3 What s New: Medical premiums will increase 19.8% Blue Access is the member portal for BCBSTX Blue Points offers health and wellness rewards at no additional cost Avesis will continue as our vision carrier, with no change in coverage and a small premium increase Changing Your Elections Section 125 plans are governed by the IRS. As such, participants cannot make changes to their annual elections after open enrollment. However, there are limited circumstances which allow participants to make election changes. Your benefit changes must be consistent with your family status change. The changes will become effective on the date of the event, provided you notify Human Resources within 30 days of the event. How to Make Family Status Changes 1. Following a change in family status, you must contact Human Resources to change your benefits. 2. You will need to provide satisfactory proof of the event (i.e. marriage certificate, divorce decree, certificate of coverage, birth certificate, HIPAA certificate, etc.) 3. The deadline for making changes is within 30 days of the event. 4. If you do not meet the 30 day deadline, you must wait until the next open enrollment period or wait until there is another family status change. Employee Benefits Overview 2

8 LIFE and AD&D INSURANCE Lincoln Financial BASIC LIFE/AD&D # CentroMed provides full-time employees with a Life and Accidental Death and Dismemberment (AD&D) benefit of 2 times annual earnings up to $500,000 in increments of $1,000. This benefit is provided at no cost to the employee and requires no medical evidence of insurability. VOLUNTARY LIFE/AD&D # You can purchase additional life and accidental death and dismemberment coverage for yourself or your eligible dependents. You can purchase up to 5X annual salary or $500,000. The Guarantee issue is $150,000. Spouse coverage can be up to 50% of your benefit amount. Guarantee Issue is $25,000 for your spouse. Evidence of Insurability (EOI) is required for late enrollees, new employees requesting greater than the guarantee issue amount or for those wishing to increase their life insurance. Lincoln has an online EOI application which is accessible via the enrollment website. You are encouraged to review your beneficiary information periodically, and update it as necessary. You may change your beneficiary at any time, not only at open enrollment.

10 FLEXIBLE SPENDING ACCOUNTS (FSA) TASC # Flexible Spending Accounts (FSA s) provide you with a means to pay for out-of-pocket medical/dental expenses and dependent care expenses with pre-tax dollars. You can elect to have money deducted from your paycheck on a pretax basis. Since this money is taken out of your check BEFORE you pay taxes, you are taxed on a smaller amount. The money in your Health Care spending account will be set aside to reimburse eligible medical, dental and vision expenses. The money in your Dependent Care spending account allows employees to use pre-tax dollars towards qualified dependent care such as caring for children under age 13 or caring for elders. There are two (2) types of FSA accounts: 1) Health Care Account and 2) Dependent Care Account. Each of these accounts is separate, and you can choose to participate in both the Health Care and Dependent Care accounts, only one of the accounts or neither. The rules and regulations of the Internal Revenue Service (IRS) govern all FSA accounts. The IRS sets specific guidelines as to what is considered an eligible expense for reimbursement. The elections you make will become effective 07/01/2013 and will remain in effect until the end of the plan year, 6/30/2014. The maximum amount you may contribute to the Health Care Account for the plan year is $2,500. The maximum amount you may contribute to the Dependent Care Account for the plan year is $5,000. The minimum contribution amount for the plan year is $240. Example: $240 / 24 = $10 per paycheck SUPPLEMENTAL BENEFITS Aflac #6255 CRITICAL ILLNESS A lump sum critical illness insurance policy provides a cash benefit if you re diagnosed or treated for critical illness events. The benefit is triggered by covered events, such as: Heart Attack Major Human Organ Transplant HOSPITAL INDEMNITY INSURANCE A hospital confinement indemnity insurance policy provides cash benefits to use as you see fit. The benefits are predetermined and paid regardless of any other insurance. Some advantages of Aflac hospital confinement indemnity insurance includes: Stroke Paralysis Coverage for in- and out- patient procedures No Precertification or Deductibles End-Stage Renal Failure Cancer Portabiity Coma No network required ACCIDENT INSURANCE These benefits can be used to cover out-of-pocket expenses and the cost associated with life changes following a covered illness. When a covered accident occurs, the last thing on your mind is the charges that may be accumulating. These costs add up fast. Aflac will send cash benefits directly to you (unless you tell us otherwise) and you decide the best way to spend them. The cost will be displayed in the enrollment system.

11 EMPLOYEE ASSISTANCE PROGRAM CentroMed provides an Employee Assistance Program (EAP) to help our employees who face personal matters that adversely affect their job performance or personal lives. Participation in the program is totally voluntary. The EAP offers a total of six confidential therapeutic counseling visits per year at no cost. The visits may be used by the employee, their family members or a combination of both, but may not exceed six visits in a 12-month period. The services are available at any CentroMed site which offers behavioral health services. If continuation of services is needed after six visits, sessions may continue on a self-pay basis or insurance, if accepted, may be used.
