INTRODUCTION 1.1 PURPOSE OF GUIDELINE The purpose of this guideline and attached appendices is to provide each Area Health Service with data standards applying to the recording, naming, identification of assets, locations and work procedures within HealthAMMS. Data standards are the first building block in the effective use of asset and maintenance management systems and to meet the strategic objectives of best practice asset management. Standards are necessary to ensure that data management, recording and reporting is consistent across NSW Health and performance indicators can be measured at the varying levels of the organisational structure. The application and ongoing management of data standards will ensure that the use of HealthAMMS and management of assets will deliver long term benefits in both the improved condition of assets and reduced cost in the maintenance and management of assets. 1.2 STRUCTURE OF THE GUIDELINE The guideline is structured into three sections with the body of the guideline providing an overview of the data standards and rules for data standards. The key sections of this document are: 1. Location Standards and Hierarchy 2. Asset Standards and Hierarchy 3. HealthAMMS Standards In conjunction with the data standards, the appendices provide the full detail of location and master asset data lists for all assets owned and maintained by the AHS in the delivery of their services. These data standard lists are to be managed and maintained by appropriate committees. These committees will ensure that the master lists are up to date and that any changes, additions or deletions are undertaken using the appropriate procedures. 1.3 DOCUMENT ROADMAP The HealthAMMS Library consists of documents related to: 1. Overview of asset maintenance and management 2. HealthAMMS business case and implementation 3. Asset Management guidelines 4. Policies and procedures 5. HealthAMMS documentation. LOCATION STANDARDS AND HIERARCHY This section of the guideline outlines the adopted location hierarchy used within HealthAMMS and provides guidance in the development of location codes and standards. 2.1 LOCATION HIERARCHY The adopted hierarchy within HealthAMMS is based on the original hierarchy implemented at pilot sites (South Western Sydney, Hunter and New England) and another HealthAMMS site (Illawarra). The original hierarchy, which consisted of 7 levels, has now been revised to a new hierarchy consisting of 5 levels with the Sector level and the Node level becoming attributes of the site and room respectively. It should be noted that it is not necessary to use sector or node unless they are required. These facility codes will be used as attributes of the site. Site codes will be AHS defined in accordance with a four-character alphanumeric code and can be the DOHRS code if suitable. These codes will be used in the development of the location codes. An example of this code is: Coding Description Comment Q230 John Hunter Campus DOHRS code for John Hunter Campus CCHS Cessnock Community Health Service A local code has been used for the Community Health Service site in Cessnock A complete list of the DOHRS codes are contained in Appendix A Statewide Site Codes Building The AHS is able to determine the local building level codes based on its own requirements. A building will be any structure that is individually recognised as a separate building. There may be instances where two buildings may have been joined either structurally or for functional purposes. If the buildings are still referred to as separate buildings then they may have a separate building identification or if the two buildings are now identified as one building then a single identification may be used. 8 The AHS will be able to determine what buildings need to be identified separately and determine the coding for the building. The building coding is restricted to a four-character alphanumeric field. Examples of this coding are: Coding Description Comment JF04 Thwaites Building JF in this coding represents James Fletcher campus JH01 John Hunter Hospital JH in this coding represents John Hunter campus Level A recommended list for the different levels within a building has been developed and can be used as the level code for the location code. The coding structure should best meet the requirements of each individual AHS Room Room codes are local to the Area and/or site. Accordingly, the code used should be based on the existing location codes (if used) or a coding structure that will allow ease of identification of the room. It should be noted that the room code does not have to represent the colloquial room number or identification. As an example the Hunter Area Health Service uses the following coding structure for their rooms at John Hunter Hospital Number: JH FM Space #: 1099 (Auto-generated space number) Area Description: Biomedical Colloquial Name: Room 1311 Biomedical File: apo103x.doc Page 9 of 181 Issue: 0.5

