Updated ECDC Public Health Microbiology Strategy and Work Plan Marc Struelens Microbiology Coordination Section, Resource Management and Coordination Unit Ole Heuer Surveillance Section, Surveillance and Response Support Unit Joint Annual Meeting of the Antimicrobial Resistance and Healthcare-Associated Infections (ARHAI) Networks, Warsaw, 23 November, 2011
The changing paradigm of microbiology Source: Sintchenko Arch Pathol Lab Med. 2009;133:916
Integration of molecular typing into EU surveillance why? Source: ECDC Concept paper on molecular surveillance;feil Nat Rev Microbiol 2004;2:483 Key tool for detection, investigation and monitoring of emerging pathogens and outbreaks NA sequencing offers «universal language» Need for technology evaluation, epidemiological validation and guidance Need for coordinated implementation based on EU public health benefits and feasibility
Enhanced surveillance of antimicrobial resistance why? Public health threat due to rise in AMR across the globe Pandemic spread via cross-border care, trade and tourism Narrow scope and slow reporting cycle of AMR surveillance in humans Multiple EU surveillance systems for AMR: o by pathogen (the big 7, gonococci, zoonotic agents, ) o by sector: in human, veterinary and food safety Image Source: APUA; EARS-Net
Updating the ECDC public health microbiology strategy why? 2007: assessment of dedicated surveillance networks & definition of specific surveillance and microbiology objectives 2010: - definition of core public health microbiology functions of reference laboratories - ECDC mapping of national reference laboratories in the EU - EURLOP project on EU reference laboratory options 2011: - Microbiology Coordination Section and Public Health Microbiology Programme - Position statement of the Commission and ECDC on human pathogen laboratories: a joint vision and strategy for the future
ECDC Microbiology Coordination - partners ECDC EXTERNAL Microbiology Steering Committee Support Sections Disease Programmes Microbiology Coordination Section Commission National Microbiology Focal Points & Advisory Forum EU Agencies WHO Training Learned societies, other stakeholders
Vision for ECDC support to EU public health microbiology system By 2016, ECDC facilitates the operation and fosters the development of an efficient public health microbiology system capable of providing timely and reliable information for infectious disease prevention and control at Member State and EU levels Source:wikimedia.org
Updated public health microbiology strategic priorities, To facilitate MS and EU prevention and control of infectious diseases 1. To consolidate the capacity of the EU public health microbiology system 2. To develop and implement a system for monitoring microbiology laboratory capabilities for European surveillance and epidemic preparedness 3. To develop and implement a roadmap for integration of molecular typing into European surveillance and epidemic preparedness 4. To further develop the EU integrated surveillance and epidemic intelligence of antimicrobial resistance in human and zoonotic pathogens
Strategy 1: To consolidate the capacity of the EU public health microbiology system Raising awareness of public health microbiology Coordination of ECDC microbiology programme Coordination of EU networks of reference laboratories Promotion of quality assurance and assessment Technical guidance and standardisation of laboratory methods Support to assessment and application of new laboratory methods Communication and information dissemination Support for microbiology training
Strategy 2: To develop methods for monitoring the capacity of the EU public health microbiology system Development of draft generic and pathogen specific capacity for public health microbiology system identification of core capability requirements for EU surveillance and epidemic preparedness with key stakeholders Development of criteria for disease priority setting Pilot testing of capacity indicators on 3 diseases, stakeholder consultations and revision of generic and specific appraisal tools Gradual implementation of indicators and monitoring for priority diseases
Strategy 3: To develop and implement a roadmap for integration of molecular typing into EU surveillance Systematic review of public health effectiveness of molecular typing and evidence based guidance on pathogen and laboratory methods of potential EU added value Expert consultation on technological feasibility and typing network priorities and public health benefits of molecular typing Development of roadmap for integration of molecular typing in EU surveillance Consultation of key stakeholders Implementation and annual review of roadmap
Strategy 4: EU surveillance and epidemic intelligence of antimicrobial resistance Objective Year Standardise AMR testing in Europe by adoption of EUCAST test methods and breakpoints Integrate surveillance methods and reporting of antimicrobial resistance in zoonotic and foodborne bacterial pathogens Develop enhanced surveillance for timely monitoring of epidemic carbapenemase-producing and XDR bacterial pathogens Assess feasibility of new EARS-Net data collection and analysis modules for enhanced AMR surveillance Integrate EU surveillance of antimicrobial resistance in viral, bacterial and fungal pathogens of public health importance Advise on methodology of microbiological AMR prevalence surveys
