Dermatologists may be planning to add to their practice by expanding or upgrading their physical office space or by relocating. Or perhaps you re building from the ground up. What s on your agenda? REMODELING? In 1985 when Jerry Bagel, M.D., opened the doors of Windsor Dermatology in East Windsor, N.J., he was a solo practitioner working in a 1,000-square-foot office. Over the ensuing two decades, he expanded and remodeled several times. Today, he works with five other dermatologists in 6,700 square feet of space in a two-story medical office building that he now owns. 4 SUPPLEMENT TO DERMATOLOGY WORLD // July

Dr. Bagel first remodeled in 1991 when space became available next door to his office. Establishing the Psoriasis Treatment Center of New Jersey (within Windsor Dermatology) required knocking down a few walls, moving electrical boxes, and adding new plumbing. Designing the center was different than designing exam rooms because it required four ultraviolet light boxes. Additionally, nurses had to be able to access the booths to supervise more than one patient s treatment at a time. In 1992, Dr. Bagel purchased the building after it went into foreclosure, and when another 1,000 square feet became available, he expanded to hire another dermatologist. In 2008, Dr. Bagel renovated an additional 3,000 square feet in which he now conducts clinical trials. Around that same time, a Mohs surgeon joined the practice. Converting the space from an oral surgeon s office to a Mohs surgeon s office was not complicated, Dr. Bagel said. He was even able to purchase the procedure chairs from the oral surgeon. >> SUPPLEMENT TO DERMATOLOGY WORLD // July

8 REMODEL, RELOCATE, or EXPAND? Dr. Bagel did, however, have to reserve extra time and resources to ensure that the space was compliant with Occupational Health and Safety Administration regulations. While clinical trial and Mohs patients are seen on the second floor, all other examination rooms are on the first floor. He is satisfied with the new setup and said the space flows well. The doctors do have to run upstairs to see trial patients, but I think the exercise is good for everybody, he quipped. He also had to consider the impact of the construction on his patients, who still needed to come to the office for care. He was able to hire a contractor who was willing to do much of the work after hours and on the weekends. Working around patient hours resulted in less service disruption for Dr. Bagel s patients as well as the other tenants patients. Additionally, the contractor couldn t have two people working at the same time, Dr. Orbuch recalled. Nearly four years later, he works with two other dermatologists in a 2,000-square-foot office. His primary consideration when looking for new office space was location. Dr. Orbuch wanted to relocate in the same area and retain his existing referral base. Additionally, Dr. Orbuch, who is also a clinical associate professor of dermatology at NYU Langone Medical Center, wanted to be within walking distance of the medical center. Location is a primary consideration when relocating one s practice. Like Dr. Orbuch, George Hruza, M.D., of Laser and Dermatologic Surgery Center in Chesterfield, Mo., preferred staying in his existing office. However, after meeting with an architect to determine the feasibility of designing an ambulatory surgery center Getting there is exciting, but... it takes a real committment to do this. had all the necessary materials in stock before he started demolition to keep the project running smoothly. In the next few years, Dr. Bagel plans to take over another 3,000 square feet. He may move the psoriasis center upstairs if the floor can hold the weight of the light boxes. Although Dr. Bagel came close to purchasing other sites over the years, he always returned to his original office space. I ve been here since the day I started, he said. It s been a good building for me. RELOCATING? But for some, remodeling is not the best option. When the office building that Philip Orbuch, M.D., of Murray Hill Dermatology Associates in New York City, worked in for 15 years converted to condominiums, he chose to relocate. The asking price for the 900-square-foot office was not only exorbitant, it was so small that he used to work three days a week and his associate worked two days a week. We split the time because you really (ASC) in the existing space, he realized that he had to build new construction to meet all the building and fire code requirements. Even if we could convert the inside of our space, we couldn t retrofit the entire building to meet these requirements, he said. After two years of looking, Dr. Hruza found a acre lot. As part of the purchase price, he received an allowance to build a 20,000-square-foot medical office building. As part of the process, he had to deal with city inspectors about zoning regulations; the bureaucracy was overwhelming at times. Because Dr. Hruza s primary focus is Mohs surgery and reconstruction, he draws patients from a large geographic area, so he was less concerned that his referral pattern would be affected if he moved far from his first office. Still, you don t want to move 20 miles away, he said, adding that a dermatologist in a general practice probably wouldn t want to move more than a couple miles away from his/ her existing office. 6 SUPPLEMENT TO DERMATOLOGY WORLD // July

