3 POST ADOPTION SERVICES The Santa Clara County Department of Family and Children s Services: For information about any of the Post Adoption Services, contact Santa Clara County Post-Adoption Service Social Workers at (408) Post-Adoption Services are provided to birth parents, adoptees, and adoptive parents after the adoption has been finalized. The services which are provided by Santa Clara County s Post Adoption Services program include the following: Reassessment of AAP (Adoption Assistance Program) benefit. One primary function of the County s Post-Adoption Services Program is the facilitation of the Adoption Assistance Program (AAP) for those families who were approved for AAP benefits prior to the adoption finalization. o AAP assists with the adoption of children who might otherwise remain in long-term foster care. This program provides financial assistance, including help with medical care, to families who adopt special needs children and who may not have the financial resources to meet the needs of these children. The Post-Adoption Social Workers can provide consultation, AAP information, and assistance with information about community resources to address the needs of your child. Request for non-identifying information from adoption records. If you were adopted, we will provide you with a report about your adoption. The information that is provided is non-identifying. (Adoption records are permanent records preserved and maintained indefinitely by Santa Clara County) Reunions: If you were adopted in Santa Clara County, we will facilitate reunions between adult adoptees and birth parents and between adult adoptees and birth siblings according to California Law. Updating of adoption records: We will update adoption records with current addresses and other information received from birth parents and adoptees. 3
4 Request to forward letters, cards, and pictures: We will forward letters, cards, and pictures from adoptive parents to birth parents and vice versa per earlier agreements made at the time of adoption finalization or to place such material into adoption records for possible delivery to the adult adoptee or to the birth parent at some point in the future. Requests for support groups by the adoption triad including the Parents Adoption Camp: Kamp Pakk. Adoption Wraparound Program: The department of Family and Children s Services is working in conjunction with Seneca Family of Agencies to provide wrap around services to adoptive families, which is designed to maintain the adopted child in the home. MEDICAL RESOURCES CHILD HEALTH AND DISABILITY PREVENTION (CHDP) Program: For Medi-Cal eligible children age 3-20 CHDP provides complete health check-ups in order to detect and treat medical and dental problems before they become serious. DENTAL RESOURCES Medi-Cal also covers dental care and in certain cases, orthodontia. To contact Santa Clara County Mental Health (SCCMH) call If experiencing a mental health crisis, staff is available 24 hours a day, 7 days a week and can speak with you in English, Spanish, and Vietnamese. For the Kids Connection Program (children 0-5 years old): call the SCCMH Call line or fax referral to: Medi-Cal Mental Health Services Value Options Program If you live in California but outside of Santa Clara County and you need information regarding health care providers, child psychiatrists and/or mental professional who will accept your child s Medi-Cal insurance, contact a representative from the Value Options program at for a list of providers in your area. EDUCATION RESOURCES The Santa Clara County Department of Family and Children s Services has an Educational Services Coordinator available to answer questions about your child. 6 COMMUNITY ADOPTION SERVICES FAIR (Families Adopting in Response) FAIR is an adoption support organization composed of adoptive parents and adoptive children. Workshops and support groups are available in addition to other services. There is a modest membership fee. To contact FAIR, call (650) or visit their Web site at ww.fairfamilies.org.
