This leaflet contains information about the services we provide, how to make an appointment and who to contact for further information and assistance. Our guiding principles are professionalism, integrity, quality, choice and value. It is our aim to make your visit to our surgery a pleasant and pain free experience. We recognise that all patients are unique and try to respect your right to be treated as an individual. Our philosophy is based upon a dedication to enhancing your confidence and improving your dental health. Our aim is to achieve and maintain good dental health through preventative dentistry. This means providing you with informed choices and relevant advice allowing you to care for your teeth and so reduce the requirement for dental treatment in the future.

5 Services Available We have agreed with NHS England to provide NHS services for all patients. NHS dental treatment includes all treatment necessary to secure and maintain your oral health. As well as the full range of general practice treatments, we are pleased to be able to offer patients cosmetic dental treatment options. Specialist Services The practice is proud to offer the services of renowned visiting specialists in various fields of dentistry and facial aesthetic medicine within our specialist centre. We have a team of Dental Hygienists and Oral Health Educators to aid in the maintenance of your oral condition. A comprehensive range of Private, Cosmetic and Specialist options are available on request. We would be pleased to advise you regarding your choices at any time. It is never too late to improve your dental health. Many patients have not attended for some years, others are embarrassed by the state of their mouths. Regrettably, some people have had a previous bad experience. Facial aesthetica and the desire to ensure that we look our best are important to many. We understand your concerns. Do not worry, we welcome all patients. We have specialist facilities for nervous and phobic patients; the practice has invested in the latest technology and equipment to allow you access to advanced and painless treatment options both for dentistry and facial aesthetics. Our practice provides a calming atmosphere in stylish and contemporary surroundings. We have full disabled facilities with a designated parking bay, wheelchair ramp, dedicated toilet, non-slip flooring and ground floor surgeries with wide doorways. Our hygienist service forms the basis of our preventative care, as good oral hygiene is fundamental to dental health. A comprehensive range of oral care products is available at reception, we will be pleased to advise you as to the suitability of each for your needs. Childrenʼs teeth require careful monitoring. We encourage children to attend from an early age and we pay close attention to their growth thus allowing us to help them establish good dental care routines. We attempt to create an interesting and happy environment in which to undertake their care.

6 An attractive and healthy smile can improve self confidence and represents a benefit to any patientʼs health and aesthetics. A beautiful smile transcends fashion and is something that benefits the patient every day. We produce a separate brochure detailing the latest cosmetic treatment options available, covering both dentistry and facial aesthetic medicine. Please ask at Reception for your complimentary copy. Regrettably for some patients more extensive intervention is required due to the nature of their pre-existing dental disease. Many advanced disease cases are still treatable with modern materials and techniques. These are available through our specialist referral centre housed in the practice. Please request a free brochure on the clinic. Following the examination, a treatment plan will be agreed with you only after full consultation. We fully appreciate any apprehensions you may have about treatment and costs and would be pleased to clarify any issues before treatment continues. You will be provided with a written estimate of the costs involved in your care. Fees are payable as treatment progresses, with all balances due on completion. We accept cash, cheques and most major credit cards. Making an appointment If you ask us for an appointment, we will try to offer you one as soon as possible at a convenient time. You can choose which dentist you would like to see at the practice, including your regular dentist, but please note that this will depend on the appointments available. Appointments may be made by telephoning; / for Weston, for Ham Green, for Clevedon, or in person at the practices. Reminders and Recalls At the end of your course of treatment, your dentist will discuss with you when you will need to see a dentist again. NHS dentists now follow guidelines issued by the National Institute for Clinical Excellence (NICE). This means you will attend as often as is needed to keep your teeth and gums healthy and may no longer need a check up every six months. We encourage you to return for regular examinations and we promote continuing care.

7 Cancellations Please give us as much notice as possible if you have to cancel an appointment. We may make a charge if you do not give reasonable notice. Missed appointments waste NHS time. If you miss more than two appointments and give less than 24 hours notice, we may not be able to complete your treatment or offer you NHS care in the future. Opening Hours Our normal hours of opening are: Monday to Friday 8.45am pm and pm and Saturday 8.45am pm. Urgent treatment and out of hours care Urgent treatment means treatment that the dentist considers necessary to relieve severe pain or prevent your oral condition deteriorating before you can make a normal appointment. If you require urgent treatment please telephone the surgery as early as possible for advice. If treatment is essential, we will endeavour to arrange an appointment within 24 hours. For emergencies outside normal hours, please telephone the practice and you will receive instructions from our answerphone message. Alternatively you can contact NHS Direct on 111. Your rights and responsibilities You are entitled to A thorough examination of your mouth, teeth and gums A full explanation of your treatment options A written treatment plan (including costs)* Information about NHS charges displayed in the waiting room Advice on how to keep your teeth and gums healthy Information about this practice and the services available A care and treatment summary if you decide to transfer to another dentist Make a complaint if you are not happy with your treatment and care You are responsible for Giving at least 24 hours notice if you have to cancel or change an appointment. We will not charge you for missed appointments but if you miss more than two appointments we may no longer be able to offer you treatment. Following your dentistʼs advice to prevent tooth decay and gum disease Paying your bill promptly Bringing proof of entitlement when claiming help with the cost of NHS treatment. If no proof is shown your eligibility will be checked Treating our staff with courtesy and respect **

8 *Please note - if your treatment is a ʻBand 1ʼ course of treatment or you do not have to pay NHS charges, you will have to ask your dentist to provide this. ** We may refuse to treat patients who are violent, fail to pay their bills or refuse to co-operate during treatment. In this case we will inform the patient and NHS England. Your dental records Your dental records will remain confidential and secure. However from time to time we may need to release these to NHS England or the NHS Business Services Authority as part of our ongoing review process.
