1 Fix Our Roads Alachua County: Funding Road Improvements by levying a ¾% sales surtax Renewal of the Existing One Mil Ad Valorem Tax for School District Operating Expenses Operation of Slot Machines at Melbourne Greyhound Park Alachua County Shall Alachua County implement a Road Improvement Plan to extend the life and improve the safety of county roads by maintaining, paving and reconstructing roads; and in the Cities of Gainesville, Alachua, Newberry, High Springs, Hawthorne, Waldo and Archer, and the Towns of Micanopy and LaCrosse, funding municipal road maintenance, construction, reconstruction and paving projects; by levying a ¾% sales surtax for 15 years subject to independent audit and citizen review? Shall the Alachua County School District s existing one mil ad valorem tax be renewed, beginning July 1, 2013, and ending four years later on June 30, 2017, for necessary operating expenses to fund school nurses, elementary music and art programs, K-12 school library programs, K- 12 guidance programs, middle and high school bands and chorus programs, academic/career technical magnet programs and to update classroom technology; with oversight by an independent citizens committee? BREVARD COUNTY Shall Melbourne Greyhound Park be allowed to operate slot machines in accordance with the authority granted by Florida law? 1

2 One-Half Cent Sales Surtax for School Facilities and Technology; and Eliminate One-Quarter Mill Property Tax Referendum Regarding Levying for Four Years 0.25 Mills for Necessary Operating Expenses of School District Ordinance Amendment to Permit Sunday Alcoholic Beverage Sales at Same Times As All Other Days As the School Board will stop levying the critical needs 0.25 mills property tax in , shall it be allowed to levy a onehalf cent school capital outlay surtax on sales in Brevard County, effective 1/1/13 for a period of 10 years unless property tax assessments return to the level. Proceeds will be used for critical improvements to school facilities and educational technology needs as described in a School Board resolution adopted 7/10/12. CITRUS COUNTY Shall the School District of Citrus County add a total of 0.25 mills to the ad valorem millage for necessary operating expenses to maintain academic programs and retain teaching positions for the fiscal years beginning July 1, 2013 and ending four fiscal years later on June 30, 2017? CLAY COUNTY Shall an ordinance be adopted amending Clay County Code Sec. 3-2 by revising subsection (b) to change the 2 p.m. to 12 a.m. Sunday alcoholic beverages sale and consumption hours within unincorporated Clay County to the period from 7 a.m. to the following 2 a.m., consistent with all other days, subject to specified limitations; deleting subsection (d) regarding December 31 alcoholic beverages sales; redesignating certain subsections; providing an effective date? 2

3 Renewal of Tax Neutral Flexible Funding for Collier County Public Schools Establishes Citizen Advisory Committee to Make Recommendations to the Board of County Commissioners Regarding Redistricting Providing Additional Method for Removal of the County Manager by the Board of County Commissioners Removes a Department Head s Option to Appeal Termination by County Manager to the County Commission COLLIER COUNTY Collier County School District proposes to renew the 2008 voter approved referendum to allow the District the option to reduce the capital tax millage by.25 mills and increase the operating millage by.25 mills to maintain economic stability. This will not result in any increase to the taxpayer. Shall the School district reduce the capital millage and increase the operating millage each by up to.25 mills for a four year period? COLUMBIA COUNTY Shall Article 2, Section 2.2 of the Home Rule Charter for Columbia County, Florida be amended to provide for the creation of a Citizen Redistricting Advisory Committee composed of registered voters residing in Columbia County to advise the Board of County Commissioners during the redistricting process and setting forth critical timeframes for action? Shall Article 2, Section 2.8(1) of the Home Rule Charter for Columbia County, Florida be amended to allow the Board of County Commissioners to remove the County Manager by a majority plus one vote of the entire board at one meeting, allowing the one meeting to be a special meeting, in addition to the existing option of by a majority vote of the entire board at each of two successive regular meetings? Shall Article 4, Section 4.2 of the Home Rule Charter for Columbia County, Florida be amended to remove the ability of a department head to appeal his or her termination by the County Manager to the Board of County Commissioners? 3

