3 INTRODUCTION The District Resource Statement (DRS) is prepared annually by the Office of Management and. Sections 2707 and 2708 of the New City Charter require that agencies report to the City's 59 community boards expense budget and service information at the agency's service district level. The reporting levels vary by agency and by program. Service districts range from police precincts which can be smaller than community districts to multi-borough offices. This edition of the DRS gives information for both FY 2013 and FY The current edition reports on agencies that deliver services directly to community districts and boroughs. These agencies manually generate the information in the DRS, except for the fiscal data which is provided by the Financial Information Services Agency. The agencies also provide performance indicators for a variety of services. An explanation of agency programs and performance indicators is included. Personnel headcounts and corresponding fiscal data, equipment information, performance indicators, and contractual services are organized as follows: Personnel Report FY 2013 headcounts and fiscal data are as of June 30, FY 2014 Adopted data is as of the Adoption in June. Equipment Report - Unless otherwise noted, FY 2013 information is as of June 30, FY 2014 information indicates proposed equipment levels. Performance Report - Once again, community boards will be able to compare select measures of agency performance within their district to a borough-wide and/or citywide average of the agency's performance. Except where indicated, the performance figures are full year figures for FY Section 12(c)6 of the New City Charter states that "for agencies with local service districts or programs within community districts and boroughs, the mayor's preliminary management report and management report insofar as practicable shall include schedules of agency service goals, performance measures and actual performance relative to goals for each such local service district or program." Indicator information in the DRS supplements the Mayor's Management Report by providing measures of performance at the local level. Totals for all the districts reporting in the DRS will be consistent with the citywide numbers reported in the MMR except where otherwise noted. Contract Report - Data on contractual services varies somewhat from program to program, but includes at least the time period of the contract, the cost of the contract, the kind of service provided and some measure of the service.

4 For each agency program, all pages for a particular local service district are grouped together in the following order: Personnel, Equipment, Performance, and Contract Reports. The Personnel Reports give a complete description of the service and the address of the local service district headquarters. The contract pages provide the names and addresses of vendors, and sometimes the location where the service is provided. The District Resource Statement is useful to the community boards at all stages of the budget participation process. boards can use the DRS to monitor service delivery throughout the year. It provides a common data base which the agencies and community boards can use to discuss service delivery and to formulate specific budget requests. The Performance Reports can help the boards assess the quality of the service in relation to the resources which are deployed in or budgeted for the community district. By comparing this year's DRS with previous editions, community boards can also use the DRS to analyze trends in funding levels, resource allocation, and service delivery. Using the Mayor's Management Report, boards are able to compare service performance indicators in their district with citywide findings. OMB publishes District Resource Statements for the following agencies: Department for the Aging Department of Buildings Department of Cultural Affairs Department of Small Business Services Department of Environmental Protection Fire Department Department of Health & Mental Hygiene Department of Housing Preservation & Development Human Resources Administration Libraries Department of Parks and Recreation Police Department Department of Sanitation Department of Transportation Department of Youth and Development Caution must be used in interpreting the information contained in this document. Agencies often need to move personnel or equipment from one district to another or employ seasonal workers depending on the time of the year. These changes are not reflected in the DRS. A reduction in staffing levels does not necessarily signal a corresponding reduction in services. Where necessary agencies provide explanatory footnotes. Also, Financial Plan Savings are not reflected in the local district reports because these amounts are budgeted at the Unit of Appropriation level. It is not possible to predict the impact of financial savings on an individual service district level.

