Keeping Your Clients in Their Homes Through the Use of Technology Presented by: Cathy Barton Director of Living Safe Technologies Lutheran Senior Services Challenges of Private Duty Care Cost of service / affordability Staffing availability / dependability Family conflict / communication Lack of family support Short term fix for more complex needs Fragmented delivery system of service Availability of services in client s living area Competition public and private sector Reputation How Technology Can Overcome the Challenges Keep your client in a safe environment. If costs or decrease in care become a concern you can still manage their needs with technology. Technology can address several areas of concern Social, Safety, Health and Wellness. Opens communication between service areas. Technology is a pro-active way to monitor your client when not there. Baby boomers are accustomed to technology and will not hesitate using for themselves or loved ones. Help sustain private duty hours and the longevity of your service by being more in tune with the clients. 1
National Level Center for Aging Services Technologies (CAST) Established in 2003 National coalition of more than 400 technology companies, aging services organizations, research universities and government representatives. Works under the auspices of Leading Age (formerly American Association of Homes and Services for the Aging). Center for Aging Services Technologies (CAST) Four areas of focus Drive a national vision of how technologies can improve the quality of life for seniors while reducing costs; Accelerate technology R&D pilots with seniors to prove out this vision; Advocate to remove barriers to the rapid commercialization of proven solutions; and Promote national dialogue about standards to insure interoperability and widespread access to aging services technologies. The Concept of Aging in Place (AiP) What does AiP mean? The ability to live in one s own home and community safely, independently, and comfortably, regardless of age, income, or ability level. Ways to Age in Place Caregiver services, funded and non-funded Home modifications / CAPS certified builders Move to smaller, more maintenance free or economical living arrangement or community based living. Care management services (GCM) Technology 2
AiP Redefined, AiP 2.0 and AiPT From the Metlife Report, AiP 2.0: AiP 2.0 is the residential design, medical and home monitoring equipment, and comprehensive care services that are integrated into a dynamic and efficient monitoring and management system. AiP 2.0 partners should prepare to work in coordination with multiple partners with intersecting market sectors rather than narrow niches. Technology development and dissemination is critical to efficient resources in AiP 2.0 and AiPT (Aging in Place Technology). Types of Technology Social Safety Passive Monitoring / Telecare GPS Medication Management Telehealth Telemedicine Social Technologies Wii ( Jitter Bug Cell Phones ( Brain Fitness Dakim ( Lumosity ( Celery ( Ojo Video Phone ( Computer programs ( Skype Real time video interaction with church services, exercise classes, etc 3
Safety Technologies Personal emergency response systems Fall prevention and detection Mobility aids Stove use detectors Smoke and temperature monitors Door and window contacts / security Wander management Safety Technologies: Personal Emergency Response Systems Monitored systems Even with some limitations the PERS system is priceless in getting help to the user in the The Golden Hour! Coverage is limited to radius of home and yard. Requires land line to work; VOIP and digital services are not always compatible. User must wear sensor for best results. Reactive system user must push button to summon help with the exception of fall detection sensors. Safety Technologies,Telecare Passive (automatic) sensors that monitor for safety and security. Can be programmed with focus on activities of daily living. Offer continuous and automatic monitoring. adaptable to specific needs of the individual. Does requires land line for monitoring unit. Proactive monitoring user does not have to wear since sensors monitor the environment. 4
Telecare Sensors Work with the PERS system and compliment it s function by providing additional passive monitoring around the home. Sensors are wireless, battery operated and simple to install. Mixed and matched according to the needs of the individual, and can be added or changed as the individual s needs change. Sensors can be customized. Types of Sensors Standard Pendant / Stationary Pendant Water Detector / Smoke Detector Extreme Temperature Sensor Motion Detector Door & Window Contacts Floor Pressure Mat Moisure Pad (incontinence) Bed/Chair Sensor Body Movement Detector Fall Detector Uses of Telecare Sensors Detect a need for assistance (fall risk) Monitor sedentary behavior Monitor wandering Changes in activities of daily living such as eating, sleeping, toileting or socialization. Provide assistive prompts to comply with key activities like taking medications, turning off the stove or even taking vital signs for chronic disease monitoring. 5
Uses of Telecare Sensors Record motion in key areas Floor sensors track footstep patterns and detect changes that warn of potential falls Elaborate setups that integrate Webcams and video conferencing systems Wireless sensors Devices to regulate temperature, lights and appliances Sophisticated medical monitors Activity Monitoring Strategically placed sensors in the home to detect activities of the individual living alone and then automatically triggers an alarm call when there is an exception to the normal expected pattern of activity. Example of Activity Monitoring Monitor activity outside the bedroom between 6:00 am and 8:00 am. If no activity, then trigger an alarm call. At same time, monitor activity in the living area of the home between 8:00 am and 10:00 pm, and if there is a period of more than 2 hours of inactivity, then trigger an alarm. 6
Telecare Hands Free Detection Fall Detection Wandering Floor mats Bed sensors Fall detector Motion detector Video Floor mats Bed sensors Door / window contacts GPS Video GPS Alzheimer s Association endorses Comfort Zone EmFinders direct line with 911 system I-tag notification by and text Advantages of GPS Can set up geo-fence Can use bread crumb tracking Limitations of GPS GPS is not reliable Long battery charge time Wearer can remove Medication Management Reminder Systems Dispenser systems to avoid wrong dosage. Phone and watch reminder calls or tones. PERS systems that can be set with recorded reminders. Easy to use systems that are more user-friendly and space efficient. Monitored Systems Will notify designated caregiver (or monitoring center) if dosage is not removed from dispenser Common Vendors: Philips, SimpleMed, Med Ready, and Epill 7
Health and Wellness Technologies Medication compliance is vital component Telemedicine/Telehealth Means of delivering information and healthcare through the use of telecommunication. Bridges the distance between patient and physician by allowing patients to remain in their home while being seen by a health care provider at a distant site. Telehealth / Telemedicine Live-interactive videoconferencing (meets face to face encounter.) *Remote monitoring of clinical data such as blood pressure, weight, heart rate, blood glucose, and oxygen levels. Store and forward imaging allows x-rays, CTR scans, MRI images and digital images be transmitted. *The Veterans Administration (VA) has one of the most recognized and documented success rates of managing long-term care clients in the home. Who Would Benefit From Using Telecare? Older adults or at risk individuals living along People at risk of falling or other household dangers People with sensory or physical impairments People transitioning back home from rehabilitation People with memory impairment Caregivers formal and informal Anyone in a compromised living situation Anyone needed peace of mind! 8
Why Use Technology to Support Your Private Duty Clients? Older adults remain independent as long as possible and experience better quality of life. Slows the progression through the various levels of the care continuum and allows care providers better management of transition between levels of care. Reduces emergency room visits. Reduces hospitalizations. Reduces nursing home stays. Realizes substantial financial benefits for older adults, care providers, family members and payers. Family members experience peace of mind knowing that adverse events are detectly as quickly as possible.
