What Is Lockout Tagout Industrial workers are greatly exposed to electrical and mechanical hazards while servicing equipment and machinery. FACT: Unprotected workers are killed or injured due to workplace accidents. Europe imposes firm regulations (several) that require all power sources to be disabled during the maintenance and repair of industrial machinery. Lockout Tagout is an important security procedure that involves shutting off the power to industrial machines or equipment during maintenance or repair Lockout Tagout procedures prevent the accidental activation of equipment and minimizes the electrical and mechanical hazards faced by industrial workers.

Definition of Lockout Lockout : The placement of a locking device on an energy control device which COMMUNICATES that the equipment being controlled should NOT be operated until the locking device has been removed THREE main types of energy that LOTO is concerned with are: Mechanical the movement of a piece of machinery E.g. a printing press Chemical - The flow of a substance or chemical in a pipe E.g. steam/water Electrical The flow of electricity in an exposed installation Locking out prevents the release of unforeseen and dangerous energy surges

Definition of Tagout TAGOUT is the placement of a tag which indicates that the energyisolating device and the equipment being controlled MUST NOT be operated until the tag has been removed. Tagout helps communicate the risk

Lockout Tagout Video Accident Lockout Tagout Is not applied

Why Lockout Tagout / Advantages Lockout tagout is normally used when maintenance work is being performed. This could be: Cleaning equipment Adjusting equipment Repairing equipment When equipment is being set up or installed It acts as a tool to communicate what is being done, who is doing it, where and for how long. * Safe Working - protects industrial workers * Prevents accidents and injuries * Protects equipment from damage * Minimizes workplace errors * Raises level of awareness on industrial hazards It is a procedure which clearly communicates risk in the workplace

Relevant Legislation The Regulators View HSE The primary responsibility or managing risks rests with those who create the risk in the first place that means employers, self employed and manufacturers or suppliers of articles or substances for use at work Judith Hackett HSE Chair Provision of Work Equipment Regulations 1998 Regulation 19 Isolation from Sources of Energy, Paragraph 2.14, European Union (EU) Guidelines 89/655 "Every piece of equipment must be fitted with clearly visible devices with which it can be separated from every energy source. OSHA (a)(3)(i), The Control of Hazardous Energy (Lockout/Tagout), employers to establish a program and utilize procedures for affixing appropriate lockout devices (Occupational Safety and Health Association) Most comprehensive legislation worldwide regarding LOTO is in the US

Important New Legislation Health & Safety (Offences) Act 2008 Became Law In January 2009 Not just Manslaughter Act - Homicide Act in Scotland Lower Local Courts Impose fines for non compliance ( 5K- 20K) Up to 12 Months imprisonment If non-compliance resulted in serious injury or death then High Courts would manage case Unlimited fine 2 years imprisonment to life Regulators View We make no apology for taking a strong stance on enforcement Judith Hackett HSE Chair

Important New Legislation Health & Safety (Offences) Act 2008 Goals : - Reduce Injuries Motivate those in Health & Safety that they contributed to improved H&S performance Those with Heath & Safety duties that fail are brought to account Consequences:- Fines Imprisonment Directors Disqualification Brand Damage/PR Business Ending Staff Morale/Welfare 21st September T:+44(0)

Example Of Lockout Tagout

Master Keyed Padlock Systems Generic System

Example of a LO-TO Application/System Correct Use Of Padlocks is Key Why? Well defined LO-TO procedure with a clear padlock control policy will ensure safety & protect investment in your Lockout Tagout Safety procedures E.G Two Types of Padlock Categories 1: Isolation Padlocks actually placed on isolation switches with tag and or hasp. 2: Safety Padlocks used with hasp on Lockout Box holds isolation key/s and fuses other isolation keys (electrical) Identifies approval by each work group that area of work has been safely locked out

Example of a LO-TO Application/System (Top Level) Permit Issuer/Primary Acceptor : Define Scope Lockout Job + Return Isolation padlock Key with Additional Elec keys /Fuses Into Lockout Box Lockout Form Completed /Updated Permit Issuer Key Padlock Cabinet Permit Issuer Places Padlock on Lockout Box and locks off box with Permit Issuers unique Padlock + key Prepare Permit To Work When Permit Acceptor accepts isolation is complete he places colour coded padlock on to lockout box E.G Permit Acceptor Types Permit Issuer = Yellow Safety Padlock Fitter = Red Safety Padlock Electrician = Blue Safety Padlock Tiffy = Orange Safety Padlock Outside Contractor = Black

Example of a LO-TO Application/System (Top Level) LO-TO Key holders Form Updated All Supplementary Acceptors Have Accepted Work Permit (Signed up to Isolation Lockout ) Work Is Now Carried Out According To Plan Colour Coded Padlock On Lockout Box Containing where all isolation padlock keys/fuses etc are locked out As work groups finish their supplementary tasks they remove their colour coded lock from Lockout Box & sign off Key Holder Form All Work Groups taken locks off Lockout Hasp Job Complete Permit Acceptor & Permit Issuer Agree work complete & execute Companies Completion procedure

Next Steps For Companies Legislation : Corporate Manslaughter/Homicide 1 st Successful conviction in Feb 11 Better Regulation Target Enforcement Personal Liability Detailed Presentation To key workers involved in Lockout Tagout/Permit To Work Systems Raising awareness of LOTO e.g. Posters Involve Staff in Loto practices/procedures/reviews (Ensure Buy in)
