28 Mission Michigan Hunger Solutions (MHS) is working to end hunger in Michigan by increasing the effectiveness of anti-hunger programs and reducing barriers for the hungry to receive assistance.

29 Child Nutrition Programs Universal School Breakfast Breakfast in the Classroom Grab and Go 2 nd Chance Breakfast Follows all federal nutrition guidelines Available to students free of charge in areas of high poverty

32 AmeriCorps - Veterans

33 These are Michigan veterans.

34 They want to access benefits. 660,000 Veterans call Michigan home Michigan has the 11 th largest state Veteran population Over the next several years it s expected more than 10,000 Veterans will return home to Michigan each year

35 But, there are barriers to access.

36 Meet the Veteran Resource Navigators. The VRN mission: To guide Veterans, active duty service members and their families back to their earned benefits and work with existing community partners to meet emergent needs

37 They guide Veterans back to benefits. 20 AmeriCorps members in greater Wayne and Kent counties Placed with non-profit service agencies Provide 24,080 hours of service to Michigan Veterans. Over 600 connections made with local businesses, community based organizations, government offices, colleges, universities and churches Recruited, trained and incorporated 92 volunteers to assist AmeriCorps members during outreach events.

45 SECC Through its partnership with United Ways across the State of Michigan and with other federations, MAUW has developed a statewide network of relationships with which SECC can leverage. This allows MAUW not only to carry out its function in Lansing, but provide key participation and support from every community across the state.

46 SECC To maintain such a relationship, MAUW prides itself on providing clear communications to participants as well as consistent expectations to implement the high standards which SECC maintains across its partner charities.

47 SECC By partnering with over 1200 charities, the Michigan SECC is able to leverage the largest workforce in the state to create substantial impact in local communities. Throughout the past 25 years, State of Michigan employees have demonstrated their commitment to continuously improve the communities in which they live, work, volunteer and raise their families.

48 2014 Highlights $797,000 raised through payroll deduction and department events 3,675 donors 245 first-time donors $207 average donation 1,227 participating charities 295 volunteers

49 Michigan Children s Healthcare Access Program

50 What is MI-CHAP? A physician-led, collaborative, community-based medical home project that will improve the health of underserved children on Medicaid. Goals: Family Level Improve the health of Michigan children on Medicaid Provider Level Improve the quality and efficiency of care provided in the medical home System Level create additional access for Medicaidenrolled children through provider incentives, expanded office hours, and an increase in the number of Medicaid appointments available for acute visits

52 What does CHAP look like locally? Eight Multi-disciplinary local CHAP teams will be expanded or started within the next 2 years: Kent and Wayne County currently operating and expanding service Genesee, Macomb, Northwest (Antrim, Emmett, Charlevoix, Otsego) will be launching within the next few months Ingham, Kalamazoo and Saginaw January Implementing Virtual CHAP to connect callers with medical homes in locations without a local CHAP team within the next few months Upper Peninsula providing full CHAP services to callers within UPCAP service area January 2016
