1 Waukesha County Health and Human Services A brief summation of WCH & HS Process: They do provide both services in the area of Mental Health and Substance Abuse. These service are not free, the cost is based on that person s ability to pay. Intake Specialist is first stop. In most cases they will need to produce information on income level before they can proceed. If not on BadgerCare, this will necessitate some form of income verification: a. Tax return b. Earning statement from Social Security (4pages) Patient is responsible for costs till proof of income is received and evaluated. The next step would be evaluation by a health care specialist who will evaluate the issues and recommend the next step in the patient s care. Catholic Charities Catholic Charities offers Behavioral Health/Counseling Services. This is a State certified outpatient behavioral health clinic providing accessible, culturally competent, high quality mental health services for individuals, families, children and couples of all faiths. All therapists are licensed to practice in the state of Wisconsin and bilingual therapists are available at all sites. Clients served are adults, children and families who suffer from emotional distress. Fees for Service The agency accepts patients who do not have any means to pay, as well as those with limited resources, on a sliding fee scale adjusted to an individual or family s size and financial situation. Additionally, third party insurance payers, Medicare and Medicaid are accepted. Addiction Resource Council 741 N. Grand Ave. # 300 Waukesha, WI Crisis Line Margaret Bayer Family Program Coordinator, Outreach Crisis Intervention - Support Group Service includes a regular Support Group offering listening support and community resources to assist families struggling to cope with alcohol or drug addictions. Drug and Alcohol Evaluation /Assessment Professionals trained to complete the assessment process efficiently and courteously. Many assessments that the agency completes are court ordered. The typical charge is between $150 and $200. The fee is sometimes waived for clients of a non-profit such as Hebron House. With more discussion between SVDP and ARC, fees may be waived for our patrons. ARC/Margaret Bayer is willing to present or discuss services at any time.

2 ProHealth Community Nurse (Jennie Soika) Provide Services for the Insured only: ProHealth provides care with referral from ProHealth doctor Have a full program / department for Behavioral Health An evaluation (assessment) is the first step. Provides counseling, psychiatric services (RX prescriptions) Referral services (ie: AA, various classes and programs anger management For the un-insured Referral services to: IMPACT 211 have counselors/crisis lines in Waukesha and Milwaukee Counties Heath & Human Services also provide services Waukesha Community Health Center (part of Sixteenth St. community Health Centers) The Salvation Army Adult Rehabilitation Center (ARC) 324 North Jackson Street, Milwaukee, Wisconsin The Adult Rehabilitation Center is a residential program that provides group therapy, spiritual guidance, and skilled counseling and referral services for men recovering from alcohol and drug abuse. Counseling and educational programs are provided by in-house counselors and residents participate in Alcoholics Anonymous and Narcotics Anonymous classes. Residents help out in operating Salvation Army Thrift Stores. Handling the processing of donated materials such as furniture, appliances or clothing allows the residents to learn new job skills and develop work habits that are necessary as they prepare to re-enter the job market. Adequate housing, meals and medical care are provided while enrolled at the Center. For more information on the ARC, the programs offered and store locations, please visit the official site. Who Is Eligible? Every potential participant undergoes a comprehensive intake interview to ensure the ARC program is the best possible match for them. If the interview process determines it s not, Salvation Army makes every effort to refer them to a program that will be. A long-term commitment of at least six months is required so patterns of poor decision-making can be broken and replaced with positive life choices changes that will help them become productive citizens of their community. Applicants with a desire to get help may be referred by families, friends, courts, clergy and community leaders or may simply call the Intake Office at SA-TRUCK ( ) to make an appointment or get more information about local ARC programs.

From Mike s write-up Vincentian Process (Home Visit Interview): While doing a home visit be aware of the underlying causes. Trust your gut feelings. If something doesn t seem right, you can call the Outpatient Clinic and bounce it off them. If you feel comfortable in the situation, you might ask a question such as Have you ever thought of why you feel this way? We can call there Outpatient Clinic (262) and ask for: a. Intake b.
