2 OCCUPATIONAL INJURY AND ILLNESS RECORDKEEPING In this section, information regarding requirements and standards for required records will be found. Time frames for recording and keeping records are also in this section. Injury and Illness Recordkeeping OSHA 300 Log of Work-related Injuries and Illnesses OSHA 300A Summary of Work-related Injuries and Illnesses OSHA 301 Injury and Illness Incident Report Optional Worksheet to help you fill out the summary 6-2
3 OCCUPATIONAL INJURY AND ILLNESS RECORDKEEPING 1. PURPOSE: In accordance with applicable requirements of the Department of Labor and Industries Division of Occupational Safety & Health (DOSH) standards, we will record all injuries and illnesses and report all work-related fatalities. 2. SCOPE: All employers covered by the Department of Labor and Industries Division of Occupational Safety & Health (DOSH) are required to keep injury and illness records. However, there are exceptions: A. Employers with 10 or less employees in previous year. B. Employer SIC Code, Standard Identification Code exempts them. C. All employers in industry classifications listed on Table 1 are exempt unless they have been specifically requested by DOSH, OSHA, or the Bureau of Labor Statistics (BLS) in writing even if they have 10 or fewer employees. 3. PROCEDURE: The OSHA Forms, 300, 300A and 301 will be maintained as follows: A. Maintain a Log and Summary of Occupational Injuries and Illnesses on OSHA 300 Form. Recordable cases include: a. Every occupational death b. Every occupational illness c. Every occupational injury or illness that involves: 6-3
4 1. Unconsciousness 2. Inability to perform all phases of the regular job. 3. Inability to work full time on a regular job 4. Temporary assignments to another job, transfers 5. Medical treatment OTHER than first-aid 6. Days away from work 7. A significant injury or illness diagnosed by a physician or other LHCP. B. Keep copies of all reports generated when an employee is injured on the job. C. Beginning February 1st of each year, post on the safety bulletin board the complete summary portion only, the OSHA 300A form for the previous years Form 300 entries to April 30th, a period of 90 days. D. Maintain all records, Forms 300, 300A and 301 or any acceptable substitute for five years following the year to which they relate. E. Recording Work-related Injuries and Illness a. Within 7 calendar days after you receive information about a case, you must decide if it is recordable b. Complete form 300, blocks A thru M as applicable c. Privacy concerns: These cases are considered private and the names of employees are excluded. These consist of the following. 1. An injury or illness to an intimate body part or to the reproductive system 6-4
5 2. An injury or illness resulting from a sexual assault 3. A mental illness 4. A case of HIV infection, hepatitis or tuberculosis 5. A needle stick injury or cut from a sharp object that is contaminated with blood or other potentially infectious material 6. Other illnesses, if the employee independently and voluntarily request that his or her name not be entered 7. The word privacy case will be entered instead of the employee name. You must maintain a separate confidential list by case number and employee names so the OSHA 300 form can be updated and be able to provide information to the government if asked to do so. d. Medical removal: If an employee is medically removed under the Medical /surveillance requirements, the case must be entered on the OSHA 300 log. e. Hearing loss: If an employee s hearing test (audiogram) reveals that a recordable threshold shift (RTS) has occurred, you must record the case on the OSHA 300 log. F. In addition to the OSHA 300, a supplementary record for each occupational injury or illness (OSHA 301) will be maintained. Other reports, such as worker compensation forms, are acceptable alternatives for the OSHA 301 if they contain the information required by the OSHA 301. ("Employer's Incident Investigation Report"). 6-5
6 4. RESPONSIBILITY: The individual or function responsible for maintaining records and ensuring proper posting is: name and/or title 5. CERTIFICATION: The OSHA 300A must be certified by the owner, executive officer or authorized individual before posting. 6. FORMS: All the necessary forms that are required are included with this program.
