2 Introduction Girard City is 25 square miles with approximately 9958 for population. It is made up of 95.3% White, 5.2% African American, 0.7% American Indian and Alaska Native, 0.5% Asian, 2.1% Hispanic, 1.8% two or more races. Girard City is mostly an urban community in Ohio and is part of the Appalachian region. Our Community Health Assessment has shown that this type of demographic region has important health disparities. People living in these regions are more likely to be less educated, not be working, be below the federal poverty guidelines, be uninsured and have unmet health care needs. In Girard City, the median age of residents is 41.8 years of age. 1,230 homes have a household member under the age of 18 years, and 1,313 homes have a member 65 years or older. In addition, in 2013 Trumbull County ranked 71 out of 88 Ohio counties for overall health outcomes; and in 2014, Trumbull County ranks 76 for these same indicators. According to the most recent U.S. Census Data, Girard City Health Department (GCHD) served a population of 9,958 residents. This being said, our health department is comprised of three divisions which include: Administration/Environmental Health, Nursing, and Vital Stats. The GCHD employs two full time professionals and one part time, who work in these divisions. The Administration is comprised of the Health Commissioner/Environmental Director, the Nursing Director, the Administrative Secretary. The Environmental department consists of the one Sanitarians, one Clerical and one contracted Plumbing Inspector who administer including: private water systems; food service operations and retail food establishments; household and semi-public sewage treatment systems; public pools; manufactured home parks; solid waste and construction/demolition debris facilities; nuisance investigations; schools; rabies surveillance; child fatality review; mosquito control; tattoo facilities; and plumbing. The Nursing department consists of one Registered Nurse who administers including: immunizations; home visiting; tuberculosis services; emergency preparedness; epidemiology; communicable disease follow-up; public education. 3 Overview of the Strategic Planning Process The Strategic plan process of the Girard City Health Department began in April of 2014 and included input from Staff as well as City Officials. The plan also referenced the following documents: Girard s Strengths, Weaknesses, Opportunities and Threats (SWOT) analysis survey. Trumbull county General Health District SWOT analysis survey. Girard City Health District s Mission Statement: We are committed to protect and promote the health and well-being of our community and prevent disease, disparity and harm to our residents. This is accomplished by responding promptly to serve the needs of the public in a professional and respectful manner with emphasis on public health education and outreach. Girard City Health District s Vision Statement: The effective exchange, collaboration, and communication of ideas and thoughts with all internal and external stakeholders will enhance our policies, procedures and. This along with the promotion of our public health services will eliminate disparities and have a stunning effect on our community. Girard City Health District s Values: Integrity / Honesty: We act with integrity and are accountable for our actions. Effectiveness: We strive to be effective when providing our services. Dependability: We provide reliable and dependable services. Service: We deliver an exceptional level of service through our. Accuracy: Our are structured to accurately reach our targeted goals. 4 SWOT (Strengths, Weakness, Opportunities & Threats) Analysis The entire Health Department Staff and Administration was involved in an internal SWOT Analysis. The following STRENGTHS, WEAKNESSES, OPPORTUNTITES, AND THREATS were the results. 5 GIRARD CITY HEALTH DEPARTMENT STRATEGIC PRIORITIES PRIORITY #1- Community Goal 1: The Girard City Health Department will strengthen the public health system in the city of Girard. Objective 1: Access to health care is increased. Number of people using the health department services and those accessing public transportation is increased 1% by 2018 Actions: 1. Educate the public and collaborate with stakeholders in the county to improve and promote transportation services 2. Promote the Girard City Health Department s Home Health Care Program, Walk-in services, and adult and children immunization. 3. Educate the public on the Bureau of Children with Developmental and Special Health Needs Program. 4. Collaborate and refer to local agencies and health facilities. 5. Collaborate and promote public and private entities to increase health Insurance enrollment. Goal 2: The Girard City Health Department will maintain daily operations. Objective 2: The Girard City Health Department will be able to maintain current services and staff through Actions: 1. Will evaluate services and current fees annually or sooner if warranted. 2. Will collaborate with stakeholders and partners to help with services and. PRIORITY #2 Environment Goal 1: To have Girard be an attractive and healthy community to live in. Objective 1: Increase the number of educational materials on Healthy Homes distributed by 5% by Actions: 1. Partner with local businesses to help with funding to increase lead screenings in children under 6 years of age. 4
6 2. Increase awareness of Healthy Homes by educating public on asthma, lead, smoking, and radon. 3. local fire department for educational. Priority #3 Community Harm Goal: To reduce crime and violence in Girard. Objective 1: To reduce drug abuse and overdoses in the city by 1%. Actions: 1. Partner and collaborate with the Girard City Schools and Police Department in establishing anti-drug. 2. Girard City Schools and TCESC on anti-bullying program. 3. local agencies for rehab and educational services and educate the public of these resources. 4. mental health agencies. PRIORITY #4 Promote Healthy Life Styles Goal: Improve the health of the citizens by educating and promoting healthy behaviors. Objective 1: To increase health educational offered in the community by 1% by Actions: 1. local universities in offering to the public. 2. Girard Multigenerational Center in promoting established. 3. Promote and educate on immunizations for all ages. 4. media on promoting healthy life styles. 5
