1 OCTOBER 2014 COBRA 2015 Annual U.S. Benefits Enrollment Bulletin 2015 Annual Benefits Enrollment Ends November 7, 2014 Welcome to the annual enrollment period for the JPMorgan Chase 2015 U.S. Benefits Program. You have the opportunity once a year to review your Health Care and Insurance Plan coverage and make any necessary changes. Now is the time to review your current benefits and your 2015 options. While we re not making significant design changes to the Medical Plan for 2015, we encourage you to review your current Medical Plan options including your health care company to be sure it continues to meet the needs of you and your family. Your Enrollment Checklist on page 7 will guide you through making your decisions for Beginning January 1, 2015, JPMorgan Chase medical, prescription drug, dental, and vision plan options will be replaced for Medicare-eligible participants and Medicare-eligible covered family members with coverage offered through OneExchange, the country s largest and most experienced private Medicare Exchange provider. More information on this change can be found on page 2. 2 Move to OneExchange for Medicare-Eligible Participants New Approach to Health Care Coverage for Medicare-Eligible Participants As previously communicated in late August 2014, beginning January 1, 2015, JPMorgan Chase will partner with OneExchange, the country s largest and most experienced private Medicare exchange. The health care plan options available through OneExchange (medical, prescription drug, dental and vision coverage) will replace the current JPMorgan Chase health care plan options for Medicare-eligible retirees and individuals on long-term disability (LTD). A new election through OneExchange is required by all Medicare-eligible participants by December 31, Enrollment through OneExchange is October 13 through December 31, OneExchange has sent enrollment materials to those currently eligible for Medicare. There will be separate elections for Medicare-eligible participants and their Medicareeligible family members as the coverage through OneExchange is individualized coverage. OneExchange will not provide coverage for you and/or your covered dependents until you and/or your covered dependents become eligible for Medicare. Medicare-eligible participants who are currently eligible for and receiving a subsidy for medical coverage, based on the rules in effect at the time of their retirement, will be provided with an annual contribution to an account called a Health Reimbursement Arrangement (HRA) on January 1, 2015, after they have enrolled in medical coverage with OneExchange. This account can be used to offset eligible medical, prescription drug, dental and vision premiums, and other medical, dental, and vision out-of-pocket costs. If you have questions, OneExchange can be reached toll-free at , 8 a.m. to 9 p.m., Eastern Time, Monday through Friday. You may also access the OneExchange/JPMorgan Chase website at If You Are a Retiree and Currently Enrolled in COBRA If you are retiree, and currently deferring any retiree coverage (medical, dental, and/or vision), you need to elect retiree coverage effective January 1, 2015, and drop your COBRA coverage. This is a one-time opportunity to enroll in JPMorgan Chase retiree benefits for January 1, There will no longer be a deferral opportunity for retiree medical, dental, and/or vision. If you are on COBRA due to severance and your COBRA subsidy continues into 2015 due to your severance, you may continue on COBRA coverage until the subsidy runs out (up to six months after being on COBRA coverage depending on your years of service). Once the subsidy expires, or January 1, 2015, whichever is later, you have a one-time opportunity to elect retiree healthcare coverage through JPMorgan Chase Medical Plan (if you are pre-medicare) or through OneExchange (if you are Medicare-eligible) for medical, prescription drug, dental and/or vision coverage. After your COBRA subsidy expires, you can continue with COBRA coverage for the remainder of your COBRA period (generally 18 months) paying the full cost of coverage. However, at the end of your COBRA coverage, you will not be able to continue coverage through JPMorgan Chase (or receive an HRA with OneExchange). 2 COBRA 2015 Annual U.S. Benefits Enrollment Bulletin