4 Providing for Appointment of Qualified County Attorney by Commission; Abolishing Elected Office of County Attorney Non-Binding Referendum to Acquire and Manage Lands Deemed Critical to Hernando County s Environment Non-Binding Referendum Regarding the Continuance of Mosquito and Other Pest Insect Control Services in Hernando County Shall Article 5, Section of the Home Rule Charter for Columbia County, Florida be amended to supercede Laws of Florida Ch , No. 997 to remove the elected office of County Attorney; amend the title of Article III; create Article 3, Section 3.6 to establish the Office of County Attorney, provide authority to the Board of County Commissioners regarding the hiring and firing, qualifications, powers, duties and terms of employment of the County Attorney? Hernando County In 1988, Hernando County s voters approved a property tax levy, at a rate of one-tenth of one mill (.1 mil), to fund the acquisition and maintenance of environmentally-sensitive lands. That levy ceased in The referendum asks whether the levy shall be resumed. Shall the Board of County Commissioners levy a property tax of up to one-tenth of one mill (.1 mil) to fund the acquisition, management, and maintenance of environmentally-sensitive lands, with the program to sunset on September 30? Since 2011, Hernando County has funded the provision of mosquito and other pest insect control services with a property tax levy, at a rate not to exceed one-tenth of one mill (.1 mil). The referendum asks whether the levy shall be continued. Shall the Board of County Commissioners continue to levy a property tax of up to one-tenth of one mil (.1 mil) to fund the provision of mosquito and other pest insect control services? 4

Shall county Charter be amended to provide that the board of county commissioners require by ordinance that a separate financial impact statement, not exceeding seventy-five words, including a two-year estimate of the increase or decrease in revenues or costs to the county resulting from approval of all proposed county Charter amendments and all proposed countywide referenda unrelated to county Charter amendment, be prepared by county budget director and placed on ballot immediately following ballot question? Holmes County Shall premises holding a license to sell alcoholic beverages be permitted to sell and serve alcoholic beverages on Sunday within Holmes County, Florida, outside of any municipality? Lee County Shall Lee County Ordinance No be approved, authorizing slot machine gaming at the Naples-Fort Myers Greyhound Track in Lee County subject to the requirements and restrictions of Florida law? 5

6 Referendum Regarding Levy of Sales Surtax by School Board to Finance Educational Facilities Bond Issued by the Marion County Hospital District to be Repaid from Increased Property Taxes Home Rule Charter Amendment Relating to Term Limits of County Commissioners Technical Amendments to Home Rule Charter Leon County Shall a one-half cent discretionary sales surtax be levied in Leon County for a period of 15 years, beginning January 1, 2013, to finance education facilities, including construction, reconstruction, retrofitting, renovation, remodeling, land acquisition and improvement and the purchase of technology equipment, hardware and software for the Leon County School District, which levy will extend the existing one-half cent discretionary sales tax that expires December 31, 2012? Marion County May the Marion County Hospital District issue bonds not to exceeding a principle amount of $65,370,000 to finance new hospital equipment and new and renovated hospital facilities, with the bonds to be repaid from increased property taxes, not to exceed one mill annually? The bonds will bear interest at a rate not exceeding the maximum lawful rate and mature between 5 and 6 years after issuance. Miami Dade County Shall the Charter be amended to provide that County Commissioners shall serve no more than two consecutive four-year terms in office excluding terms of service prior to 2012? Shall the Charter be amended to clarify the titles of subsections, correct and update cross-references between provisions, and delete references to offices and agencies which have been abolished? 6

7 Charter Amendment Requiring Extraordinary Vote to Include Additional Land within the Urban Development Boundary Charter Amendment Pertaining to Changes in Municipal Boundaries and Creation of New Municipalities Charter Amendment Regarding Penalties and Enforcement of Citizens Bill of Rights Charter Amendment Related to Option for Filling Mayoral or County Commissioner Vacancy Shall the Charter be amended to require a two-thirds vote of County Commissioners then in office to include additional land within the Urban Development Boundary established by the County s Comprehensive Development Master Plan? Shall the Charter be amended to: Require the County Commission to consider the benefits of any proposed annexation of commercial areas, when approving or authorizing an annexation Establish alternative procedure for creation of new municipalities in unincorporated areas of the County by petition which provides conditions for creation of new municipalities and a single election to approve the creation of a new municipality and approve its Charter, instead of two elections for these purposes? Shall the Charter be amended to eliminate the provision providing for forfeiture of office if a public official or employee willfully violates the Citizens Bill of Rights and allow, in addition to suit in circuit court, the Commission on Ethics and Public Trust to enforce the Citizens Bill of Rights with penalties authorized by the Code? Shall the Charter be amended to: Extend the time to conduct an election to fill a mayoral or commissioner vacancy from 45 to 90 days from the decision to call such election and provide a timeframe for qualification and any necessary runoff; Temporarily transfer, during a mayoral vacancy or incapacity, certain mayoral powers to the Commission Chairperson, Vice Chairperson or Commissioner chosen by the Board? 7

