3 How to Use This Book The UPMC for You Provider Directory is set up to make it easy to find what you need. If you are looking for the name of a hospital, service, or facility, simply go to the Participating Hospitals or Services and Facilities sections in this book. All of the other listings in this directory are divided by county and are in alphabetical order. Go to the table of contents to locate the type of listing you want in the county where you live. Index The index in the back of this book lists all of the hospitals, facilities, providers, and pharmacies in this directory. If you know the name of your provider, you can use the index. Everything is listed in alphabetical order. The index will tell you on which page or pages you will find each listing. New providers join UPMC for You s network often. The listings within this book may not include some doctors who recently have been added to the network. This listing of providers is current as of the publishing date. 5 How to Receive Medical Care When you joined UPMC for You, you chose your primary care practitioner (PCP). Your PCP will help you manage your health care and arrange for services you need. When you think you need a medical service, call your PCP. The telephone number of your PCP is on the front of your UPMC for You member ID card. You can visit your PCP in the privacy of his or her office, where you will get personalized health care. Your PCP will send you to a specialist if necessary. Some members may have limits and have to pay a small copayment for some prescriptions and for covered services ordered by their PCP or participating specialist. Choosing a Provider When picking a provider, it is important that you have all of the information you need to make the choice that best meets your needs or the needs of your family. This book gives you some basic information about each listed provider. To learn more about where your provider went to medical school, where he or she completed his or her residency, or other professional qualifications, call UPMC for You Member Services at Representatives are available Monday, Tuesday, Thursday, Friday from 7 a.m. to 7 p.m., Wednesday from 7 a.m. to 8 p.m., and Saturday from 8 a.m. to 3 p.m. TTY users should call toll-free Providers may have one or more of the following codes in their listing. The codes mean the following: SP means specialty. A specialty means that the provider can treat patients with a particular type of disease, illness, or condition. BC means board certification. To get a board certification, a provider must pass a test on that specialty. HA means hospital affiliation. These are hospitals where the provider can admit his or her patients. Listings also have the provider s normal office hours and the age ranges of the patients that the provider sees. The provider s address and telephone number are also included in the listing. If the provider has more than one office, each office is listed by ZIP code. Changing Your PCP The relationship you have with your PCP is very important. If you feel your PCP is not right for you, you can pick another PCP. UPMC for You members can change their PCP for any reason, at any time. Just call Member Services at Representatives are available Monday, Tuesday, Thursday, Friday from 7 a.m. to 7 p.m., Wednesday from 7 a.m. to 8 p.m., and Saturday from 8 a.m. to 3 p.m. TTY users should call toll-free Your personal designated Member Service Representative will help you pick a new PCP who will meet your needs. Your PCP selection is effective immediately after calling Member Services. You will receive a new UPMC for You member ID card with your new PCP s name and phone number. Behavioral Health Services UPMC for You does not manage your behavioral health benefits. These are services for mental health, drug abuse, and alcohol abuse. If you have any questions about these Medical Care iii

6 services, refer to the toll-free behavioral health telephone numbers listed in your UPMC for You Member Handbook, or call Member Services at Representatives are available Monday, Tuesday, Thursday, Friday from 7 a.m. to 7 p.m., Wednesday from 7 a.m. to 8 p.m., and Saturday from 8 a.m. to 3 p.m. TTY users should call toll-free Emergency Care If you have an emergency, go to the nearest emergency room, call 911, or call your local ambulance service. After a visit to the emergency room, you must call your PCP for any follow-up care. If you have an emergency and need to be admitted to the hospital, you should let your PCP know. You should call within two days or as soon as you or your family can call. A medical condition is considered an emergency when your symptoms happen suddenly and seem life threatening. You should get emergency care when your health or the health of your unborn child, if you are pregnant, is in danger; you could have serious injury to a body part; or you have or could have loss of normal body function. Here are some examples of emergencies: Heart attack Chest pain Severe headache (this could be a sign of a stroke) Severe bleeding Unconsciousness Poisoning Sudden loss of feeling or not being able to move Strong feeling that you may kill yourself or another person Severe pain Non-Emergency Care You should not go to the emergency room for non-emergency or routine (basic) health care. Call your PCP to schedule an appointment for routine health care needs. Here are some examples of routine (basic) or non-emergency health care problems: Earache Backache Cuts, scrapes, and bruises Cold or flu Sore throat Coughing or sneezing Minor skin rashes Sprained ankle or wrist Physical exams Toothache When you have a minor illness or injury and need to receive care but you cannot reach your PCP, an urgent care or convenience care center can be a very good choice. Care you receive at many urgent care or convenience centers is a UPMC for You covered benefit. Urgent care center doctors and nurses stitch wounds, take x-rays, and treat sprains. They treat earaches, back pain, dental pain, and sore throats. They care for minor rashes, burns, cuts, and scrapes. Call your PCP or go to an urgent care or convenience care center for routine (basic) or non-emergency health care needs. If you do not go to your PCP or an urgent care or convenience care center for routine or nonemergency health care, you may be required to pay an emergency