1 PERSONAL MEDICAL ASSISTANT TERMS AND CONDITIONS The fllwing terms and cnditins shall gvern the prvisin f medical assistance t Yu (the Member ) in terms f the agreement entered int between First Natinal Bank Zambia Limited ( FNB ) and Speciality Emergency Services Limited ( SES ). Yu autmatically qualify fr this benefit depending n which qualifying cheque accunt Yu hld with FNB: QUALIFYING CHEQUE ACCOUNT MEDICAL ASSISTANCE BENEFIT Smart Cheque Accunt hlders SMS Advice Line Service FNB Gld Cheque Accunt hlders SMS Advice Line Service Talk Advice Line Ambulance Respnse Service Platinum Cheque Accunt SMS Advice Line Service Talk Advice Line Ambulance Respnse Service Mbile Medic DISCLAIMER FNB and SES recmmend that qualifying custmer d nt cancel any existing medical insurance cvers r arrangements that they might have withut first seeking prfessinal advice. NATURE AND CONDITIONS OF ASSISTANCE (the Assistance ) A. SMS Advice Line Service: Yu will be able t SMS SES n the designated number (767), and SES will respnd with First Aid medical advice. A.1. This advice will be in the frm f: A.1.1. Which is the mst apprpriate facility t visit, A.1.2. Which is the clsest facility t visit, A.1.3. Whether in the view f SES immediate treatment is necessary r nt, A.1.4. Hw t best manage Yur illness: This will nt be a diagnsis service, but Yu will be able t ask general questins that might be freely available frm nline/ffline media. The call centre will nt advise n pssible diagnsis r treatment ptins. A.2. Any messages sent t SES will be billed at nrmal rates by the caller s netwrk prvider. A.3. All SMSs sent by SES are paid fr by SES. A.4. N diagnsis will be dne ver the phne, as per HPCZ Guidelines. 1

2 B. Talk Advice Line Service: Yu will be able t call SES n the designated number (767), and SES will respnd with medical advice. B.1. This advice will be in the frm f: B.1.1. Which is the best facility t visit, B.1.2. Which is the clsest facility t visit, B.1.3. Whether immediate treatment is necessary r nt, B.1.4. Hw t best manage Yur illness: This will nt be a diagnsis service, but Yu will be able t ask general questins that might be freely available frm nline/ffline media. The call centre will nt advise n pssible diagnsis r treatment ptins. B.1.5. In the case f a Medical Emergency, SES will prvide an emergency respnse. B.2. Any calls made by Yu t SES will be billed at nrmal rates by the caller s netwrk prvider, B.3. All calls made by SES t the member are paid fr by SES, B.4. N diagnsis will be dne ver the phne, as per HPCZ Guidelines. C. Ambulance Respnse Service: Yu will be able t call SES n the designated number (767), and SES will respnd, in a Medical Emergency, with an Ambulance Respnse Service. C.1. The areas which this Ambulance Respnse Service is available in is: C.1.1. Lusaka, C.1.2. Livingstne, C.1.3. Kitwe, C.1.4. Ndla, C.1.5. and Slwezi. C.2. SES will nly respnd in these areas up t a radius f 50km frm the central pst ffices, as the crw flies, in the respective twns/cities and within the brders f Zambia. C.3. Any calls made by Yu t SES will be billed at nrmal rates by the caller s netwrk prvider. C.4. This service is based n availability which shall be mnitred thrugh a Triage methd. C.5. All calls made by SES t Yu are paid fr by SES. C.6. Yu will be required t prduce a valid FNB card that is linked t Yur FNB Gld Cheque Accunt r FNB Platinum Cheque Accunt at the time f the arrival n the scene, alng with a recgnised identity dcument. C.7. This service is available t the member and his r her immediate family (definitin parents, spuse, parents f spuse, children. D. Mbile Medic Service: Yu Yu will be able t call SES n the designated number (767), and SES will dispatch a Mbile Medic subject t the fllwing cnditins: 2

