Instructions for Domain Name Registration The Application Form must be completed online to register, modify or delete a Domain Name in BermudaNic top-level Internet domain (.BM). A separate Application Form must be completed for each Domain Name to be registered, modified, or deleted. Any queries, comments or questions should be submitted to DNSManager@gov.bm Presence Requirements To have a.bm Domain Name registered, modified or deleted you must be: (a) A Corporation, entity or organization that is incorporated in Bermuda under the provisions of the Companies Act and any subsequent amendment thereto; or (b) A Registered Charity which includes an unincorporated organization, association, or club registered under the Charities Act; and any subsequent amendment thereto; AND Fully complete the Application Form Submit on the corporate letterhead of the Applicant /Registrant Organization a summary of the purpose of the application. This letter which must be signed by an authorized person should preferably be faxed to or hand delivered or mailed to the : DNSManager- BermudaNIC Registry General Department Intellectual Property Section Fourth Floor Government Administration Building 30 Parliament Street Hamilton, HM 13.

Section 1 General Information The current Domain Name Registration Authority for.bm Domain Names is the Registry General. It is the policy of the Registry General that the only.bm Domain Names that can be registered are as follows: "yourcompany.bm. "yourcompany.com.bm" "yourcompany.edu.bm" "yourcompany.gov.bm" "yourcompany.net.bm" or "yourcompany.org.bm", This section should contain the complete name of the Domain Name that you wish to register, modify or delete including the.bm. You are required to indicate the procedure for which you are applying. New Registration For new registrations, you must choose the option New Registration. Modify Existing Domain For modifications, you must choose the option Modify Existing Domain. Changing an existing record is done by replacement. That is, the contents of various fields in the database are replaced with new information from the Application Form. If the modification involves registering a Contact or Name Server that is not in the database, you are required to provide all the relevant information pertaining to the New Contact or Name Server. The Domain Name itself CANNOT be modified. You are required to briefly describe the purpose of the modification. If the Applicant is not a currently listed Contact, you are required to explain your relationship to the current Contacts and/or the organization holding the Domain Name. 2

Delete Domain For deletions, you must choose the option Delete Domain. A Domain Name may be removed from the BermudaNIC database and hence from the Root Servers under the following conditions: (i) a request for deletion comes from an authorized source (ii) as part of a transfer to another organization A request for deletion should come from the Registrant, Administrative or Technical Contact. When a Domain Name is transferred from one organization to another, the BermudaNIC will first delete the existing record and then process a new registration. Two separate Application Forms are REQUIRED to transfer a Domain Name. The first Application Form requests a DELETE of the Domain Name by the current Domain Name holder. The second Application Form requests a NEW Domain Name registration from the organization seeking to obtain the Domain Name. Section 2 Organization (Registrant) Information The Domain Name is considered to be registered to either an individual or an organization. It is important in this Section to list the name and address of the end-user organization, not the provider organization. Please be reminded that Applicants must meet the Presence Requirements as stated above. Section 3 Contact Information This section should contain the records to be placed in the DNS for the Domain Name. 3

Section 3A Administrative Contact /Agent The Administrative Contact is the person who can speak on behalf of the organization listed in Section 2. This person should be able to answer nontechnical questions about the organization's plans for the Domain Name and procedures for establishing Sub-Domain Names, and should be able to represent the organization regarding use of the Domain Name. Please note that the street address, city, country, telephone number, and address are required. The registration of Domain Names for which the Application Form does not have the required information will not be processed. Section 3B - Technical Contact The Technical Contact is the person who tends to the technical aspects of maintaining the Name Server for the Domain Name, resolver software, and database files. This person keeps the Name Server running, and interacts with technical people in other domains to solve problems that affect the Domain Name. The Internet Service Provider often performs this role. The procedures for completing Section 3B are the same as those for Section 3A. The Information Technology Office performs the role of physically administering the Bermuda Name Server on behalf of the Registry General and the Government of Bermuda. The Technical Contact for.bm Domain Names would however be the Technical Staff contact at your firm. If the Technical Contact is missing from the Application Form, the Administrative Contact will be listed as filling both roles. If the Contact is an individual, enter INDIVIDUAL and list the name as follows: LAST NAME, FIRST NAME, MIDDLE NAME For example: SMITH, John X. If it is a group or organization where several individuals may be acting in that role, enter ROLE. If the Contact is a role account, list the name of the role account and the name of the organization the role account represents. If the name of the role account is the same as that of the organization, simply list the name of the role account. 4

At least one line of address information is REQUIRED. This address should be the postal mailing address for any correspondence directed to the Contact. Every Contact is REQUIRED to provide at least one valid telephone number. For informational purposes, you may supply a fax number. A valid Internet address is REQUIRED for all Contacts. Separate the username and hostname parts of the mailbox by symbol. A Contact can only be changed by replacing it with another individual or role Contact. Section 4 Name Server Information Applicants for Domain Names should provide at least two Name Servers for translating names to addresses for hosts in the Domain. Incomplete information in Section 4 will result in delay of the registration and activation of the Domain Name. You may list up to five (5) additional Name Servers that will be authoritative for this Domain Name. The Name Servers should be in physically separate locations on a different network, if possible. The Name Servers should be active and respond to Domain Name System (DNS) queries BEFORE this application is submitted. Most ISP s can provide one or more Name Servers if you do not have your own. Please provide the Fully Qualified Domain Name (FQDN) of the Host that is to be the Name Server. For example, enter NS.EXAMPLE.COM and not just NS. DO NOT list Name Servers if you do not have permission from the owners to do so. Listing Name Servers without the explicit approval of the owners is not only unethical, but can cause operational problems for the Name Servers listed. NOTE: Administration fees may be imposed by organizations who are administering your Domain Zone files. Please see your Internet Service Provider for more details. When modifying the Name Server list, provide the COMPLETE LIST of Name Servers The same instructions for creating a new Domain Name applies. 5