8 Charter Amendment Regarding Mayoral Conflicts in County Procurement Referendum Regarding Structures and Modification of Existing Agreements for the Tennis Center at Crandon Park Non-Binding Straw Ballot on Funding Improved Animal Services Programs Shall the Charter be amended to provide that when the County Mayor declares a conflict of interest in a particular procurement of a County contract, the chairperson of the Board of County Commissioners shall exercise all authority provided by the Charter or the County Commission to the Mayor with regard to such procurement including the authority to recommend a bid waiver? In accordance with Article 7 of the Home Rule Charter, do you approve as set forth in Resolution R : Erection of permanent structures and expansion of existing structures at Crandon Park Tennis Center for public park and tennis tournament use, which shall be funded solely by tennis center and tournament revenues and private funds; and Modification and extension of agreements with operator of Sony Open Tennis Tournament or its successors? Would you be in favor of the County Commission increasing the countywide general fund millage by mills and applying the additional ad valorem tax revenues generated thereby to fund improved animal services, including: Decreasing the killing of adoptable dogs and cats (historically approximately 20,000 annually); Reducing stray cat populations (currently approximately 400,000 cats); and Funding free and low-cost spay/neuter programs, low-cost veterinary care programs, and responsible pet ownership educational programs? 8

9 Non-Binding Straw Ballot on Contracting with Companies Doing Business with State Sponsors of Terrorism Referendum on Extending the One Cent Infrastructure Sales Tax Through 2033 Changing Florida Keys Aqueduct Authority Board of Directors from Gubernatorial Appointments to Elected Positions Local Method Filling Vacancies in Office of Commissioner and Mayor When Authorized Under Florida Law Would you support, to the extent permitted by law, prohibiting further the use of taxpayers dollars to procure services or capital improvement projects from companies actively doing business in countries that are on the U.S. Department of State s list of state sponsors of terrorism? Monroe County Shall the existing one cent infrastructure sales tax be extended through 2033 (it otherwise expires in 2018) with the proceeds used for the following projects: wastewater facilities and, only if those wastewater projects are completed or fully funded, recreation and conservation lands, marinas, courthouses, parking, offices, roads, bridges, airports, libraries, piers, auditoriums, riprap/seawalls, solid waste, jails, police/fire facilities, land acquisition, storm water, and any public purpose authorized by law? Should the selection of the members of the Board of Directors of the Florida Keys Aqueduct Authority be changed from positions appointed by the Governor of the State of Florida to positions elected by the voters of Monroe County, Florida? Orange County Shall the Orange County Charter be amended to prescribe, when authorized under Florida law, a method for locally filing offices of Commissioner and Mayor during vacancy or suspension, providing generally for appointment to fill vacant and suspended offices until the next general election, and for special election to fill the vacant office of Mayor where the Mayor s remaining term exceeds one year? 9

10 County Charter Amendment Changing Due Date of Charter Review Commission s Report Effect Within Municipalities of County Ordinances Concerning Simulated Gambling or Gambling County Charter Amendment Authorizing Designation of Unincorporated Villages and Creation of Village Advisory Boards Special Referendum Requiring County and Municipal Approval of Zoning or Comprehensive Plan Amendments Affecting Overcrowded Public Schools Shall the Orange County Charter be amended to change the due date of the Orange County Charter Review Commission s report to a date no later than the last day of qualifying for election to county office? Shall the Orange County Charter be amended to provide that Orange County ordinances shall be effective within municipalities and prevail over municipal ordinances when Orange County sets stricter minimum standards for prohibiting or regulating simulated gambling or gambling? Shall the Orange County Charter be amended to provide that, upon petition and approval at referendum by the electors of an unincorporated US Census Designated Place with population exceeding 25,000, such area shall be designated an unincorporated "village," with an appointed village advisory board having such powers, duties, and functions as set forth by charter and ordinance? Shall the Orange County Charter provision be reapproved to allow the continued effectiveness of the ordinance requiring that rezonings or comprehensive plan amendments (or both) (1) that increase residential density in an overcrowded school zone and (2) for which the school district cannot accommodate the expected additional students, take effect only upon approval by each local government located within the boundaries of that school zone? 10