9 COST CONSIDERATIONS Regardless of whether you choose to remodel or relocate to a newly built space, there are some key factors to consider, including cost. Dr. Bagel cautioned that building projects always cost more and take longer than expected. The key is minimizing those factors. Dr. Hruza split the costs with another dermatology group. He believes it is more cost effective to build a larger building, which will yield a higher price at some point down the line if they decide to sell it. OPTIMIZING YOUR FLOW Patient flow and work flow are two other significant factors. Dr. Hruza was very involved in planning the layout. He separated the 4,000-square-foot ASC from the remaining 6,000-square-foot office space. Dr. Hruza designed the H-shaped space for two dermatologists working simultaneously, although at this point he is practicing solo. There is a second waiting room on the office side for patients who are in between Mohs stages. The Mohs lab and nursing station are located in the middle, making them accessible from all of the patient rooms. The ASC includes one operating room, one procedure room, three pre-operative rooms, three recovery rooms, a waiting room, and ancillary space. The flow has worked out very well, he said. The contractor with whom Dr. Bagel worked proposed a design plan that optimized patient flow. Next, the office personnel reviewed it. Obtaining feedback from people who work in the space resulted in some improvements to the layout, said Frida Bagel, Dr. Bagel s wife who has an interior design background and assisted with the remodeling projects. For example, staff identified some glitches in the floor plan that would have created bottleneck areas. Of course, size is a consideration. Some patients don t move as fast as others, Dr. Bagel noted. If you have more rooms, people can flow in and out easier. Plus, the slower patients are not being rushed. Additionally, it s harder to attract people to work with you if you have a small space, he said. When planning a new office, Dr. Bagel recommended making it large enough to last five to six years. His wife believes that it is better to have extra space than tight quarters. Don t limit yourself with regard to space, she advised. People are more at ease and get along better when they have more space. When employees are bunched together, they will be less patient with patients. In addition, Dr. Bagel was able to put a histotech lab in a small extra room for which they originally had no purpose. In general, regarding remodeling, Dr. Bagel said, It s not something to be fearful of, but it is something to keep your finger on the pulse of, because it s costly and time consuming. PROJECT MANAGEMENT Dr. Hruza says he was fortunate to have a very sharp and detail-oriented office manager who oversaw the project. Without such an individual, it may be worthwhile to pay a consultant who could identify some design issues upfront. When you are asked to sign off on a 100 pages of blueprints at a time, you can miss some things, he said. An example is the fancy eye wash station that the contractor installed in the Mohs lab because he had installed similar ones in other medical office buildings. We could have used a $30 squirt bottle for an eye wash station that meets the requirements, Dr. Hruza noted. It not only cost $2,000, but the eye wash station was poorly installed and caused flooding in the unit one year later, resulting in hundreds of thousands of dollars worth of damages. Another example is the backup generator that was four times larger than necessary and four times as expensive. He recently learned that he can reconfigure the ASC to allow for one more procedure room, which would double his capacity, something an experienced consultant may have known. Dr. Hruza is currently doing minor remodeling. Getting there is exciting, he said. But it takes a lot longer, more money, and more of your time than you think it will. It requires a real commitment to do this. Now that the practice is up and running, he believes it was worth the effort. The building is gorgeous. 13 BY LISETTE HILTON, CONTRIBUTING WRITER Patient-centric office ensures excellent service Making extraordinary your patient satisfaction goal. Physicians are in the service business. We serve other people. That s really all we do. We don t make a product, produce or manufacture anything. We re not in the manufacturing industry. We don t grow anything; so, we re not in the agrarian industry. We re a pure service industry, said Victor J. Marks, M.D., director of the department of dermatology at Geisinger Health System in Danville, Pa. Like any service industry, health care is customer driven. We ve been measuring patient satisfaction at Geisinger for 15 years, and we think it s absolutely crucial. We use our patient satisfaction data to drive profits and personal improvement, Dr. Marks said. Dermatologists might want to phase out the term customer service, and replace it with service excellence, according to Charles Ellis, M.D., professor and associate chair of dermatology at the University of Michigan Medical School in Ann Arbor. I hear from doctors they don t like the concept of using the word customer. They don t want to call their patients customers, because, of course, they have professional relationships with their patients, said Dr. Ellis, who is working for the University of Michigan Health System in an effort to improve service. By calling it service excellence, we don t have to identify people by a specific title, like customers or patients. >> SUPPLEMENT TO DERMATOLOGY WORLD // July

