5 Other Voluntary Benefits Voluntary Benefit Plans Universal Protection Service offers voluntary insurance plans through UNUM Provident: Voluntary Whole Life Insurance, Short Term Disability, and new benefit for 2005 Accident Insurance. The individually owned policies are paid 100% by the employee, and you may make through the convenience of payroll deductions. You are eligible to enroll during Open Enrollment at which time details of these plans and rates, based on the level of coverage you elect, will be provided to you. Employee Assistance Program New Benefit as of January 1, 2005 All Universal Protection Service employees and their family members have unlimited telephonic access 24 hours a day, seven days a week, 365 days a year. Members can contact a licensed staff clinician for crisis situations. There are no costs to the employee or their family members for accessing EAP services. All information is absolutely confidential, with no specific information is reported to the employer. Every day life problems that may be discussed, but not limited to: Marital Issues, Stress, Job-Related Problems, Alcohol & Drug Dependency, etc. 401(k) Plan The Company is pleased to provide employees with the opportunity to save for their retirement on a pre-tax basis through our 401(k) retirement plan. TransAmerica administers this benefit and there are several investment options and contribution plans available. You are eligible to begin participation following six months of continuous service. You may contact TransAmerica or your branch HR Representative directly to enroll once you become eligible.
6 Paid Time Off Anniversary Bonus We recognize your longevity and service to us by offering you an annual Anniversary Bonus. This bonus is paid annually to those employees once they complete a full year of continuous service. The bonus is paid out on your anniversary date with the Company and is our way of saying thank you for your contribution during the year. (Note: some union accounts and non-union accounts pay for vacation time for our officers, which is in lieu of the anniversary bonus.) Leaves of Absence The Company provides a number of leaves of absence to eligible employees, including Family and Medical Leave, pregnancy disability leave, California Family Rights Leave (including baby bonding time), school activities leave, jury duty leave, and military leave. If you have a situation that may require you to be absent for an extended period of time, contact our Corporate HR department to discuss your situation and what leaves may be available to you. Flexibility We offer flexibility in schedules and assignments. Our client sites need protecting 24/7. That means we can offer flexible schedules, whether you are looking for full-time, part-time, evening or weekend hours, day, swing or graveyard shifts. Our clients include professional high-rise buildings, retail and industrial sites, entertainment venues, public, government, educational and healthcare facilities, and gate-guarded homeowner associations. This variety provides you with many different settings and environments to choose from, depending on your interests and availability. And if your situation changes during your employment, talk with us and we will do our best to find a new schedule or location that works with your needs. Training and Development We offer on-the-job training and continuous education opportunities. We provide all the necessary skills and training to be a Universal Security Professional. Our training includes a hands-on orientation process, site-specific security and safety training, a year-long, formal bi-weekly training program on critical security topics, and a unique Certified Security Professional Program that provides advancement opportunities for those who wish to continue their professional training and development in the security field.
7 Enrollment Eligibility Full-time employees who work an average of 30 or more hours per week are eligible to participate in our health insurance benefit plans. Eligibility generally begins on the first day of the month following six (6) months of continuous full-time service. Cafeteria Plan Universal Protection Service offers benefits through a Section 125 Cafeteria Plan, which allows you to make your benefit premium payments on a pre-tax basis through convenient payroll deductions, thereby reducing your taxable income and saving you money. When you sign up for your benefits you will have the option to have your premiums deducted from your paycheck on a pre-tax basis. The IRS strictly regulates the program, so enrollment in and out of our benefit programs is very regulated. Changes to your benefits may only be made during Open Enrollment or if you experience a Qualifying Event as defined by the IRS. Enrollment You will be provided with enrollment information during the Open Enrollment Period, prior to your eligibility date. You must either sign the ENROLLMENT form electing benefit coverage, or sign the BENEFIT WAIVER, declining all coverage. If you do not make an election, your benefits will remain the same as the prior year with the corresponding change in payroll deductions. If you do not elect coverage by the end of your waiting period, you must wait until the next Open Enrollment period, a year from now. Making Changes to Your Benefits Once you are enrolled in the benefit program, you may not make any changes to your coverage (add/change or drop coverage or dependents) except during Open Enrollment, which is conducted once per year. The only exception is if you have a Qualifying Event such as the birth or adoption of a child, marriage or divorce to add or drop a spouse, loss of other coverage (for example, you may add your spouse if he/she loses other coverage from his/her employer), or you experience a reduction in your work hours making you ineligible for continued coverage. If you have questions at any time during your employment about your benefit plans, your eligibility or if you need assistance with membership, please contact your Human Resources.
8 Questions & Answers Changes that can be made effective January 1, 2005: Change Kaiser medical plans (i.e., HMO-High to HDHMO-Low). Change PacifiCare dental plans (i.e., HMO to PPO, or PPO to HMO). Enroll in the PacifiCare vision plan. Enroll or terminate individual and/or dependent coverage in the medical/dental/vision plans. Add or make changes to the UnumProvident Voluntary Benefit plans including the new Accident Insurance. Forms to be completed IF making changes or first time enrollment: Your Universal Protection Service Personalized Benefit Worksheet and Enrollment Form has all the benefit choices included. What Forms MUST be completed? Complete your personalized Benefits Worksheet noting enrollment choices and your new costs. If you do not fill out a form, you will continue with your same elections. If you did not enroll in this past year you will again miss the opportunity for benefits. Where do I find these forms? Forms are available at the enrollment meeting, mailed to those with no meeting available, or thru Human Resources. When are the forms due and where do I return them? Enrollment forms can be completed at your benefit meeting with an enrollment specialist and turned in then. All forms are due by Monday, November 1 st and must be returned to Human Resources. Who do I contact with questions?
