2 INTRODUCTION TO ATTITUDE AND PRECEPTION SURVEY This survey is for parents or guardians of youth who live in or go to school in Community. Completing this survey is voluntary. If you are not comfortable answering a question, just leave it blank. This survey should take about 10 minutes to complete. Please DO NOT write your name on this survey. No one will know what you write. Please answer the questions based on what you know and think. The questions about your background will be used to describe the types of participants completing this survey. This information will not be used to find out your name. Please note: For all questions about your child in this survey, please answer thinking about your child(ren) in grades 6 through 12 in Community. If you have more than one child in grades 6 through 12, please answer for the child who had the most recent birthday.