11 Non-Partisan Elections for County Commissioners 2015 Penny for Pasco to Fund Job Creation, Public Safety, Environmental Lands, Infrastructure, and Education County Property Tax Exemption Osceola County Shall the Osceola County Home Rule Charter be amended to provide for nonpartisan elections of County Commissioners? Pasco County Shall a One-Cent Sales Surtax continue to be levied, beginning in 2015, to fund public infrastructure to 2024, and shared among Pasco County Schools (45%), Pasco County (45%), and Pasco s Cities (10%) for: Job Creation and Economic Development Projects (pursuant to Section (2)(d)3, Florida Statutes); Public Safety facilities, vehicles and equipment; acquiring Environmentally Sensitive Lands; school technology, career academies, and Educational Facilities Improvements; infrastructure related to Transportation and Safety; and public infrastructure within the Cities? Polk County Shall the board of county commissioners of this county be authorized to grant, pursuant to s.3, Art. VII of the State Constitution, property tax exemptions to new businesses and expansions of existing businesses that are expected to create new, full time jobs in the county? 11

12 Charter Amendment Providing Timetable for Proposed Charter Amendment Referendum; Effective Date of Voter-Approved Amendments Seminole County School District Ad Valorem Millage Election Slot Machines at Licensed Pari-Mutual Facilities Located in Palm Beach County Economic Development Property Tax Exemptions for New Businesses and Expansions of Existing Businesses Sarasota County Shall subsection 7.1 be amended providing: (a) referendum for Charter amendments proposed by citizen initiative, County Commission, Legislature held at next general election rather than within 60 days after filing, provided amendment filed with County Commission 120 days before election, (b) referendum for amendments proposed by Charter Review Board held at next general election, rather than next countywide election, provided proposed amendment filed with County Commission 120 days before election; (c) amendments effective upon certification of election results. Seminole County Shall Seminole County School District ad valorem millage be increased by up to one mill beginning July 1, 2013, and ending not more than four (4) fiscal years later on June 30, 2017, for essential operating expenses to: preserve A rated academic, vocational, arts, and athletic programs; retain highly qualified teachers; and repair and maintain school buildings with annual reporting to the county s citizens to ensure fiscal stewardship of the funds? Palm Beach County Shall slot machines be authorized within licensed pari-mutuel facilities in Palm Beach County subject to the restrictions of state law? Shall the Board of County Commissioners of Palm Beach County be authorized to grant, pursuant to s. 3, Article VII of the State Constitution, property tax exemptions to new businesses and expansions of existing businesses that are expected to create new, full-time jobs in Palm Beach County? 12

13 Approval of the Continuation of One-Half Mill Ad Valorem Tax for School District Operating Expenses Ad Valorem Tax Exemption Referendum Regarding Ad Valorem Tax Millage for School Operational Purposes of School District Economic Development Property Tax Exemptions for New Businesses and Expansions of Existing Businesses Pinellas County Shall the Pinellas County School District ad valorem millage of one-half mill per year be continued beginning July 1, 2013 and ending four fiscal years later on June 30, 2017, for necessary operating expenses including funds to recruit and retain quality teachers; preserve reading programs and music and art classes; and provide up-to-date textbooks and technology, with oversight of these expenditures by an independent citizens financial oversight committee? St. Lucie County Shall the Board of County Commissioners of St. Lucie County be authorized to grant pursuant to Section 3, Article VII of the State Constitution, property tax exemption incentives to new businesses and expansions of existing businesses that are expected to create new, full-time jobs in the County? Volusia County Shall the School District of Volusia County voter-approved ad valorem millage be one mill for school operational purposes to support academic programs, including the arts, the recruitment and retention of teachers, and student activities and athletics, beginning July 1, 2013, and ending on June 30, 2017, with annual reporting of these funds to the citizens of Volusia County? Wakulla County Shall the Board of County Commissioners of Wakulla County, Florida, be authorized to grant, pursuant to s. 3, Art. VII of the Florida constitution, property tax exemptions to new businesses and expansions of existing businesses that are expected to create new, full-time jobs in the County? 13