3 What s Changing for 2015 Medical You will continue to have access to the Consumer Driven Health Plan (CDHP) Option 1 and Option 2, offered through Cigna and UnitedHealthcare (UHC). CDHPs encourage participants to become better consumers of health care services and take a more active role in managing and improving their health. To get the most out of these types of plans, participants need to be more involved for example, by researching treatment alternatives and provider quality and taking part in wellness programs to improve and maintain their health Medical Plan Changes We ll be making these changes to the Consumer Driven Health Plan (CDHP) Medical Plan options: Complete your biometric Wellness Screening and online Wellness Assessment by December 31, 2014 to earn $200 for your 2015 Medical Reimbursement Account (MRA). When you choose to complete both a Screening and Assessment between September 1 and December 31, 2014, you will earn $200 in your MRA. If your covered spouse/domestic partner completes both activities within this timeframe, you will get $100 in your MRA. o o Information on how to get a biometric Wellness Screening and take the online Wellness Assessment at your health care company s website can be found on My Health. There is no change to your total available MRA funds for 2015: $1,000 ($1,400 if you cover a spouse/domestic partner). Starting January 1, 2015, you can go on to earn $800 ($200 each) for four Additional Wellness Activities you complete in 2015, once your Wellness Screening and Wellness Assessment are completed. Your covered spouse/domestic partner can complete three Additional Wellness Activities ($100 each) to earn $300 for your MRA during Out-of-pocket in-network coinsurance maximum under Medical Plan Option 2 will be reduced in 2015 to comply with limits required under the Affordable Care Act (ACA). Because our Medical Plan has higher coinsurance maximums for those with higher pay, to comply with these limits, the out-of-pocket coinsurance maximum will be reduced from $3,500 to $3,050 for Participant Only coverage and from $7,000 to $6,100 for Family coverage for participants who earned $60,000 or more in Total Annual Cash Compensation when employed by JPMorgan Chase. Once you reach your out-of-pocket coinsurance maximum (the most you pay out of pocket during the year-or safety net ), JPMorgan Chase pays 100% of the cost of care for the rest of the year for eligible medical expenses. Increased Total Lifetime Infertility Benefits when you use a Center of Excellence (COE)-$30,000 for a COE versus $10,000 for a non-coe. Please contact your health care company at the number on the back of your ID card for more information. Prescription Drug Plan Your prescription drug coverage is part of the Medical Plan and is administered by CVS Caremark. Though it is part of the Medical Plan, it has a different plan design, with separate deductibles and a separate safety net in the form of per-prescription maximums and annual out-of-pocket maximums. The CDHP plan offers free preventive generic drugs, a flat copayment for generic drugs, coinsurance for brandname drugs (lower coinsurance for preferred brand-name), and the option of having long-term prescriptions filled through a convenient mail-order program or at an in-network retail pharmacy. 4 2015 Prescription Drug Plan Changes 1. The CVS Caremark covered drug list is changing for With this change, approximately 100 drugs and drug supplies (e.g., certain diabetic test strips) that have either a generic or cost-effective therapeutic equivalent will no longer be covered under the Prescription Drug Plan and you will pay the full cost of the drug if you continue to take it in For a list of excluded drugs, go to My Health > Resources > 2015 Drug Exclusion List. Participants who are currently taking a drug that will no longer be covered, along with their doctors, will be notified on this change in late-october and December Please Note: An independent committee made up of pharmacists, physicians, and medical ethicists review and approve the drug list (also known as Formulary). 2. The maximum out-of-pocket costs for prescription drugs will be lower. As a result of regulations under the Affordable Care Act (ACA), the prescription drug co-insurance maximum will be reduced from $2,000 for Participant Only/$6,000 for Family coverage to $1,000 for Participant Only/$1,950 for Family coverage (does not include retail deductible). Once you reach your out-of-pocket coinsurance maximum (the most you pay out-of-pocket during the year), JPMorgan Chase pays 100% of the cost of covered prescription drugs for the rest of the year. 3. Certain specialty self-administered drugs will continue to be covered by the prescription drug plan but excluded from the Medical Plan. Certain self-administered specialty drugs, which can be administered orally, via inhalation or self-injected (such as Forteo and Procrit), will be covered by the Prescription Drug Plan instead of the Medical Plan. Participants who are currently taking medications affected by this change will be notified in early November 2014 to set up medications for delivery by Caremark Specialty in Your Other Benefits Take this chance to review your dental and vision elections and related costs, which may have changed, to make sure they are still the best options for you and your family in A WORD ABOUT DEPENDENT ELIGIBILITY If you are eligible for benefits, generally your dependents are also eligible. Children are eligible until the end of the month in which they turn