14 Patient-centric office ensures excellent service The department of dermatology at University of Michigan Medical School has been number one in the health system in patient satisfaction for a decade. But the benefits of being focused on providing great service go beyond patient satisfaction, according to Dr. Ellis. We found out that this great service concept is not just for patients; it s also for the physicians and staff. We found that our department is a much nicer place to work. We ve retained our terrific employees for longer periods of time, and it has been easier to accomplish things, Dr. Ellis said. It also has reduced significantly the number of complaints that we ve had to deal with. By reducing complaints, you can use that time for other more purposeful things. SERVICE FUNDAMENTALS Dr. Marks breaks down service, or how a patient perceives his or her experience with a practice, into four categories: systems and processing, environment, amenities, and communication skills. We have found that this great service concept is not just for patients; it s also for the physicians and staff. We tend to think about the fifth, which is our diagnostic, clinical, technical skills, Dr. Marks said. Unfortunately, those are the things that patients have a lot of difficulty judging, because they don t have the same expertise we do. So, they judge us by our systems and processes, environment, amenities, and communication skills that we exhibit. SYSTEMS AND PROCESSING This service area includes the processes by which patients experience their visits. How easy is it to make an appointment? How easy is the check-in process? How easy is the check-out process? How accurate and timely is the billing? What is the flow like in the office? Very often, patients will complain about a system that is not working. For example, they might say that it takes too long to check out, Dr. Marks said. People don t like to wait, Dr. Ellis said. Part of providing good service is to analyze your wait times, and see if you can reduce the time that your patients are waiting, he said. Dr. Ellis said his department measured the times from when patients checked in to when they entered exam rooms. When we first measured it, it took an average 26 minutes. Now we re down to an average of five minutes. We did this without changing the numbers of patients or workers. It took a long time, continually working on our scheduling template until we were able to reduce the time that patients had to wait, Dr. Ellis said. We don t even call it a waiting room anymore; we call it a reception area. ENVIRONMENT AND AMENITIES We all experience the world through our senses: our sight, smell, hearing, touch and taste, Dr. Marks said. So, if you think about your practice, how is the patient experiencing your practice through those senses? What does the environment feel like? Smell like? Sound like? Taste like? Appear like? Physicians might enhance the experience for patients by having background music. A professional environment, where staff wears neatly tailored polo shirts, is an example of a small change that could enhance the office environment for patients. These are little extras that patients aren t expecting when they experience your practice. Do you offer up-to-date magazines in your waiting room? Fresh-cut flowers? Artwork? Refreshments? Samples? There are certain things that are and aren t desirable in a reception area, according to Dr. Ellis. For one thing, you don t want a clock in the reception area, he said. Many offices have a water dispenser in the reception area. Of course, if you have a water dispenser, you want to make sure it s full. I d rather have no water dispenser than one that is empty. COMMUNICATION SKILLS Physicians, nurses, receptionists, secretaries everyone who interacts with patients should be skilled in and in tune to how they communicate, verbally and nonverbally, according to Dr. Marks. Each of us can improve our ability to communicate in a way that s pleasing to patients, he said. There s an easy way for physicians to improve communication, he said. 12 SUPPLEMENT TO DERMATOLOGY WORLD // July