9 The local number is an identification code with the following structure: JH = Site or Campus (John Hunter) 01 = Building Code (John Hunter Hospital) 1311 = Room number (this is the number shown on the door to the room) The recommended numbering structure that can be adopted in the absence of an existing numbering system is: SS.BB.RRRR SS = Two alpha character campus/site code BB = Two numeric character code for building number RRRR = Four numeric code for the room number (in this example the first number represents the level, for example 2131 is the 2 nd level and so forth) Room codes can have up to 12 characters in the code Physical Structure Code By using the guidelines the following code structure will create the location codes within the hierarchy. Based on the above example, the full room code will be as follows X140 Q230 JH Area Code Facility Code Building Code Level Code Room Code The complete code would be structured as: X140-Q230-JH Location description: Newcastle, John Hunter Campus, John Hunter Hospital, Level 01, Room LOCATION ATTRIBUTES For each location a number of attributes will be required so that future business processes can be incorporated to use the data from HealthAMMS. These parameters will be applied at the room level and are discussed below Space Type Within the HealthAMMS system space type has been developed based on space Design Series Classification. This space type is contained in Appendix B HealthAMMS Space Types Space Uses The space use codes have been developed based on the Health Planning Unit Guidelines. 10 2.2.3 Program Funding Code An additional space attribute that is required is the identification of the program funding code to each space. In the first instance of the base implementation of HealthAMMS the program with the highest use will be assigned to the room or space Space Dimensions The space dimensions will be calculated automatically during the Aperturisation of the drawings/plans. The dimensions of the room calculated will be: 1. Circumference (mm) 2. Area (m 2 ) 3. Height (mm) This height dimension will need to be added manually from field measurements. ASSET STANDARDS AND HIERARCHY This section of the guideline outlines the asset hierarchy used within HealthAMMS and will provide accurate referencing and identification of assets. 3.1 ASSET HIERARCHY The following hierarchy is used with HealthAMMS to allow users to search and query asset information and to allow the development of reports based on groups and types. The asset hierarchy is a typical 5 level hierarchy used by other asset owners in managing their physical assets. Asset Class Asset Group Asset Type Asset Subtype Asset/Equipment Asset Class The asset class is the highest level in the hierarchy and divides assets into the following broad asset groupings: 1. Building Fabric includes all building fabric such wall finishes, doors, floor finishes etc 2. Building Services includes mechanical, hydraulic, electrical and fire protection services 3. Facility Equipment includes FF&E, office equipment, training and education equipment 4. Infrastructure, Grounds and Gardens includes all external assets and tools 5. Medical Equipment includes all medical equipment from patient care to imaging, laboratory etc 6. IT & Communications includes all IT equipment and communication equipment such as PCs, servers, mobile phone, PABX, Nurse call system 7. Security includes alarms, CCTV, monitors etc 8. Laundry Equipment includes washing machines, dryers, presses etc 9. Catering Equipment includes kitchen equipment, dishwashers, delivery systems etc File: apo103x.doc Page 12 of 181 Issue: 0.5

12 3.1.2 Asset Group The next level in the hierarchy is the Asset Group level that groups assets into similar type assets under the Asset Class. The Asset Group expands on the description and identification of the asset and a full list of the asset groups is contained within Appendix D Asset Groups by Asset Class Asset Type/Subtype The last two levels of the asset hierarchy are the asset type and subtype. It is at these levels that the asset description is completed. It is also at these levels that standard statewide equipment code will be assigned. These codes have been split into two types of equipment: 1. Medical Equipment 2. Facility Equipment (all non-medical equipment) The medical equipment code is denoted by the letter M at the beginning of the code. A subcommittee of the Biomedical Engineering Advisory Group (BEAG) manages this medical equipment master list code. All changes in relation to naming conventions and equipments codes are subject to review and approval by the BEAG subcommittee. BEAG are currently investigating the use of a new nomenclature system being developed called the General Medical Device Nomenclature (GMDN) system as the coding system for use with medical equipment. A complete list of the M codes is contained in Appendix F Medical Equipment Master List. For the Facility Equipment an F code will be assigned to all asset type and subtypes. The difference between coding for facility equipment and medical equipment is that the codes for medical equipment are manufacturer and model specific while F codes are only asset type/subtype specific. A HealthAMMS subcommittee will manage the F code master list and will review and approve all changes in relation to this master equipment list for non-medical equipment. A complete list of the F codes is contained within Appendix E Facility Equipment Master List. Please note that these equipment codes do not identify individual assets or equipment rather they identify the type of asset/equipment. These codes will be used in the future for purchasing, monitoring and updating of assets/equipment Asset Description The asset description will be a combination of the asset type and subtype names. It is at this level that the asset will exist in the system and have all the asset information and maintenance completed. Further details of the asset information are described in the following section of this guideline. 