age 26. For more on eligibility requirements, access the 2015 Dependent Eligibility Requirements via My Health. IMPORTANT REMINDER ABOUT PAYING FOR YOUR COBRA COVERAGE Your monthly payments are due on the first day of each month. To ensure that payment is received by the due date and to avoid cancellation of your COBRA coverage, you may wish to consider having your payments deducted directly from your checking or savings account via direct debit, if you have not already done so. Please call the accesshr Contact Center for more information (see page 12 for contact information). Please Note: If you fail to remit your monthly payment within 30 days of the due date, your coverage will end retroactive to the last day of the month for which payment was received. 5 How You Earn 2015 MRA Funds JPMorgan Chase believes that completing a biometric Wellness Screening and online Wellness Assessment questionnaire are key steps in your journey toward getting and staying healthy. Together, they can help you get a better understanding of any health risks you may have that could lead to a condition and the sooner a condition is diagnosed, the sooner you can treat it. That s why, for the past three years, we ve rewarded you for getting a biometric Wellness Screening and completing an online Wellness Assessment by contributing funds to your Medical Reimbursement Account (MRA). Because we feel that getting this snapshot of your health is so important, in 2015, if you complete a Screening and Assessment between September 1* and December 31, 2014, you ll earn $200 in 2015 Medical Reimbursement Account (MRA) funds (and get another $100 if your covered spouse/domestic partner also does these activities between September 1* and December 31, 2014). *Please note: Wellness Screenings completed between March 1 and August 31, 2014, also count toward earning 2015 MRA funds. After getting a Wellness Screening and completing an online Wellness Assessment questionnaire, you can earn $200 for each Additional Wellness Activity you complete, up to $800, between January 1, 2015, and December 31, 2015 (plus $100 each, up to $300, when your covered spouse/domestic partner completes activities during 2015 (after he or she completes a Wellness Screening and Wellness Assessment). UNDERSTAND HOW THE BIOMETRIC WELLNESS SCREENING AND ONLINE WELLNESS ASSESSMENT WORK A biometric Wellness Screening provides overall key indicators of your health. Screenings give you your blood pressure, blood sugar, cholesterol, triglycerides, and body mass index (BMI) numbers. The online Wellness Assessment is a simple health and lifestyle questionnaire that takes about 15 minutes to complete. Together, your Wellness Screening and Wellness Assessment results provide you with helpful information about what you re doing well, recommendations for improving your health, and potential issues to discuss with your doctor. If you complete both the Wellness Screening and Wellness Assessment by December 31, 2014, funds will be credited to your MRA in mid-january To see if you ve already completed your biometric Wellness Screening and online Wellness Assessment, go to My Health > View my MRA Tracker. So, when you complete these wellness activities, you can earn a maximum of $1,000 in your MRA for 2015 (plus $400 more if your covered spouse/domestic partner completes these activities). DON T FORGET MRA CARRYOVER FUNDS When planning for 2015, don t forget that any unused funds in your 2014 MRA on December 31, 2014, will carry over to your 2015 MRA on January 1, 2015, and may be used for eligible 2015 expenses as long as you are covered under the JPMorgan Chase Medical Plan. IMPORTANT NOTE ABOUT YOUR MRA FUNDS IF YOU CHANGE YOUR HEALTH CARE COMPANY If you decide to change your health care company for 2015 (from Cigna to UnitedHealthcare or vice versa), you ll also change the company that administers your MRA. It s important to note that there will be a delay in transferring your 2014 unused MRA funds to your 2015 MRA at your new health care company. This delay allows your 2014 health care company access to 2014 funds in your MRA to pay any late year 2014 medical and pharmacy claims that are processed in the first four months of If this causes you a financial concern, contact your 2015 health care company who can work to move those funds faster. Get a physical, cervical or prostate cancer screening, mammogram, or colonoscopy Participate in Health Coaching Program, such as stress and weight management, nutrition, maternity support, condition management, and/or treatment decision support Biometric Outcomes: Body Mass Index (BMI) under 25** or progress toward it as defined by your health care company New! Blood Pressure under 140/90** New! Choosing Care Wisely online program-become a good health care purchaser using tools and resources available on your health care company s web site New! Personal Action Call with your health care company to discuss Wellness Screening & Assessment results, understand tools & resources available. TOTAL 2015 MRA funds that can be earned $200 $100 Take advantage of all the age- and genderappropriate screenings that apply to you (each are covered at 100% in-network), but you can only receive funds once a year for this activity. $200 $100 You can complete up to four telephonic Health Coaching Programs; spouse/domestic partners up to three. 