15 I pause outside the door, before entering the exam room (to gather myself). Knock; then enter the exam room with energy and a smile introducing myself and shaking hands with the patient. That s rather than rushing into an office, Dr. Marks said. One of the things we ve taught our nurses: When you go to the waiting room to call a patient, don t just stand at the door and call Mr. Jones. Rather, when you call Mr. Jones and see him standing up, walk over to him, help him, introduce yourself, shake his hand, and introduce yourself to his family. It s a more welcoming way of greeting the patient. People want to be greeted warmly. They want to have their names used. They want to be listened to and cared for, according to Dr. Ellis. They want a warm goodbye as they leave, he said. These are the types of interactions that tell patients that they are really being appreciated. We ve found that one of the easiest tips is to say, when appropriate, my pleasure, instead of you are welcome, Dr. Ellis said. You re welcome is such a neutral phrase. My pleasure is a much warmer and much stronger statement about the importance of the patient. After you ve said it a few dozen times, you begin to internalize it, and then continuously realize it is a pleasure to take care of patients and help people get better. PINPOINTING PATIENT CONCERNS One way dermatologists can gauge how their patients perceive their experiences with your practice, as well as whether or not the practice is improving in service excellence, is to conduct regular patient satisfaction surveys. Ready-made surveys are available online or through companies that specialize in patient and customer satisfaction tool. Or, practices can develop their own survey questions and ask a sample set of patients. The American Academy of Dermatology offers an online survey tool to its members, which also helps dermatologists satisfy one of their MOC-D requirements (go to According to Dr. Ellis, low-cost alternatives include having the office manager interview a few hand-picked patients about what they think of the services they ve received. Practices might also create a card with three or four questions and hand it to patients for a few days. You can collect those in a locked box upfront, on their way out. That s the easiest way to do it, Dr. Ellis said. The staff would tabulate results after they ve received 30 to 50 surveys and consider what changes if any should be made, before starting the process again. Most surveys don t ask questions about medical outcomes; rather, they are focused on the experience that the patient had and interactions during the visit, according to Dr. Ellis. NOTHING SHORT OF EXEMPLARY The keys, according to these experts, are to monitor results, act to improve gaps, and survey patients on a regular basis. Here s the most important thing: Most patients will rate you well. The real key is to use the data to achieve exemplary status. If a patient says pretty good, that s not good enough, Dr. Marks said. Certainly, if you get a negative evaluation, think critically about it. Don t just brush it off well it s probably just a patient who is having a bad day. More often than not, it s a breakdown in your system, or your process of communication, or the environment in which you provide the care. Use the negative and non-exemplary responses to improve, advises Dr. Marks. If you don t do anything about it, you re not improving, he said. Whether using surveys or not, practices can successfully focus on service excellence, according to Dr. Ellis. DERM LEADERSHIP IS ESSENTIAL This is an opportune time for dermatologists to look at whether there are gaps in their service, Dr. Ellis said. The MOC-D program requires dermatologists to conduct periodic patient surveys. The 2010 health reform law offers physicians financial incentives to participate in programs like MOC-D. Further, the health reform law requires that patient experience information be published on the Physician Compare website in the future. It s important that doctors lead the commitment to service excellence, according to Dr. Marks. You can t delegate this process to an office manager. I think that we, as physicians, are the leaders in our offices, and if we want our staffs to see this as an important aspect of our practices, we need to lead it. It needs to be a priority, Dr. Marks said. If physicians and their staffs make a commitment to service excellence, they can make substantial improvements, according to Dr. Ellis. It takes time. It s often gradual. But I think if you re focused on it and speaking about it in your staff meetings, everybody will be thinking about it from a patient-centric approach, Dr. Ellis said. I think you ll see that you ll have much higher patient satisfaction; you ll reduce the complaints; and you ll have a better workplace. dw SUPPLEMENT TO DERMATOLOGY WORLD // July

17 Like doctors in other specialties, dermatologists are feeling the pressure to identify, report, and implement processes to reduce errors, as the focus on patient safety moves from the inpatient to ambulatory setting. In medicine, in general, there is a move to close the gaps and create safety nets to improve patient safety, said Dirk Elston, M.D., director of the Ackerman Academy of Dermatopathology in New York. Institute of Medicine reports, starting with To Err Is Human: Building a Safer Health System, released in 1999, compelled physicians and other health care providers to face their vulnerabilities, according to Dr. Elston. We re owning up to the fact that people do make mistakes, and there are ways to prevent them by putting engineering into place that reduces the chance of errors occurring and creates safety nets to catch errors when they occur, he said. >> SUPPLEMENT TO DERMATOLOGY WORLD // July

