3 22% psychiatric inpatients vs. 9% of the general population consume > 750 mg caffeine a day. A 19-year-old woman on long-term clozapine therapy developed unexpected clozapine toxicosis. She had been drinking 6 caffeinated cola drinks per day. Stopping the cola intake was followed by a substantial reduction in clozapine concentrations. In a study of 7 patients on clozapine who drank caffeinated beverages, stopping caffeine for 5 days resulted in a 47% reduction in clozapine plasma concentrations. After resuming the caffeine intake, their clozapine concentrations return to the original levels. Patients on clozapine should avoid wide fluctuations in their caffeine intake. Clinicians should consider changes in caffeine intake in patients on clozapine who develop unexpected clozapine toxicosis or lack of clozapine response. (JR Horn and PD Hansten.
