while a surviving spouse is eligible for up to *$1,113 per month. A Veteran with a Spouse is eligible for up to *$2,054 per month. *The pension income calculation of each category will allow for a benefit from $0 to the Maximum Allowable Pension Rate (MAPR). Any War-Time Veteran with 90 days active duty who serves at least 1 day (beginning or ending during a period of War) is eligible to apply. 32 VOCABULARY VA benefits: Veterans and their dependents may be eligible for assistance, health care, military pensions, housing, burial aid, life insurance, and other benefits. 20 REAL ESTATE Baby boomers are getting older, and despite their desire to remain home, some seniors will be unable to accomplish their goal. There are many reasons a senior or family member of a senior may opt to sell their home. Such reasons may include: too much home for a senior to maintain or afford, too much isolation from familial support, or too many modifications are required for a comfortable and safe living environment. Once all the options have been considered, and the decision is made to sell a home, finding a Real Estate agent that specializes in assisting seniors is the next step. A REALTOR who has completed a series of courses on how to help seniors and their families with real estate transactions is known as an SRES. They specialize in helping people age 50 and older, and can assist in selling, buying, or renting real estate. An SRES will help seniors look at all the available choices, and can coordinate with other professionals which handle estate taxes and gifting. In order to pay for senior care costs and to remain eligible for government benefits, an SRES can provide help in staying within asset guidelines.