13 3.2 ROOM ASSETS In order to maximise the efficiency of HealthAMMS and reduce the management of assets within the system, each room or space has been designated as an asset. When the user is looking at the assets within a room or space, one of the assets will be the room itself and the asset number will be the room number. The purpose of creating the room as an asset is so that the typical assets within a room can be designated as subcomponents of the room and thereby reducing the number of assets maintained within the system. The room asset attributes are shown in the following table and further details regarding the value lists for each attribute are contained in Appendix G Room Asset Standards. Furthermore a condition rating can be recorded for each of the subcomponents. The benefit of establishing a room as an asset and assigning the above assets as subcomponents means that the number of assets per room has been reduced by up to 11 assets. As an example, John Hunter is a 3,000 room hospital and will have 33,000 less assets in HealthAMMS to manage. 3.3 ASSET DATA STANDARDS For each individual asset or system of assets additional information is required to identify and manage the asset. The following items have been identified and guidelines for their population stipulated Asset Equipment ID Every asset collected is to be assigned a unique alpha numeric Equipment ID. Equipment ID s should be in one standard format throughout the Area Health Service. Issues to consider when devising an Equipment ID format as follows: File: apo103x.doc Page 14 of 181 Issue: 0.5

14 whether the assets belonging to a facility or AHS can be readily recognised by the ID? If required this could be achieved by having a fix length prefix denoting the asset facility; whether the format will allow easy detection of input errors? Standard lengths for ID components make it easier to detect errors and easier to sort data e.g. by using AE rather than AE500; whether the format will be of sufficient size to allow for future growth in asset numbers? As Equipment ID s must be unique to the AHS it is important that their issue and use is centrally controlled and monitored through a process at the local level. Format: UPPER Field Size: Locations Assets must be assigned a valid location code in order for them to be located within HealthAMMS. The location code assigned should represent the area able to be serviced by the asset. Based on the broad asset definitions, outlined in Section 3.1.1, the following criteria apply: Non-Fixed Assets - Location Code should represent the area in which the asset could be reasonably assumed to be used in. Most commonly this would be the Department or Level Location Code but in some instances could extend to a Building or Site Location Code. Fixed Assets - Where the asset is wholly located in a room, for example an electric wall heater, it should be assigned a room based Location Code - Where the asset is part of a system, for example ducted air-conditioning, it should be assigned a code representing the area serviced. Most commonly this would be a Site or Building Location Code. Format: UPPER Field Size: GL Account Code The GL account code in part defines the cost centre financially responsible for an asset. The GL Account Code must be one that is valid and in current use by the Area Health Service. Prior to any data collection being commenced, the AHS should define a methodology for the collection of GL Account Codes. This methodology should consider the following issues: What asset types will generally have a GL Account Code related to their Location Code? What asset types will generally have a standard GL Account Code, within a defined area, no matter where they are located? What asset types have a complex GL Account Code assignment process and how will they be collected? Who will be responsible for ensuring GL Account Codes are correct? Generally the AHS GL Account code will follow that of the Corporate Real Estate model for the definition. Format: ALN Field Size: 17 File: apo103x.doc Page 15 of 181 Issue: 0.5

15 3.3.4 Make The make field should contain the short name of the asset make or manufacturer. Where possible all assets should be assigned a make conforming to the following guidelines: make names should have the first letter of each word capitalised; where the make is initials (e.g. IBM) the whole brand should be capitalised; make names should not be abbreviated; company status indicators (e.g. inc, corp, pty & ltd) should not be included; where details of both make and manufacturer are present the make shall be used as the make and the manufacturer shown as a pre-fix to the Model. Format: UPPER Field Size: Model The model field should contain the information relating to the identification of the asset type, generally this is a model code and/or a build/type code. Model codes are common on mass produced items whereas small run or one off plant often will only have a build/type code. Where possible all assets should be assigned a model with coding conforming to the following guidelines: where model code is in text format the first letter of each word should be capitalised; where the model code is initials (e.g. TTX99) the whole code should be capitalised; model codes should not be abbreviated; build/type codes should be prefixed by BC: and presented wholly in uppercase; where both model and build/type code are available the format is Model BC:Build Code. Format: ALN Field Size: Serial Number The asset serial number is only required for certain asset types, outlined in the Guideline for Asset Data Collection (APO1030). When recording serial numbers extra care is required as mistakes in this area are not easy to detect. Serial numbers should be collected in the following manner: recommend that the complete serial number without spaces or dashes; do not prefix with S/N or any other designation; ensure the correct number is recorded; ensure the barcode or number copied relates to the serial number and not some other designation (e.g. model code). It should be noted that the combining of model code and serial number on a single barcode is common practice for most products. Format: ALN Field Size: 40 File: apo103x.doc Page 16 of 181 Issue: 0.5