18 NOT TO ERR DERMATOLOGY-SPECIFIC SAFETY ISSUES: THE BIOPSY Literature collected by the American Academy of Dermatology s Performance Measurement Task Force suggests that gaps in dermatologic care occur more often with surgeries and biopsies. This realization triggered the development of a biopsy follow-up process measure, which was submitted for inclusion in the Medicare s Physician Quality Reporting System (PQRS) in There are many steps involved in the labeling, transmittal, processing, and reporting of those biopsies, said Dr. Elston, who chairs the Academy s task force. Finding the site of a recent biopsy can be a problem in dermatology practices. Dermatologists, according to Dr. Elston, are very adept at doing biopsies that don t leave undue scars so good, in fact, that biopsy sites may be hard to find when patients return for surgery. On the other hand, the patient may have many scars from prior procedures, and the biopsy site gets lost in a sea of scars. In either case, which is the recent biopsy? he said. Pre-procedure photographs and diagrams have been cited as the best ways to accurately identify biopsy sites. Another common error is the mislabeling of specimens. In a typical scenario a dermatologist may perform three biopsies (a, b, and c) on a patient, on the same day. Biopsy b is almost always in the right bottle but a and c might be switched, because of errors in communication between the dermatologist and the assistant. For the patient, it can make a big difference whether the melanoma is in bottle a or c, Dr. Elston said. There are all sorts of variations on that scenario. Following the universal protocol of the Joint Commission should help avoid biopsy mislabeling, according to James S. Taylor, M.D., consultant dermatologist and the quality improvement officer for the Dermatology and Plastic Surgery Institute at Ohio s Cleveland Clinic. It s where, before you finish the procedure, you double check that the specimens are labeled correctly, with the correct patient s name, biopsy site, and differential diagnosis, he said. The next step is the follow-up. If, for example, a report comes back as malignant melanoma, there s the issue of timely reporting to the patient. Do you send a letter? A letter could be lost in the mail. It s critical, according to Dr. Taylor, that the dermatologist or staff records the pathology or laboratory results in the patient s chart and documents when and how the patient is notified. For skin cancers I recommend both written and verbal communication with the patient, he said. The important thing is to make sure that you tell patients that they will hear from you one way or another, and, if they haven t, they should call your office, Dr. Taylor said. The AAD s Performance Measurement Task Force has been identifying gaps in care through its participation in PQRS. The National Quality Forum, a voluntary consensus standards body that reviews and endorses quality measures, has endorsed four measures relevant to dermatologists: Melanoma Coordination of Care; Over Utilization of Imaging Studies in Stage 0-IA Melanoma; Melanoma Continuity of Care Recall System; and Biopsy Follow-up. Dermatologists can currently report on the three melanoma measures in the 2011 PQRS program. The Biopsy Follow-up measure is tentatively slated for inclusion in the 2012 PQRS program. SURGICAL PITFALLS Dermatologists can avoid fire hazards, electrical shock, and more with such safe practices as electrical equipment grounding. We use electrocautery; so, proper grounding of all the electrical equipment (lights, sockets, everything in the room where the surgery is being done) is important, said Dr. Elston, who was lead author on a continuing medical education article about patient safety in the August 2009 Journal of the American Academy of Dermatology. The proper use of electrocautery is another area where dermatologists need to be particularly aware of patient safety. Dermatologists need to consider if the patient has a pacemaker or implantable defibrillator, as well as to decide whether to continue anticoagulation for patients. If you continue the anticoagulation in someone who needs it for medical reasons, the patient may develop a hematoma. If you discontinue it, the patient may have a stroke. So the usual guideline is that if the anticoagulation is needed for a medical reason, it is not discontinued, Dr. Elston said. Other important safety considerations for surgical cases include sterility and infection control and proper patient positioning. Most injuries to patients in the office setting from surgery are from the patient fainting and associated falls, Dr. Elston said. Proper positioning of the patient during injection of anesthesia is important. MEDICATION ERRORS PREVENTABLE The most frequent errors in medicine, in general, relate to medications, according to Dr. Taylor. Dr. Taylor suggests that in order to avoid medication errors, dermatologists should think in terms of the five rights: right drug, right patient, right dose, right route (orally, intramuscularly, or intravenously) and right time. There are many ways to prevent medication errors. One safety net is electronic prescribing, which can be implemented with or without an