4 What is the cost of fatigue? Fatigue was reported as the cause of over 30% of all transportation accidents (US National Transportation Safety Board, 1999). Quoted as a precipitating factor in 57% of truck accident occurrences (Walsh, Dement, Dinges, 2005). Factors such as shift work systems, high production demands, constant transportation to and from the mine site (among others) increase fatigue build up among workers and consequently the risk of accidents (Folkard & Tucker, 2003). The cost of a fatal accidents may soar to US$5 million (Corporate Occupational Health & Safety Management, Codelco). Between 2007 and 2009, 43% of death related accidents were associated to driving of all types of vehicles (Mutual Safety C.Ch.C.). 25% of accident reports area linked to light vehicles and 23% to heavy vehicles (Codelco 2012).
5 What is the cost of fatigue? In 2010, 7.7 % of traffic accidents were directly associated to driver s fatigue, National Traffic Safety Commission, Ministry of Transport, Chile. USA estimates suggest that drowsiness is responsible for 1 3% of highway accidents and that nearly 96% of accidents are caused by passenger vehicles (Knipling & Wang, 1994). Fatigue is the reason of 24% traffic accidents (Mexican Transportation Institute, 2004). In Ireland, fatigue is a contributing factor in 1 out of 5 death related vehicle accidents (The Accident Claims Helpline, 2011).

11 Learned Lessons Organizations must make efforts to determine the true magnitude of fatigue and drowsiness, collecting reliable data and defining costs. Fatigue and drowsiness are risk factors to be incorporated into risk management systems since these represent a critical variable in accident generation. Fatigue and Drowsiness Programs should be comprehensive, gradually implemented, consider OHS indicators (frequency rate, severity rate, fatigue levels, etc.), and production indicators. Direct and indirect costs should be considered and assumed. This will help replicate good practices, technology development, and suppliers management. Maintainability costs should be considered by incorporating the necessary adjustments. Short and long term benefits must be assessed. Leadership should be permanent by a leader team.