5 DEPARTMENT OF HEALTH AND MENTAL HYGIENE PROGRAMS AND PERFORMANCE INDICATORS Mission The mission of the (DOHMH) is to protect and promote the health and mental well-being of all New ers. It does much of its work indirectly, by providing information and recommendations to other policy-makers, health care providers, and New ers in general, who in turn take actions to promote health, but it also provides direct health services and enforces health regulations. DOHMH makes reducing deaths from chronic diseases such as heart disease and cancer a priority by reducing smoking, unhealthy diet, and other risk factors. The Department contracts for mental health services, developmental disability services, and alcohol and substance abuse treatment services. DOHMH works with health care providers to improve the use of preventive health services, including immunizations, and works with both health care providers and nonprofit organizations to prevent and treat HIV infection. The Department oversees the City s Early Intervention Program for infants and toddlers with developmental delays. DOHMH s community-based services include: three District Public Health Offices; four tuberculosis clinics; eight STD clinics; health services at more than 1,200 public school sites; and health and mental health services in the City s correctional facilities. DOHMH also issues birth and death certificates; works to create a healthier and safer environment by conducting inspections of restaurants, day care centers, and other entities; and protects public safety through immediate response to emergent public health threats. The District Resource Statement has five sections. The Division of Health Care Access and Improvement o (Includes Child and Adolescent Health Clinics; and Health Insurance Programs) The Division of Disease Control o (Includes Immunization; Tuberculosis; Sexually Transmitted Disease; and HIV Prevention ) The Division of Health Promotion and Disease Prevention o (Includes School Health; Maternal Infant and Reproductive Health; and District Public Health Offices) The Division of Environmental Health o (Includes Pest Control Services and Lead Poisoning Prevention Services) The Division of Mental Hygiene (Includes mental health services; developmental disability services; alcohol and drug use services; and Early Intervention services for infants and toddlers with developmental delays Services This report highlights ongoing services provided at the community level in all five boroughs. In addition to these services, the Department also provides a broad range of public health and mental hygiene services throughout the city. Some of these services include: public health education and training, disease surveillance and laboratory investigations, vital statistics registries, enforcement of the city's public health and mental hygiene laws and policy analysis and program development designed to identify and address the needs of local communities. Some services are operated directly by the Department. Such services include Immunization Clinics; Sexually Transmitted Disease Clinics; Tuberculosis Clinics; Pest Control; Lead Poisoning Prevention and School Health. Child Health Clinics are operated by the New City Health and Hospitals Corporation (HHC) through a contractual agreement with the Department. HIV Prevention services are administered by the Department and operated by both the Department and community-based organizations. The DRS includes Mental Hygiene and HIV programs contracted out to locally-based not-for profit organizations or voluntary hospitals. Page 2 of 292

8 FY 2013 District Resource Statement Child and Adolescent Health Services Child Health Clinics provide primary and preventive health care, as well as support and referral services, to infants, children and adolescents. These clinics strive to ensure that the content and quality of care provided is consistent with accepted pediatric practice standards and that their services address the needs of children in the under-served communities where the clinics are located. There are 20 Child Health Clinics currently open in New City. Each clinic is integrated into one of the seven HHC Hospital Networks and is staffed by one or more pediatricians, as well as by nurses and other support staff. In addition, each Clinic is assigned to a network administrative oversight staff (physician, nurse and administrator) who are responsible for oversight of the day-to-day Clinic operations. Clinics serve children and adolescents from birth through eighteen years of age. Each registered child or adolescent is assigned to a primary care provider for continuity and coordinated management of his/her health care. Some of the components of comprehensive care include immunizations, health education, asthma care, new admission examinations for school and summer camp, and day care examinations. Report This report contains one section: 1. 14 FY 2013 District Resource Statement Health Insurance Programs Services DOHMH Facilitated Enrollers are based in 9 DOHMH Health Centers located in five boroughs (one is temporarily closed for renovations). Working in the community and through home visits, their efforts help families complete the application process; enabling individuals to enroll in public health insurance; educating applicants about managed care plan options; helping the applicant select a doctor and health plan; and assisting each applicant to navigate through the health care system. DOHMH facilitated enrollment staff have screened 66,599 New ers and qualified 9,442 adults and children into public health insurance programs over the past year. Report 1. 