electronic medical record (EMR), according to Dr. Taylor. Dermatologists can receive government incentives to implement e-prescribing and face penalties beginning next year for failure to do so. See for more information. The technology eliminates the problem of handwriting illegibility, because prescriptions are computer-generated and printed. It s not foolproof, however. If, for example, you have a drug with three different dosages: 100mg, 200mg, and 300mg, you may need to give the patient 100mg, but highlight 200mg accidentally, Dr. Taylor said. You have to proofread the prescription with electronic prescribing. It does not avoid all errors, but, overall, the odds are that it improves prevention of medication errors and expedites processing. Dermatologists and staff should avoid interruptions when filling a prescription, according to Dr. Taylor. A physician 16 SUPPLEMENT TO DERMATOLOGY WORLD // July

19 writing a prescription might be interrupted by a nurse, lose his or her train of thought, and make a mistake. Dermatologists should avoid prescribing look-alike and sound-alike medications and shouldn t use abbreviations when writing prescriptions. To avoid confusion, include both the generic and brand name; the drug strength; and the dosage form whether it s topical, oral, etc., the amount, directions for use, purpose, and refills in addition to the patient s name and address, Dr. Taylor said. The use of tall-man letters is also helpful and is now used by our electronic record. For example, write hydoxyzine to avoid confusion by the pharmacist with HydrALAZINE. Latin and other abbreviations are a significant source of medication errors. Dermatologists should spell out drugs and dosages, even when using electronic prescribing, according to Dr. Taylor. MTX is commonly used by dermatologists as methotrexate, but others might confuse it as Mustargen, Dr. Taylor said. HCT commonly is hydrochlorothiazide but a pharmacist might consider it to be hydrocortisone. Avoid verbal orders if at all possible. If you have to give a verbal order, enunciate slowly and distinctly. Spell out long and difficult drug names and specify concentrations. Then, have the nurse or pharmacist repeat your verbal order back to you. Carefully calculate dosages. Make sure medications are labeled correctly and identifiable. Botulinum toxin, for instance, is diluted with bacteriostatic 0.9 percent normal saline, but some practices might also be using concentrated sodium chloride for other procedures; so, it would be absolutely critical that you not mix up those two vials when preparing botulinum toxin for injection, Dr. Taylor said. PLAN; THEN, HAVE A BACKUP The Joint Commission s universal protocol and safety check list for hospital outpatient departments, according to Dr. Taylor, recommends that physicians, including dermatologists performing procedures, such as Mohs surgery, should conduct a pre-procedure verification including allergies, medications, informed consent, right equipment, and key laboratory results and then take a time out to check for the right patient, the right side and site, and the right procedure, at the right position. Everybody in the room should agree or voice concern if they don t. The post-procedure sign out checklist should include correct specimen labeling and whether the patient received and understands postoperative instructions, he said. Failure mode analysis, according to Dr. Elston, is an important patient safety practice that involves looking at what could go wrong and preparing for it. It requires that you ask questions, such as: What would happen if a patient had a myocardial infarction while in your office? Do you have all the equipment in place or are you in an office complex where someone else is supposed to respond? If you re in a group practice, is someone else supposed to respond? How do you reach that person? Do you call? What do you do if the phone lines are down because of a storm? Do all staff members and physicians know where the fire alarm is? Does everyone know where the fire extinguisher is? Does everyone know what to do with a patient if the fire alarm goes off? The goal of the analysis is not only to develop a plan, but also to create a backup plan. When an emergency occurs in the office, everyone s adrenalin is running. You want to be sure that everyone knows what they re supposed to do, there s a clear plan of action, and there have been practice drills. Instead of panicking, everyone knows what they re supposed to do, and they also know what the backup plan is, Dr. Elston said. GET ONBOARD Quality of care, which includes avoiding preventable errors, could affect dermatologists future reimbursement and certification status, according to Dr. Taylor. Payment to hospitals is going to be based, in part, on patient satisfaction surveys. I think that will eventually be the case in ambulatory settings, he said. Quality and patient safety issues are now part of maintenance of certification. Preventing errors requires not only reflection on one s practice, but also staff buy-in. This requires that you create a culture of patient safety in your office, Dr. Taylor said. The culture is one of fixing system issues rather than blaming individuals.
