1 DUAL ENROLLMENT ADDENDUM Course information The following information identifies the requirements for each course delivered as dual enrollment at a high school. As part of the Memorandum of Understanding for dual enrollment courses: Each district s responsible personnel (noted below) must review and agree to each item (initial and date to signify). When item is completed, at least one person must initial and date on that line. Zane State College must receive a copy of the completed Dual Enrollment Addendum for each course within one week of course start date.
