1 Application Cover Sheet Belmar Apartments 1725 Ocean Ave, Santa Monica, Ca Thank you for your interest in Belmar Apartments, a new affordable housing community featuring 158 beautiful Studio, 1, 2 and 3 BR apartment homes. Amenities will include on site subterranean parking, steps from the new Tongva Park, bike storage, outdoor lounge and BBQ grills, community room, fitness center, computer lab, and on site professional management. Please complete the attached Application for Occupancy. Please note that all adult applicants must sign and date the application. Please mail a fully completed application to: BELMAR APARTMENTS P.O. BOX 7250 SANTA MONICA, CA Applications received will be selected utilizing a random lottery process and will then be dated, numbered and logged in the order that they are opened. APPLICATIONS MUST BE POSTMARKED NO LATER THAN JANUARY 2, 2014 TO BE INCLUDED IN THE LOTTERY. Ten studio units have been set aside for Artists as Live/Work units. Eligibility is determined by household size and income for all units. Maximum income limits apply up to 60% of the Area Median Income. 2 Rental Selection Criteria Effective December 3, 2013 CREDIT HISTORY The first step in the application process is a credit report which will be processed on each adult applicant. The credit report will be part of a scoring model. Approval will be based on the indicators of future rent payment performance. A positive credit score must be obtained for the approval process to continue. CRIMINAL BACKGROUND Applicants acknowledge that a criminal background check of all adult household members will be part of the application process. INCOME Applicant(s) must have a verifiable source of income that when combined the gross amount is equal to a minimum of 2.5 times the monthly rent. The minimum income requirement is waived for Section 8 voucher holders as rent will be calculated based on the household gross monthly income. PROOF OF INCOME When applying, it is the obligation of the applicant(s) to provide proof of all household income. For employment we require copies of the last 3 months of paycheck stubs; or job offer letter stating the start date and salary on company letterhead. For self employed individuals the previous two year s of filed tax returns, Schedule C and 1099 s. RENTAL HISTORY Applicants must provide cancelled checks or rental payment receipts/payment history for the past 6 months. We will verify rental history for the last 3 years and will only verify with a landlord, apartment community or mortgage company, (including a prompt payment record and compliance with all community policies). CONDITIONAL APPROVAL An Additional Deposit will be required if the credit score is in the conditional approval range. AUTOMATIC DENIAL FOR RESIDENCY An Applicant will automatically be denied for the following reasons: Inappropriate household size for the unit Total household income exceeds the applicable income limits published by HUD or does not meet the minimum income limit. Insufficient points on our credit scoring model. Anyone currently in the process of filing a bankruptcy or with a bankruptcy filed in the past 12 months. History of late payment of rent that demonstrates more than 2 late rental payments per year of residency or more than 1 returned check (NSF) in a one year period. Any evictions by a previous landlord in the past three years. Any unresolved debts to a landlord or mortgage holder, (unless debt is paid prior to approval of rental application). Negative landlord reference(s). Rental applicants who have been convicted of one or more criminal offenses involving fraud, violence, sexual offenses, drug related activity, injury to person or damage to property, including, without limitation, homicide, battery and assault, sexual assault, possession or sale of an illegal substance, burglary or theft, illegal possession or sale of weapons, or any other activity that may threaten the health, life, safety of any individual. Rental applicants who have pleaded guilty or no contest to one or more of such criminal offenses. Rental applicants who have had one or more of such criminal offenses disposed of other than by an acquittal or finding of not guilty. Household cannot pay the full security deposit at move in. Household refuses to accept the second offer of an apartment. Providing or submitting false or untrue information on your application or failure to cooperate in any way with the verification process. Household fails to respond to interview letters or otherwise fails to cooperate with the certification process or failure to sign consent forms. ANY adult household members fail to attend eligibility interview. Household is composed entirely of full time students and does not meet the exceptions outlined in Section 42 of the Internal Revenue Code. Applicant has failed to provide adequate verification of income or we are unable to adequately verify income and/or income sources. Unit assignment will NOT be the family s sole place of residency. Family members, age 6+ failed to provide proof of a social security number or refused to certify that they have never been assigned a number. GRIEVANCE/APPEAL PROCESS Failure to meet one or more of the foregoing screening criteria may be grounds for rejection, however, each application is considered as a whole and the above factors are considered as part of a weighted formula. Should the applicants fail to meet the screening criteria, they will receive a notice in writing indicating that they have the right to appeal the decision.