17 FY2013 District Resource Statement IMMUNIZATION Services The mission of the Bureau of Immunization is to prevent the occurrence and transmission of diseases for all age groups through immunization, education, health system support and surveillance. Our goal, set by Healthy People 2020, is that 80 percent of children are appropriately immunized by their second birthday, and that 90 percent of adults aged 65 years and older receive an annual influenza vaccination, as well as a pneumococcal vaccination once in their lifetime. In FY 2013 the Bureau provided vaccination services to the public at three walk-in clinics in Brooklyn, Queens and the Bronx. Report Clinic Usage Report: This section shows the number of immunizations given broken out by walk-in clinics 1 where they were administered. 1 Data are derived from the Bureau of Immunization s electronic health record, called the Screening and Immunization System (SIS). 19 FY2013 District Resource Statement Tuberculosis Tuberculosis Services The mission of the Bureau of Tuberculosis Control is to prevent the spread of tuberculosis (TB) and eliminate it as a public health problem in New City. The goal of the Bureau of TB Control is to identify all individuals with suspected or confirmed TB disease and ensure their appropriate treatment, ideally on a regimen with Directly Observed Therapy. The Bureau also works to ensure that individuals who are at high risk for progression from infection to active disease (e.g., contacts of active cases, immuno-compromised individuals, and recent immigrants from areas where TB is widespread) receive preventive treatment and do not develop disease. The Bureau achieves its goals through case management, education, surveillance, outreach, and direct patient care at four Chest Centers throughout the City. Report The Tuberculosis report contains two sections 1. Indicators Report by CD of Residence: This section shows TB cases, and TB suspects by community district of residence, regardless of which chest center was used Number of active TB cases: Number of active TB cases citywide during Calendar Year 2012 Suspect TB cases: Number of suspect cases of citywide TB during Calendar Year Clinic Usage Report: This section shows the number of clinic visits to DOHMH Chest Centers for Calendar Year 2012 Clinic volume reductions In 2012, the NYC TB testing requirement for secondary school age children entering the NYC school system for the first time was eliminated. 26 FY2013 District Resource Statement STD STD: Introduction Services The mission of the Bureau of Sexually Transmitted Disease (STD) Control is to promote healthy sexual behavior and reduce the impact of sexually transmitted infections in New City. In addition to policy development, research, and education, the Bureau maintains 9 full service STD clinics across the five boroughs, which provide free and confidential STD testing and treatment, as well as free, confidential and anonymous HIV testing and counseling (T&C). The Bureau also monitors STD cases citywide to ensure appropriate follow-up and treatment. Follow-up activities include outreach efforts that seek to identify other individuals infected with or incubating STDs. Report 1. Clinic Usage Report: This section shows the following statistics, broken out by clinic. Total Physician Visits: All physician visits include a medical history and physical exam with diagnostic tests (may include syphilis, gonorrhea, Chlamydia, HIV tests, and interview for partner elicitation). Total Non-Physician Visits: Includes visits for express visits, test results, treatment, or visits for interview for partner elicitation. If included as part of a physician visit, such services are counted only once, in the category Total physician visits. (Note: Prior to FY08, counts for nonphysician visits included delivering results via phone) Total STD Treated: Sum of STD cases diagnosed and those presumptively treated. STDs Page 23 of 292

29 FY2011 District Resource Statement HIV PREVENTION SERVICES Services The HIV Prevention Program serves as the central coordinating unit for HIV prevention services within the Department of Health and Mental Hygiene. Its purpose is to provide an effective medium for communication and action on HIV/AIDS issues between the Department, public and private sector agencies, prevention planning groups, and research institutions. The Program maintains a leadership role in planning and implementing City policy related to HIV prevention education and technical assistance. In addition to operating its own prevention programs, the HIV Prevention Program funds and oversees numerous community-based HIV/AIDS prevention service organizations. Contracted organizations receive technical assistance necessary to plan and implement their HIV programs. Report The HIV section included in this report is based on information provided by the HIV Prevention Program. The report provides a comprehensive listing of contractual as well as directly operated HIV prevention programs throughout the City. Information provided includes program name, site name, address, program budget, initiative, and District. Also included is one map that displays the sites of service for the prevention contracts. The geographic boundaries on the map denote community districts. Three (0.2%) of the 1654 HIV service sites could not be mapped due to incomplete geocoding information. On pages , the agency s total budget for an initiative is listed for each of the sites involved in the particular initiative. For example, Hetrick- Martin s total budget for their Co-factors of HIV Transmission program is $136,000, and this amount is listed for each of their service sites. Page 26 of 292