room copayment and UPMC for You may not pay the bill. You may be responsible for the bill. 7 Member Identification Card Your UPMC for You ID card does not replace your ACCESS card, so you should carry both cards with you. Your UPMC for You ID card is good as long as you stay in the plan. New members will receive a UPMC for You ID card shortly after their effective date. Peel off the UPMC for You ID card and keep it in your wallet with your yellow or greenish-blue ACCESS card. You will need to show your UPMC for You member ID card, ACCESS card, and any other insurance cards you may have whenever you make an appointment, go to your PCP or other provider, pharmacy, hospital, or use any other medical facility. The name and phone number of your PCP is on your member ID card. The UPMC for You number is also on the card. 9 Provider Definitions Certified Nurse Midwife (CNM) A registered, professional nurse licensed to practice as a midwife. A midwife cares for pregnant women and newborns throughout the maternity cycle. Certified Registered Nurse Practitioner (CRNP) A registered nurse with advanced training in a specialty area, certified by the boards in that specialty area. A CRNP may diagnose a condition or prescribe medicine under the direction of a doctor. Family Practice Doctor A doctor who treats all family members. Gynecologist (Gyn) A doctor who provides women s health services such as Pap smears, annual checkups, and family planning services. A gynecologist can also perform surgical procedures such as a D&C (removal of uterus tissue lining) or a hysterectomy (removal of uterus). Internal Doctor A doctor who treats adult illnesses. Most internal medicine doctors see patients who are age 16. Obstetrician (Ob) A doctor who takes care of pregnant women and delivers babies. Ob-Gyn (Obstetrician-Gynecologist) A doctor who cares for pregnant women, delivers babies, and provides women s health services such as Pap smears, annual checkups, and family planning services. An ob-gyn also can perform surgical procedures such as a D&C (removal of uterus tissue lining) or a hysterectomy (removal of uterus). Ophthalmologist A doctor who treats eye problems. An ophthalmologist may perform surgery and write prescriptions. Also, an ophthalmologist may perform eye exams and fit patients for eyeglasses and contacts. Optometrist A health care provider who performs eye examinations and fits patients for eyeglasses or contact lenses. Pediatrician A doctor who treats children. Most pediatricians see patients age 21 and younger. Physician Assistant (PA) A health professional who is trained to practice medicine with physician supervision. A PA may perform physical exams, order and read lab tests, diagnose and treat illnesses, prescribe medicine, and counsel patients. Primary Care Practitioner (PCP) Your personal doctor or CRNP who will provide or arrange for all of your health care needs. Different types of PCPs include family and general practice doctors, pediatricians, internal medicine doctors, and CRNPs. Specialist A health care provider who treats a particular type of illness, disease, or condition. For example, a cardiologist is a specialist who cares for conditions and diseases of the heart, and a dermatologist is a specialist for skin conditions and diseases. Definitions vii

10 Definitions For Terms Used in Directory Listings Accepting New Patients This phrase is used when a provider or practice is willing and able to accept new patients. If a provider is not able to see new patients, the listing will read existing patients only. Accreditation Accreditation means to certify as meeting an official requirement or standard of excellence. Two common hospital accreditation boards include the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the American Osteopathic Association (AOA). Facility Affiliation Affiliation means that the physician can make referrals to the hospitals listed. A hospital gives certain doctors privileges to admit patients to its facility. Gender The provider listings in this book indicate whether a doctor is male or female. Languages Spoken The listings show which languages a provider can speak, other than English. The listings may also tell you if the provider is able to communicate through American Sign Language or other forms of visual/gestural communication. Specialty The listings show which type of health care a doctor can offer. Board Certification Board certification shows that a doctor has received a certificate from a certain group. The doctor has to meet certain requirements of the board to get a certificate. UPMC Health Plan recognizes board certification by either the American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA). Wheelchair Accessibility Some PCP locations are wheelchair-accessible. At other locations, special arrangements can be made for access. Providers who have wheelchair accessibility have a wheelchair symbol next to their names in this directory. 11 How To Check Certification You can check on your doctor s certification with certain groups that review medical providers. Check the links below if you have a computer and Internet service. If you do not have a computer, call Member Services at Representatives are available Monday, Tuesday, Thursday, Friday from 7 a.m. to 7 p.m., Wednesday from 7 a.m. to 8 p.m., and Saturday from 8 a.m. to 3 p.m. TTY users should call toll-free The following link is for the American Board of Medical Specialties. Doctors who have certification have taken an extra career step. They have volunteered to have their qualifications judged by a group of other physicians. The following website was created by the American Osteopathic Information Association and the American Osteopathic Association (AOA). It shows which doctors are certified by the AOA. Once on the website, follow these instructions: Select Find a DO on the right side of the page. Complete the form to search the AOA physician database. Click on the Search button. Once on the website, follow these instructions: Click on Consumers. Register for the Search Service by completing the information requested to set up a user account including your address. You will receive an containing your password and instructions on how to log in. Log in with your address and password. Search by name and location or by specialty and location.