7 Important Steps to Take: Your Enrollment Checklist Enroll for your 2015 benefits by November 7. Go to My Health and click Enroll now to make your 2015 benefits elections. You can access My Health via the internet at myhealth.jpmorganchase.com. Review your Medical Plan options, and elect the coverage that is best for you and your family. Compare your options both within the JPMorgan Chase Medical Plan and with any other plans available to you, such as through your spouse/domestic partner s employer or former employer, or other options available to your child(ren). Take everything into consideration when making your choice. Go to My Health and click Enroll now to access the Benefits Web Center where you can review the Health Plan Comparison Charts and compare the costs and coverage provided under each option. Decide whether your health care company is still the right fit for you. Both Cigna and UnitedHealthcare (UHC) offer the same design and monthly contributions to COBRA participants. Check out the tools, resources, and wellness programs each health care company offers, and whether your doctors are in their networks. You can access the health care company websites through My Health, or through the Benefits Web Center. Review the way you pay for health care through your MRA automatic claim payment or debit card-to see if it s still the best approach for you. See the Helping You Choose: Automatic Claim Payment vs. Debit Card Tip Sheet on My Health for details. Take a look at your dental and vision coverage to be sure they still meet your needs. If you re happy with them, you don t need to do a thing, but it s always good to revisit them once a year, especially since they can provide valuable coverage for you. ELECTIONS LAST ALL YEAR Elections you make during this annual enrollment period will become effective January 1, 2015, and remain in effect through December 31, The only time you can make a change during the year is if you have a qualified status change, like getting married or having or adopting a baby. In the event of a status change, any resulting coverage changes must be made within 31 days after the date of the event. Call the accesshr Contact Center right away if you experience a qualifying event. If you don t make changes for 2015, generally your elections for 2014 will continue at 2015 rates. Update your dependent information. Your Personalized Fact Sheet includes the information currently on file for you and your covered dependents. Please review this information carefully, including your dependent information (e.g., date of birth, SSN), and make any necessary changes by visiting the Benefits Web Center via the "Enroll now" link on My Health. To review JPMorgan Chase s dependent eligibility requirements, please visit My Health > Resources. Be sure your dependents information is correct. If you cover dependents under your JPMorgan Chase plans, be sure their name, date of birth, and Social Security Number are up to date. This information is important for us to have and is required by the Affordable Care Act (ACA). Review the 2015 Domestic Partner Tip Sheet, if applicable, on My Health. It describes the eligibility requirements and enrollment process for domestic partner coverage. Plan how you will build your 2015 MRA by selecting the wellness activities in which you want to participate. The Initial Wellness Activities (biometric Wellness Screening and online Wellness Assessment questionnaire) must be completed between September 1 and December 31, You have the opportunity to earn up to $1,000 in funds for your 2015 MRA, as well as up to $400 in funds for your 2015 MRA when your covered spouse/domestic partner completes wellness activities within this timeframe all of which you can use to pay for eligible out-of-pocket medical and prescription drug expenses in See page 5 for details. 