16 3.3.7 Vendor / Supplier The vendor detail is only required for certain assets, detailed in the Guideline for Asset Data Collection (APO1030). When recorded, the vendor field should be entered with coding conforming to the following guidelines: vendor names should have the first letter of each word capitalised; where the vendor is initials (e.g. IBM) the whole word should be capitalised; vendor names should not be abbreviated; company status indicators (e.g. inc, corp, pty & ltd) should not be included; vendor coding will come from the financial system. Format: UPPER Field Size: Ownership Status The Ownership Status is required to indicate whether an asset is leased or owned by the AHS. The Ownership Status field should be completed with leased, owned or rented. A check box will also be included to identify where leased assets are maintained by the AHS. For leased assets maintained by the supplier then the warranty date will be extended for the term of the lease. Format: ALN Field Size: Legacy Asset Equipment ID Where an asset has previously been assigned an Equipment ID this should be recorded in the Legacy Asset Equipment ID field. 17 4. HEALTHAMMS STANDARDS In addition to the location and asset standards described there are a number of other standards that have been adopted for the standardisation of HealthAMMS across all AHS. These standards generally refer to the standard value lists within HealthAMMS and are described below. 4.1 RESOURCING STANDARDS Resourcing standards have been developed to allow the effective monitoring and management of resources in relation to the actual work carried out within an Area. By using the resource standards, managers are able to identify which resources are critical to service delivery and which resources are under utilised. By using the set standard of work management descriptions Users are able to identify the status of their work request or work order through interrogation of the system rather than contacting the responsible service supplier to find out what the status of a particular request is. The field in which the status is viewed and changed is: Name: Status Format: UPPER Field Size: 8 In addition to the work management standards, a standard list of work types has been developed in order to categorise and monitor the type of work being undertaking in the Area. These work types are also used to report KPIs. The standard types of work are: Code Description Meaning BD Breakdown Unplanned maintenance required in the short-term to restore an maintenance asset to working condition so it can continue to deliver the required CM Corrective maintenance service or to maintain its level of security and integrity. Maintenance is required and the maintenance unit has identified the work, primarily, as a result of preventative maintenance activities and/or condition audit. CP Capital works Is work undertaken to create new assets or to increase the capacity of existing assets beyond their original design capacity or service potential. MO Modification A facility or asset is modified if work required to be carried out changes the plant, engineering, facility of equipment design, or affects existing Standard Work Procedures or a material change. PM Preventative maintenance Maintenance carried out at pre-determined intervals or corresponding to a prescribed criteria and intended to reduce the probability of failure or the performance degradation of an asset. 19 4.3 WORK REASON In addition to the work type a reason for work can be recorded against a work request/order. This allows further reporting and analysis of maintenance issues within the health environment. 20 5. SUMMARY The implementation of data standards is critical in deriving the maximum benefits from the system and to better manage assets and maintenance. Therefore it is necessary to ensure that these standards are followed during the implementation phase of HealthAMMS. It is widely recognised that the extra effort during implementation to ensure the standardisation of data will produce better outcomes in the future and will reduce the need for rework. It is recommended to ensure that all the necessary quality assurance practices have been employed to guarantee the quality and integrity of the data that will be uploaded into HealthAMMS. It is the responsibility of all Users to follow and maintain the integrity of the data and to comply with the set data standards. This is necessary so that effective monitoring processes can be adopted and meaningful reports can be produced and this is only achievable through implementation and ongoing preservation of the data standards within HealthAMMS. It is important to note that this document is not definitive and there will be additions and deletions in the future as new types assets are acquired, old assets are disposed and business process evolve and improve. Therefore it is important that a committee or a group be tasked with the ongoing management of the data standards and their recommendations and changes are reflected in the centralised system. For