3 D.1. This service will nly be available between the hurs f 08:30 and 19:00, 7 days a week. D.2. The areas in which this service is available are: D.2.1. Lusaka, D.2.2. Livingstne, D.2.3. Kitwe, D.2.4. Ndla, D.2.5. and Slwezi. D.3. SES will nly respnd in these areas up t a radius f 25km frm the central pst ffices, as the crw flies, in the respective twns/cities and within the brders f Zambia. D.4. The Mbile Medic will be able t prvide the fllwing primary healthcare checks at yur residence r ffice. The Mbile Medic will be able t: D.4.1. Treat stings r minr bites, D.4.2. Treat minr flu-like illness, D.4.3. Treat minr injuries, D.4.4. Treat headaches and ther nn-specific pains, D.4.5. Treat fd pisning r minr infectins, D.4.6. Advise n the need and urgency t see a Dctr, based n the findings f the n-site clinical examinatin, and/r D.4.7. Call an SES ambulance, if the Mbile Medic in their sle pinin deems the incident t cnstitute a Medical Emergency. D.5. Any calls made by Yu t SES will be billed at nrmal rates by the caller s netwrk prvider. D.6. All calls made by SES t Yu are paid fr by SES. D.7. This service is based n availability which shall be managed thrugh the Triage methd. D.8. N diagnsis will be dne ver the phne as per HPCZ Guidelines. D.9. Dispensed drugs will be billed directly t Yu at the time f the cnsultatin. D.10. Yu will be required t prduce a valid FNB card that is linked t Yur FNB Platinum Cheque Accunt at the time f the cnsultatin, alng with a recgnised identity dcument. D.11. N children will be treated by a Mbile Medic. GENERAL TERMS 1. As the Assistance prvided by this agreement is t be rendered by SES n behalf f FNB t Yu, FNB shall nt be liable t Yu fr lss r damage f whatsever nature sustained by Yu as a result f the failure, fr whatsever reasn, f SES t render Assistance in terms f this agreement timeusly r at all r as a result f the manner in which such Assistance may be rendered by r n behalf f FNB r frm any ther cause whatsever, including negligence, missin and willful default. 2. FNB shall nt be liable t Yu fr the lss and damage caused r attributed t the negligence (whether grss r therwise), wrngful acts and/r missins f any f the Dctrs, paramedics, nursing staff r ther health care prfessinals r ther persns, whether r nt being in the emplyment f SES, wh may prvide direct r indirect services t Yu in terms f this agreement. 3. Neither FNB nr any persn required t render Assistance hereunder shall have any liability t Yu, yur heirs r executrs, arising ut f the failure t render Assistance r any delay in the rendering f such assistance, where such failure r delay is caused by vis majr, frce 3

4 majeure, adverse weather cnditins, strikes, lckut, labur disputes r unrest, rit r civil cmmtin and/r the refusal f a gvernmental/prvincial r lcal authrity t grant r allw the use f its services r facilities r where lcal laws, regulatins r functinaries limit the capacity f SES r any persn t render such Assistance. 4. Cmpliance by Yu with the terms and cnditins f this agreement shall be a cnditin precedent t any entitlement t Assistance hereunder. 5. FNB and SES shall nt be liable in terms f this agreement fr cnsequential lss f any kind. 6. Yu shall nt be entitled t cede, transfer r assign any f yur rights t Assistance in terms f these standard cnditins. 7. FNB and SES may, in cnsultatin and by agreement, amend any f these terms and cnditins at which time such amendment shall becme effective and binding n Yu. Such amendment shall nt cnstitute a nvatin f the terms heref. 8. These terms and cnditins cntain the entire agreement between Yu and FNB and neither party shall be bund by any undertakings, representatins r warranties nt recrded herein. 9. N indulgence, latitude r extensin f time either party (the grantr) may grant r shw t the ther shall cnstitute a nvatin f the terms and cnditins, nr shall it in any way prejudice the grantr r preclude the grantr frm exercising any f his rights in the future. 10. FNB has the right, at its discretin, t cancel r remve this service with 30 days ntice 11. The headings appearing in this agreement have been used fr reference purpses nly and shall nt affect its interpretatin. 12. Unless the cntext clearly indicates the cntrary intentin, wrds imprting - any ne gender shall include the ther; the singular shall include the plural and vice versa; 13. Definitins "Acute Illness" shall mean any sudden and unfreseen deteriratin f health which may in certain circumstances be ptentially life threatening. Ambulance respnse shall mean any Medical Emergency incident as defined in clause 1.1 that requires ambulance transprt. "Bdily Injury" shall mean any accidental, sudden, unfreseen and severe r vilent Physical Injury which is ptentially life threatening and ccurs at an identifiable time and place. "Dctr(s)" shall mean a persn wh: is a registered medical practitiner, duly qualified and licensed t practice medicine; is acting within the scpe f his/her license; Emergency Care means the rescue, evaluatin, treatment and care f an ill r injured persn in an emergency r medical crisis situatin and the cntinuatin f treatment and care during the transprtatin f such persn t, r between, health establishments. Emergency Care persnnel means persns registered with HPCZ as Emergency Care fficers wh prvide Emergency Care t the persn, with level f skill as Advanced Life Supprt, Intermediate Life Supprt and Basic Life Supprt. First Aid is the assistance given t any persn suffering a sudden illness r injury, with care prvided t preserve life, prevent the cnditin frm wrsening, and/r prmte recvery. FNB FIRST NATIONAL BANK ZAMBIA LIMITED HPCZ is the Health Prfessins Cuncil f Zambia 4

5 "Medical Emergency" shall mean any incident resulting in Bdily Injury r Acute Illness as defined herein, where, in the pinin f SES, the Member requires urgent ambulance respnse, hspitalisatin and/r treatment. Mbile Medic shall mean a suitably medically qualified persn wh is dispatched t a Member, fr a nn-emergency medical care t prvide medical treatment, advice and medicatin, as per SES Guidelines SES Specialty Emergency Services SES Guide lines shall mean any treatment and/r dispensing f medicatin, which has been set up and apprved by SES fr bth primary healthcare and emergency medicine. Triage means the prcess f determining the pririty f patients based n the severity f the cnditin. 5
