Please accept this document concerning Payment Application No. 11 which includes Change Order No. la, lb, 2a, 2b, 6, 10, 14 and 18 for Pl- City Ridge Apartments. An ensite assessment was conducted by staff of Lynx Engineering Consulting under the Supervision of Christopher Mo ran, PE and lawrence Shapiro from Lawrence M. Shapiro, Inc. The following information contained here-in and also attached for processing provides documentation of the progress of work as obse rved in the field on 09/01/2010. Additionally as discussed in the field the activities listed below have been deferred and the balance of the avai lable funds will be used to repair and/or complete MAKE READY units that the site needs immediately. Following is a breakdown of these activities and the cost/funds ava il able as of this req uisition: 1. Install 7ft high pointed perimeter fence along wood area 2. Remove damaged stucco and repair on buildings 3. ITEM Additional Interior Abatement Prep Work Units. Activity has been 100% completed. Units were occupied at time of walk through. As per meeting held on site in the presence of Mr. Shapiro the application was reviewed and agreed that the contractor is currently performing in accordance with the agreed upon schedule and the user is satisfied with the final product. Overall project at City Ridge Apartments is currently at 71% completion. Site Observations and Follow ups: 1. Final Oversight report and O&M manuals are being prepared by URS (environmental consultant) and will be distributed under separate cover. 2. The light poles are on hold pending approval of the prototype. 3. Replacement of metal stairs and repairs to landings at breezeways is in progress. 4. Repairs to metal railings on balconies and breezeways are in progress. 5. Repair of armor-flex insulation on Freon lines has been completed. 6. New condensate dry wells have been finished. 7.
