1 Professional Preparation Program for Teacher Licensure & M.Ed. 900 Dayton Street Yellow Springs, Ohio midwest.antioch.edu Checklist for Application Application Application Fee A non-refundable fee of $75 in U.S. funds, payable to Antioch University Midwest, should accompany all applications. Transcripts from all colleges and universities previously attended. (Be sure to include transcripts for programs for which no degree was awarded.) Transcripts should be sent directly from the institution to the address below. Indicate the name of the program to which you are applying. If you attended a college or university outside the U.S., please check with the Office of Student Services. Alert the Office of Student Services if any transcript will be in a former name. Applicant Transcript Evaluation (AYA and MCE only) (Please include your Applicant Transcript Evaluation when submitting application with letter grades and content grade point average.) Second License Candidates Only - Must provide a copy of their current Ohio teaching license Educational Goals Statement Refer to page 3 of the application for instructions on completing your educational goals statement. Résumé The resume should include both educational and professional background and any volunteer or certificate training programs, especially as they relate to the teaching field. Skills Assessment - Contact Vicky Cook at (937) Refer to the following page for instructions on completing your skills assessment. Fingerprints - Contact Vicky Cook at (937) Education candidates are required to complete their back ground checks (BCI&I and FBI) through the School of Education at Antioch University Midwest. Candidates will not be permitted to enroll in any teacher licensure program until AUM has received a clear background checks for BCI&I and FBI. Background reports are valid for one year and must be current for all supplemental learning activities and clinical experiences. The fee is $55 and may be paid by cash or check. Interview Applicants will interviewed the applicable Program Chair and admission is based on the profes sional discretion of the Education Admissions Committee. 2 overview of admissions process Teacher Licensure Admissions Procedures The application must be completed according to instructions contained on the application form provided, and submitted by the applicable deadline. The application fee is non-refundable. Applicants must arrange for official transcripts to be sent from all institutions previously attended. It is the responsibility of the applicant to ensure that the application, with all necessary supporting documents and fees, are complete and in order. When the application is complete, it will be reviewed by committee and a faculty interview will be scheduled. Admission is based on the professional discretion of the Education Admissions Committee. Prerequisites for Admission Applicants are required to have the following when seeking admission into Antioch University Midwest's Teacher Licensure program: A bachelor s degree from a regionally accredited institution. Overall GPA of 2.5. Access to internet. An interview with the Program Chair. A math, writing, reading, critical thinking and literacy skills assessment (see below), conducted by Antioch University Midwest Teacher Education Program. Important: The assessment, while part of the applications process, is used as a tool to assess your skill level. The assessments are factored into your overall application results, but will not be the determining factor of your admissions. Assessment Testing The assessment test will take approximately one hour. 3 Educational Goal statement Below are the four tenets from the Antioch University Midwest School of Education Conceptual Framework, which characterizes our education programs. 1. Integrator of Theory to Practice 2. Reflective Practitioner 3. Critical Thinker 4. Educator as Change Agent Please select two to three of the tenets and write a goal statement in which you: 1. discuss your personal responses to those elements you chose and; 2. share your thoughts about how they are reflected in your future professional practice as a teacher Your statement should be electronically produced or typed and included in the packet of material you send to the Office of Student Services.