8 Understand the rules for mid-year changes. If your family situation changes, you may be eligible to make changes to your benefits as follows: Qualified status changes: The annual benefits enrollment period is the only time you can enroll in or make changes to your benefits during the year, unless you experience a qualified family status change (such as gain coverage from another source or divorce). Take advantage of tools to help you choose. See page for details about the tools available through My Health and the Benefits Web Center to help you make your 2015 enrollment decisions. Electing Onsite Health & Wellness Centers, Wellness Screenings, and Employee Assistance Programs (EAP) You may elect access to JPMorgan Chase s onsite Health & Wellness Centers, participate in Wellness Screenings, and utilize EAP while on COBRA. These benefits are offered as a single election. If elected, all covered dependents may access EAP, but Wellness Screenings are available only to former employees and their covered spouse/domestic partner. Access to JPMorgan Chase s onsite Health & Wellness Centers is only available to former employees. If you are interested in participating, please contact the accesshr Contact Center and speak to a representative. NEED MORE TIME TO MAKE DECISIONS? Your Medical Plan At-a-Glance The JPMorgan Chase Medical Plan is designed to help you manage your health by providing tools and resources that help you make better decisions about your health and the care you and your family receive. The key components of CDHP Option 1 and Option 2, which continue in 2014, include: You use the money in your Medical Reimbursement Account (MRA) to pay for eligible out-of-pocket medical and prescription drug expenses. You earn funds in your MRA by participating in certain wellness activities. Remember that any funds that remain in your MRA at the end of 2014 carry over to In-network age- and gender-appropriate preventive care is covered at 100% with no deductible. This includes physical exams, recommended screenings, and immunizations. In-network primary care office visits are covered at 90% with no deductible. This includes visits to a family practice doctor, internist,0f1 pediatrician, and OB/GYN, as well as visits to convenience care clinics. Other in-network medical care has a deductible. The Plan generally pays 80% of costs for other care, such as specialist visits, hospital stays, labs and outpatient facilities after an annual deductible is met. Out-of-network care does not require a referral, but you may want to check with your health care company to assure services are covered. You will pay 100% of the cost for services for out-of-network doctors, until you reach a separate, higher out-of-network deductible. After this deductible is met, out-ofnetwork eligible medical services are covered at 60% up to the reasonable and customary (R&C) limit. You are responsible for amounts above the R&C limit. 1 Internists must be contracted with Cigna or UHC as primary care physicians (PCPs). 9 The Plan s coinsurance maximum gives you a financial safety net that limits the amount you have to pay in coinsurance each year. Prescription drug benefits are part of medical coverage and are provided through CVS Caremark. This benefit has a separate retail deductible for brand-name drugs and a separate safety net in the form of perprescription maximums and annual out-of-pocket maximums. Eligible generic preventive drugs are covered at 100% (no deductibles or copayments/coinsurance). You pay flat copayments for generic drugs and different coinsurance percentages if you use preferred or non-preferred brand-name drugs. Learn more about your Medical Plan and how it works on My Health, where you can access resources and link to the health care companies websites or click Enroll now to reach the Benefits Web Center for tools such as the Health Plan Comparison Charts. WHAT S THE DIFFERENCE? Keep in mind that the key difference between CDHP Option 1 and Option 2 is that Option 1 has higher monthly contributions and a lower deductible and coinsurance maximum, while Option 2 has lower monthly contributions and a higher deductible and coinsurance maximum. Make Sure Your Provider Is In-Network It s important to check to see if your providers are in-network to receive the most cost-effective level of benefits provided by your Medical Plan option. Go to My Health > 2015 pre-enrollment sites. You can visit My Health via the internet at myhealth.jpmorganchase.com. And, you may want to check with your provider to make sure he or she plans to stay in-network for Please Note: Providers can change networks at any time. If your provider leaves a network, it does not qualify as an event that allows you to change health care plan options or company during the year. Your Wellness Program Administrator The health care company you choose for your Medical Plan (Cigna or UHC) will administer wellness programs, such as health coaching and condition management. Please Note: Your health care company may contact you if they think you can benefit from their programs. A health coach can help you manage your health by explaining Wellness Assessment results, helping you set and reach goals, assessing treatment options for certain conditions, or giving advice on preventive care. If you have a chronic condition such as asthma, diabetes, heart disease, or low back pain, the Condition Management Program can provide you support from a registered nurse to help manage your personal condition and coordinate treatment with your doctor. Your Privacy Is Important The privacy of your health information is important to you-and JPMorgan Chase. You can be sure that your personally identifiable health information, including Wellness Screening results and Wellness Assessment answers, is not shared with JPMorgan Chase. Your information is protected and secure, and complies with privacy laws, like those outlined in the Health Insurance Portability and Accountability Act (HIPAA). 