1 Amarillo Legacy Medical ACO Wellness Screenings for AISD We are honored to be selected by AISD to perform the annual wellness screenings. In order for your wellness visit to be as fast and efficient as possible, we ask that you register and make appointments online. This will only take a few minutes of your time. Amarillo Legacy Medical Accountable Care Organization is a group of independent medical practices that have joined together and are working to improve the health of our individual patients, improve the health of our population, and lower health costs. You may schedule your wellness visit at one of the following 5 locations: Amarillo Family Physicians, PA 1215 S. Georgia Amarillo, TX CareXpress Urgent Care at Summit / 34 th 7306 SW 34 th, Suite 3 Amarillo, TX CareXpress Urgent Care in Canyon rd St Canyon, TX STEP 1 : PRE-REGISTRATION The link to our pre-registration and lab report portal is: While your visit may take place at any of these locations, your data will be located with Amarillo Medical Specialists. If you are already a patient with Amarillo Medical Specialists, and you know your patient portal login information, you may use your existing username and password to skip the pre-registration step, and go directly to the appointment reservation.

2 If you are not a current patient at Amarillo Medical Specialists, pre-register at portal.aisdhealth.com and click on the gray button labeled Pre-Register Please fill in the registration information in all three tabs: Personal Details, Emergency Contact, and Employer. Please note that we need a working address in order to send you the login information to schedule your appointment. We will complete your registration and send you your credentials by within 2 business days. STEP 2: CREATE YOUR APPOINTMENT You will receive an with your user name and temporary password. Your account will not be fully activated until you log in and create an appointment. A) Login at Wellness visits will begin on June 10, We will open the scheduling program on or before June 3. You will initially need to accept the terms of service, and choose a new password to activate the online services. One you have changed your password, you will see the following home screen. B) On the left column, click on the choice New Appointment

4 C) Click on the drop-down menu under FACILITY and CHOOSE YOUR PREFERRED FACILITY For each FACILITY, there is only one PROVIDER, but the scheduling program requires you click on the PROVIDER name to choose them. Then Select the VISIT TYPE as AISD Screening.

5 Choose the preferred date of your screening, then click on The system will take about 10 seconds to search for a time, and then will display the ten earliest times available on the date for location you selected. If there are no remaining times available, it will go to the next available date. You can choose to MODIFY SEARCH CRITERIA if you want to change the location or date. Choose the time that you prefer best. After you choose, you will be asked to confirm your choice, and when you do, you will get a confirmation screen: If you need to CHANGE or CANCEL your reservation, simply log back into the web portal, and select UPCOMING appointments. From there you can cancel your appointment. Then go to New Appointment to schedule a new visit time.

6 3) YOUR WELLNESS VISIT At your visit we will ask you for your written consent for your testing and sharing the results with AISD Benefits Department and Blue Cross. Additionally, we will include your results in the ALMA Health Information Exchange so that your participating doctor can also see your test results, at no additional charge. If you do not wish for your information to be included in the Health Information Exchange, you may sign a form to opt out. We will be measuring your blood pressure, height, weight, and collecting a blood sample. One of the main components of your testing is blood sugar and cholesterol, which are both affected by recent eating. Please do not eat anything, and drink only water for the 10 hours before your test. You will be asked if you use any tobacco products. If you indicate that you do not, a urine sample will be collected to confirm that. Please be aware that the urine nicotine test is very sensitive and if you inhale a large amount of second hand smoke it can result in a positive test. You will also be asked to complete a 2 page health questionnaire. The lab results will be available in about 1 or 2 business days, and you will receive an when they are completed. You may use your web portal login information to retrieve them. Once your lab tests have been completed, we will forward your information to AISD Benefits Department and Blue Cross.
