3 Section 1: General Information 1.1 Purpose of RFP Summit Pointe has issued this Request for Proposals (RFP) to obtain an appraisal report for identified real properties to inform Summit Pointe on Market Value either individually or grouped together that promotes the highest yield of return. 1.2 Description of Summit Pointe Summit Pointe is a Community Mental Health Authority. Summit Pointe operates under the authority of its own Board of Directors. The Summit Pointe designated service area encompasses Calhoun County in the State of Michigan. 1.3 Scope of Services Requested Below is what Summit Pointe expects to need in regard to an appraisal report for identified real properties to inform Summit Pointe on Market Value either individually or grouped together that promotes the highest yield of return. Responders will provide an appraisal report for the identified real properties to inform Summit Pointe on market value based on, individual or as grouped together, that promotes highest yield of return. This will include, but is not limited to, capitalization rate and net operating income (after expense which includes, but not limited to, operational cost, reserve monies and management fees). The report will take consideration of all approaches (sales comparison, income and if applicable, cost). The report will include the data necessary to support market value. The properties are located with Calhoun County in the city of Battle Creek. Property Descriptions: 1. Lots 58 66, Inclusive, and Lot 67, except the South 10 feet of lot 67, of Lindale. According to the plat recorded in Liber 9a of Plats, page 57, in the office of the Register of Deeds for Calhoun County, Michigan. Common Address: 504 E. Section One: Section Two: Vendor Overview Information The information contained in this section is detailed in the following section of this RFP. Scope of Work and Product Delivery The information contained in this section should provide full details around the required scope of work and the product that will be provided to Summit Pointe. 6 Vendor Overview Information requirements This section of the Vendor response is designed to provide an overview of the Vendor It should contain the following information: Vendor Name, Address and Contact Information Name, Address, Telephone, cell, fax Legal name of organization, owner, tax ID Local contact information Billing/Payment contact and address Person Authorized to sign contract Copy of w 9 Errors and Omissions Insurance Vendor Key Qualifications Describe your company and what key qualifications you have to meet our appraisal needs. Ensure your response includes the following information and where applicable please provide evidence. Qualifications: Certified General Appraiser Licensed in the State of Michigan with experience in multifamily more than 4 units. The appraiser must have the appropriate license and be in good standing and have competency in, but not limited to, the subject s property market area, the necessary analytical method(s), and knowledge of, and full access to, all data sources necessary to develop and report credible assignment results. The appraisal must conform to applicable Federal and State Laws in addition to USPAP guidelines. Scope of Work and Product Delivery Requirements Appraisal Report Provide an appraisal report for the identified real properties to inform Summit Pointe on market value based on, individual or as grouped together, that promotes highest yield of return. This will include, but is not limited to, capitalization rate and net operating income (after expense which includes, but not limited to, operational cost, reserve monies and management fees). The report will take consideration of all approaches (sales comparison, income and if applicable, cost). Report will include the data necessary to support market value. The properties are located with Calhoun County in the city of Battle Creek. Pricing Methodology Clearly define all fees that may be incurred by Summit Pointe for delivery of the detailed appraisal report as identified in this request for proposals. Completion Timeframe Identify the timeframe required to complete the scope of work and deliver a final product to Summit Pointe. 6 P a ge
7 Proposal Review Summit Pointe reserves the right to request additional information or clarification from vendors, to allow correction of errors or omissions, and to waive irregularities and/or formalities when so doing may serve the best long term interests of the organizations involved. Summit Pointe reserves the right to reject any or all RFP submissions and to proceed in any other manner selected by Summit Pointe. Summit Pointe also reserves the right to discontinue the RFP process at any time and for any reason. The right to amend this Request for Information, giving equal information and cooperation to all vendors, is also reserved. Summit Pointe reserves the right to award the vendor that it believes, in its sole discretion, best meets the needs of the organization. Responses to Requests for Information are subject to the terms of the Freedom of Information Act, and will be retained by Summit Pointe. A vendor s response to this RFP may become part of the final contract. A. Proposal Retention Summit Pointe will retain all proposals submitted and all proposals become the property of Summit Pointe upon submission. B. Acceptance of Proposal Content RFP responses of selected vendor may become contractual obligations. Failure to accept these obligations may result in cancellation of the selection, and the Vendor may be required to reimburse Summit Pointe for damages incurred. C. Non Discrimination Summit Pointe Vendors shall not discriminate against a Vendor with respect to hire, tenure, terms, conditions or privileges of employment, or a matter directly or indirectly related to employment, because of race, color, religion, national origin, age, sex, height, weight or marital status, or disability that is unrelated to the Vendor s ability to perform the duties of a particular job or position. The Vendor shall observe and comply with all applicable federal, state and local laws, ordinances, rules and regulations which shall be deemed to include, but not be limited to, the Elliott Larsen Civil Rights Act and the Persons with Disabilities Civil Rights Act. D. Non Collusion The vendor certifies that this proposal has not been made or prepared in collusion with any other vendor and the prices, terms or conditions thereof have not been communicated by or on behalf of the vendor to any other firm and will not be so communicated prior to the official receipt of this proposal. This certification may be treated for all purposes as if it were a sworn statement made under oath, subject to the penalties for perjury. Moreover, it is made subject to the provisions of 18 U.S. C.