10 Tools to Help You Choose and Enroll My Health: Your Centralized Online Resource In addition to this Annual Benefits Enrollment Bulletin, several resources are available on My Health to help you during this benefits enrollment, including a link to the Benefits Web Center, where you will make your enrollment choices. Getting to My Health is easy; you can access the site anytime, and the Single Sign-On feature gets you around without having to re-enter passwords. If you want to make changes or use the online tools, you can access My Health via the internet at myhealth.jpmorganchase.com and select COBRA Participant (or Dependent of COBRA Participant for your covered family members). Once on My Health, click the Enroll now green button. IF YOU ARE NOT A FORMER JPMORGAN CHASE EMPLOYEE My Health is available to you for information only. You must enroll through the accesshr Contact Center at Representatives are available to assist you from 8 a.m. to 7 p.m. Eastern Time, except certain U.S. holidays. 12 Now Is the Time to Enroll Your first stop for enrollment is My Health via the internet at myhealth.jpmorganchase.com. Click Enroll now to access the Benefits Web Center. You will need to enter your Standard ID number, shown on your enclosed Personalized Fact Sheet, and your JPMorgan Chase Single Sign-On password (see How to Use Single Sign-On on page 15). If you are not a former JPMorgan Chase employee, you cannot enroll through My Health. You may request any of the information in this bulletin by calling the accesshr Contact Center at You must enroll through the accesshr Contact Center; representatives are available to assist you from 8 a.m. to 7 p.m. Eastern Time, Monday through Friday, except certain U.S. holidays. Confirming Your Coverage If you make any changes to your assigned coverage or you enroll a dependent for the first time in 2015, you must confirm your elections: Online through the Benefits Web Center; you ll need to Confirm your choices in order to complete your enrollment. You ll know that your elections have been saved when you see the Completed Successfully screen. Please print a copy of this screen and retain it for your records. You get help: Understanding your JPMorgan Chase medical, prescription drug, dental, and vision benefits; Evaluating coverage options available through the Health Insurance Marketplace or other aspects of health care reform; Resolving benefits claims issues; Finding quality providers in your network; Maximizing your health care benefits; Scheduling appointments with hard-to-reach specialists; Locating community resources and assistance with non-jpmorgan Chase benefits issues, and more. To contact Health Advocate, Inc., call , Monday through Friday, from 8 a.m. to 9 p.m. Eastern Time. Take These Steps After You Enroll 1. Get a biometric Wellness Screening and complete an online Wellness Assessment. Get both a biometric Wellness Screening and complete an online Wellness Assessment between September 1 and December 31, 2014 to earn $200 for your 2015 MRA. If your covered spouse/domestic partner completes both activities within this timeframe, you will earn $100 for your MRA. 2. Complete Additional Wellness Activities. Complete Additional Wellness Activities between January 1, 2015, and December 31, 2015, and earn more MRA funds (after completing your Initial Wellness Activities). See page 6 for the Additional Wellness Activities you and your covered spouse/domestic partner can complete. 3. Get the most out of your Medical Plan all year long. By being a wise consumer of health care, you can select quality providers and save money on your health care throughout the year. Complete the Choosing Care Wisely online program on your health care company s website (available January 1, 2015). Understand how you can become a better health care purchaser using the tools and resources available to support your decision making. You will receive tips such as: Receive care from in-network doctors. You ll pay rates that have been negotiated between your doctor and health care company, which are on average 50% lower than the rates you pay when you receive care from doctors outside the network. Plus, you receive a higher level of benefits. Check if your doctor is in your health care company s network by going to My Health to access Cigna s or UnitedHealthcare s websites. Use generic prescription drugs. Generic prescription drugs cost less and have the same effectiveness as brand-name drugs. In addition, generic preventive prescription drugs are covered at 100%. Go to CVS Caremark s website through My Health > View my prescription plan to see if there is a generic option available for any of your current drug therapies. Explore stand-alone high-technology radiology providers, because they may be less expensive than hospital-based providers. This type of radiology provider is used for X-rays, MRIs, etc. Use nationally affiliated labs (such as LabCorp and Quest) instead of independent labs when you need lab work. They ll cost you less-especially when you go to an in-network lab. Check if the lab is in your health care company s network by going to My Health to access Cigna s or UHC s websites. 