1 ADVERTISEMENT FOR BIDS Bids will be received at the Business Operations Office, One Donham Plaza, 4th Floor, Middletown, Ohio until 9:00am on November 10, 2014 and will be opened and read immediately thereafter for: This solicitation is for the purpose of getting bids and supporting information related to improving the entrances of the schools to be more secure. No bids shall be considered if they arrive after the time noted above. Bids must be in accordance with the Invitation for Bid/Contract on forms available from the Middletown City Schools, Middletown, Ohio. The Invitation to Bid/Contract is available free of charge to prospective bidders. Said forms are available on the Middletown City Schools web site at listed under Staff and District Departments. Only electronic ( format) questions during the bidding process shall be considered and shall be addressed to Mr. George Long Business Manager, Middletown City School District, All questions must be received by November 3, Each bid shall be accompanied by a Bid Guaranty in the form of a firm commitment such as a bid bond postal money order, certified check, cashier s check or irrevocable letter of credit in the amount of no less than 5% of the bid price. Field inspection of the existing facilities will be available. This inspection has been scheduled for October 27th, 2014 at Middletown High School. It will begin promptly at 11:00 am. The address of the High School is 601 North Breiel Blvd., Middletown, Ohio The Middletown City School District reserves the right to reject any or all bids, to waive any informalities or irregularities in the bids received and to accept any bid which is deemed most favorable to the District at the time and under the conditions stipulated. The District will not be held accountable for any inaccuracies in existing site information provided herein. 4) AWARD CRITERIA The Board, by resolution, may award a bid to the lowest responsive and responsible bidder. For a bidder to be considered responsive, the proposal must respond to all bid specifications in all material respects and contain no irregularities or deviations from the bid specifications which would affect the amount of the bid or otherwise provide a competitive advantage. For a bidder to be deemed responsible, the Board may request evidence from the bidder concerning: A. the experience (type of product or service being purchased, etc.) of the bidder; B. the financial condition; C. the conduct and performance on previous contracts (with the District or other agencies); D. the bidder's facilities; E. management skills; F. the ability to execute the contract properly; G. a signed affidavit ensuring that neither the bidder nor any sub-contractor has entered into an agreement with any labor organization regarding the public improvement project. 4

7 ADDENDUM #2 SOLICITATION OF BIDS FOR IMPROVING THE ENTRANCE OF SCHOOL OPENINGS ALL PROVISIONS OF THE BID DOCUMENTS REMAIN THE SAME EXCEPT FOR THE FOLLOWING: Further clarification on the specifications to include: Contractor to provide and install Video Aiphone intercom system at nine district school locations: Amanda, Central Academy, Creekview, Highview, Mayfield, Miller Ridge, Wildwood, High School and Middle School. The system installed at each location will consist of a JM Series Master Station, with one additional desk station and one vandal resistant door station. Contractor will install an electric strike at the main entry of the High School and Middle School and will tie into existing electrified crash bars at the other seven locations. 7