HealthAMMS to produce the reports and information required at the Department level then it is necessary for all Areas to ensure that the data standards are maintained and followed at the Area level. For changes to occur it is important that a central committee agree the changes and that those changes are adopted across the state. The application and ongoing management of data standards will ensure that the use of HealthAMMS and management of assets will deliver long term benefits in both the improved condition of assets and reduced cost in the maintenance and management of assets. PU FU Planning Definition Code Code Unit/Functional Unit 1 Inpatient 1A Ward Medical/Surgical Medical/Surgical inpatient accommodation is for general medical and surgical patients. In larger health facilities this unit includes specialist medical and surgical patients, for example cardiac, neurology/neurosurgery, transplantation, infectious diseases, integrated palliative care and in smaller hospitals paediatric, palliative care and obstetric patients. Patients awaiting placement may also be accommodated in this type of facility. 1B Ward (Rehabilitation & GEM) accommodation is for rehabilitation patients geriatric and management patients (GEM), and sub-acute patients awaiting placement. It is colocated with either acute medical/surgical or sub-acute inpatient units. 1J 1K 1L 1M 1N Cardiovascular Interventional Labs (Cardiac Catheter Labs and Angiography) High Dependency Unit (HDU) Coronary Care Unit (CCU) Intensive Care Unit (ICU) Intensive Care Unit/Coronary Care Unit (ICU/CCU) The Cardiovascular Interventional Laboratories (CVIL) is a suite of procedure room/interventional laboratory and recovery trolley places for patients undergoing interventional procedures cardiac/vascular angiography. Cardiac Catheter Suites and Angiography Suites may be located in day procedural areas or discrete units; and will be termed as CVIL, as the functional and area requirements for the Interventional Rooms are comparable. High Dependency Unit is an inpatient unit that provides high dependency nursing care. In smaller health facilities, the HDU is an area of observation, within the acute ward for patients of higher acuity than general inpatients. In larger health facilities a HDU may be specialty specific, for example Cardiac Surgery HDU or as a step down from either ICU or CCU. Coronary Care Unit is a dedicated ward area for patients requiring telemetry (cardiac monitoring) and/or resuscitation. Intensive Care Unit is a dedicated unit for critically ill patients (adult and paediatric) who require invasive life support, high levels of medical and nursing care. Staff amenities such as staff property lockers, shower and toilet facilities are also included in all wards and departments. 8M Security The Security Service provides a high level of security to patients, staff and the public who enter the Health Facility. Security Services are often responsible for Fire Protection and therefore the area includes Fire Control Room. 9 Plant and Travel 9A Travel Travel is defined as the inter-unit spaces provided to allow circulation of people and goods, both vertically and horizontally eg. Corridors, stairs, ramps, tunnels, links and detached covered ways joining main buildings. 9B Engineering Engineering areas comprise of all service areas, both vertically and horizontally eg main vertical service ducts, electrical 9C Plant and Equipment Plant and equipment comprises of Central Energy plant areas, communication rooms, tank rooms and air conditioning plant rooms. 10 Discretionary Units 10A Positron Emission Tomography (PET) Positron Emission Tomography (PET Suite) is computerised radiographic technique using radioactive substances to examine various body structures. It must have access to a Cyclotron. 10B Radiotherapy Radiation Oncology provides consultative services and treatment to patients with certain forms of malignant disease by either Radiotherapy or Brachytherapy. The linear accelerators are housed in bunkers with thick concrete walls and usually with a long maze for scatter radiation. 10C Sleep Laboratories Sleep Laboratories are for the assessment of all aspects of sleep disorder medicine with a specific focus on respiratory disorders in sleep. 10D Dental A Dental Service is a specialist clinical service, where outpatient consultations and/or treatment occur. 10E Hydrotherapy Pool The Hydrotherapy Pool includes hydrotherapy pool for patients who benefit from exercising in a weight free environment during the acute phase of their treatment or as part of physical conditioning during the rehabilitation phase. 10F Ambulance Station Ambulance Stations may comprise of ambulance bays, plant area, administration area and overnight accommodation for staff. 10G Food Services, Cook/Chill Production As a production kitchen is generally a campus-wide or external provider facility, which produces and provides cook-chilled food to a receiving kitchen. 10H Car park Carparking includes accessible car parking, motorcycle parks, bicycle racks as well as car spaces. Public car parks and staff car parks may be separate. 10I Academic Research Academic and scientific research programs, which significantly enhance diagnostic and treatment regimes, may have dedicated areas with the health facility for both wet and dry research. This area is in addition to areas included in clinical areas and research. They are generally located with Level 5 or 6 health facilities. 10J GP surgeries A discrete unit for General Practitioners to consult and treat patients.