14 4. Have a chronic condition? Take advantage of the tools and resources JPMorgan Chase provides to help you manage your conditions, such as asthma, coronary artery disease, congestive heart failure, COPD, diabetes, and others. If you have a chronic condition, you may receive a call from your health care company inviting you to participate in its Condition Management Program be sure you do! You can get valuable information about your condition and earn Wellness Rewards in your MRA when you participate in a Condition Management Program. 5. Taking maintenance medications? If you re taking a long-term medication, you can receive your 90-day supply by mail through CVS Caremark s mail-order prescription service or by picking up your 90-day supply at a CVS pharmacy, where the same discounts are available through the CVS Caremark Maintenance Choice Program. (Please Note: You may opt out of Maintenance Choice and obtain a 90-day supply [or a 30-day supply] at any participating network pharmacy. Keep in mind that it may be more cost-effective for you to use the CVS Caremark mail-order prescription service or a CVS retail pharmacy.) 6. Go to your primary care doctor or a convenience care clinic (such as CVS Minute Clinic or Walgreens Healthcare Clinic) if possible, instead of specialists, for a non-threatening illness or injury. Remember: Convenience care clinics are available for minor medical problems like a cold or the flu while urgent care centers can provide care for more serious problems like sprains, minor cuts, and burns. Don t hesitate to use the emergency room if you require immediate medical attention for a serious condition, like having trouble breathing or chest pain. Benefits Reminders HIPAA Privacy Rights and Protected Health Information The Health Insurance Portability and Accountability Act (HIPAA) requires employers to communicate how certain protected health information under employee health care plans may be used and disclosed, as well as how plan participants can get access to their protected health information. Accordingly, JPMorgan Chase will distribute once every three years a Privacy Notice of Protected Health Information Under the JPMorgan Chase Health Care Plans to you that describes in detail how your personal health information may be used and your rights with regard to this information. (This notice will be distributed in December 2014.) Newborns and Mothers Health Protection Act In accordance with the Newborns and Mothers Health Protection Act, group medical plans and health insurance issuers may not, under federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 hours following a normal delivery, or to less than 96 hours following a cesarean section. Further, the plan cannot require that any medical provider obtain authorization from the plan or any insurance issuer for prescribing a length of stay less than these periods. Medical Plan Post-Mastectomy Benefits All options under the JPMorgan Chase Medical Plan cover certain breast reconstructive benefits in conjunction with a mastectomy for eligible participants. Coverage under the Medical Plan is available for: Reconstruction of the breast on which the mastectomy was performed; Surgery and reconstruction for the other breast to produce a symmetrical appearance; and Prosthesis and treatment of physical complications for all stages of mastectomy, including lymphedema. This coverage is subject to the terms of the Medical Plan option in which you participate, including relevant deductibles and coinsurance provisions. Step 1 ENTER YOUR STANDARD ID ON THE SINGLE SIGN-ON SCREEN. The Standard ID that was in effect at the time you left JPMorgan Chase will remain in effect after you leave. ENTER YOUR DEFAULT PASSWORD, which is composed of: The first three characters of your Standard ID; The last four digits of your Social Security number or national ID number; and The first letter of your last name. For example, if Joe Smith s Standard ID is U and his Social Security number is ; his initial password is: U126789S. CLICK LOGIN. Step 2 CHANGE YOUR PASSWORD. Re-enter your initial password in the Current Password field, and then create and confirm a new password following the rules outlined at the top of the screen.
