3 Your Program EmblemHealth s program* provides a standard level of benefits for all members, and may be upgraded with a number of available benefit riders. Please note that our optical program includes only eyeglass frames and lenses, and in some instances may also cover contact lenses. Eye exams and written prescriptions for corrective lenses are covered benefits under your medical plan. How the Program Works Step 1: get Your Eyes Examined and Obtain a Prescription. To schedule an eye exam, check this listing for the name of a network optical provider. Call the optical provider and ask if the optometrist at the location is a network optometrist, and if he or she is accepting new patients. You can also ask your primary care physician for the name of a network optometrist, or simply check your medical provider directory or go online at and conduct a provider search for a network optometrist. The optometrist will examine your eyes and give you the prescription you need to have eyeglass frames and lenses made at the network optical provider. Please note that depending on your particular coverage, you may be able to obtain an eye exam from an out-of-network provider (subject to the out-of-network deductible and coinsurance); however, you must go to a network optical provider to receive eyeglass frames and lenses. Step 2: Visit a Network Program Provider. Once you are given a prescription, please bring your eyeglass order with your member ID card to one of the network optical providers listed in this directory to have your eyeglass frames and lenses made for you. We recommend that you schedule an appointment ahead of time. Step 3: Choose Your Frames. Our selection of covered frames is available at every network optical provider. You will find safety, oversize, single-vision, bifocal and trifocal glasses. Lenses in glass or plastic and cosmetic tinting are included. All of our network optical providers guarantee that the frames in our selection conform to current standards established by the American National Standards Institute (ANSI). If you prefer, you can choose frames offered through the network optical program provider that are not included in our selection. If so, then you simply pay what you would normally pay for a frame from our selection plus the difference in cost between the highest-priced selection frame and the frame of your choice.
