1 Moving Alachua County Forward One Percent Transportation Sales Surtax Tourist Development Tax Valorem Tax Exemptions for New Businesses and Expansions of Existing Businesses One-half Cent Sales Surtax for Critical School Facilities Renewal, School Security and Technology Upgrades Extension of the One Percent (1%) Local Option Sales Tax From January 1, 2015, to December 31, 2020 Sales Surtax for Roadway Maintenance and Resurfacing Within Citrus County County Wide Indigent Care One- Half Cent Sales Surtax for Payment of DeSoto Memorial Hospital Indebtedness Alachua County To improve funding for the planning, development, financing, construction, reconstruction, operation, capital investment and maintenance of roads and bridges, bus systems, and bicycle and pedestrian projects, shall Alachua County levy a countywide one percent (1%) sales surtax for eight years, beginning January 1, 2015, with funds to be applied to the transportation project lists adopted by Alachua County and the cities of Alachua County, and subject to citizen oversight and annual independent audit? Bay County Shall the area subject to the Bay County Tourist Development Tax be expanded to include the area within the city limits of the City of Panama City with the funds collected to be used as described in the Panama City element of the Bay County tourist Development Plan, all as described in Panama City Resolution No and Bay County Resolution No. 3236? Brevard County Shall the Board of County Commissioners of Brevard County continue to be authorized to grant, pursuant to s. 3, Art. VII of the State full-time jobs in the county? Would you support the School Board levying a one-half cent surtax on sales in Brevard County, beginning 1/1/15, for a period of six years, to be used exclusively for critical school facility renewal projects, school security and technology upgrades? A needs-based prioritized list of projects has been published and expenditures will be monitored by an independent citizen oversight committee. Charlotte County Should the one percent local option sales tax be extended for six (6) years from January 1, 2015, to December 31, 2020, with the proceeds to be used for infrastructure as defined by law, including school security and technology improvements; road improvements; public safety and service buildings; and libraries, parks and recreational facilities? Citrus County Shall Citrus County fund ongoing maintenance and periodic resurfacing of roads by levying a one percent sales surtax from January 1, 2015 until December 31, 2034, with proceeds to be deposited into a dedicated trust fund, shared with the cities of Crystal River and Inverness, and restricted for these transportation expenditures? DeSoto County Shall DeSoto County levy a countywide one-half cent sales surtax? The proceeds of the sales surtax shall be used to service existing indebtedness of DeSoto Memorial Hospital. The proceeds shall not be used to pay the salaries of the Hospital staff or the day-to-day operating expenses of the Hospital.

2 Continuation of Current Fire Protection as Required by Existing Federal Guidelines Continuation of One-Half Cent Sales Surtax to finance Educational Facilities for Ten (10) Years Extended Levy of County-Wide One-Cent Sales Tax for Infrastructure/Public Facilities Increase Existing Tourist Development Referendum Election to Consider Increasing the Existing Tourist Development Tax from Two Percent to Four Percent on Rentals of Less than Six Months for the Purpose of Advertising Tourism, to Construct and Maintain Tourist Related Infrastructure and Beach Facilities, Including Debt Service on Bonds Referendum to determine the granting of Economic Development Ad Valorem Tax Exemptions Dixie County Are you in favor of increasing the fire services assessment on real property from $40 per residence to $75 per residence and from $45 per business to $100 per business with no increase on vacant land in order to continue the current level of fire service provided by Dixie County as required by National Fire Protection Association guidelines? Escambia County Shall the School Board of Escambia County, Florida, continue to levy a one-half cent sales surtax for a period of ten (10) years beginning January 1, 2018 to fund construction of new schools, renovations and additions to existing schools, land acquisition and improvements, and the upgrading and equipping of schools for technology? Shall the one-cent sales tax, approved by the voters of Escambia County by referendum held March 7, 2006 for eleven years, be extended from January 1, 2018, through December 31, 2028, to provide for law enforcement/fire and public safety facilities and equipment, transportation and drainage improvements, infrastructure projects/public facilities, recreation/natural resources, jail/court facilities, Growth Management Act mandates, capital equipment, community services, and economic development projects per section (2)(d)3, Florida Statutes? Franklin County Shall Franklin County increase the existing Tourist Development Tax from the current two percent tax to a four percent tax on each dollar for every person who rents, leases or lets for consideration any living quarters or accommodations in any hotel, apartment hotel, motel, resort motel, apartment, apartment motel, roominghouse, mobile home park, recreational vehicle park, condominium or timeshare resort for a term of six months or less, to promote and advertise tourism; to construct and maintain tourist related infrastructure; to provide and maintain beach park and facilities, beach renourishment, maintenance and erosion control; and other purposes authorized in Florida Statute Section , including debt service on bonds to finance the construction, reconstruction or renovation of a convention centers, sports complexes or museums? Gilchrist County Shall the Board of County Commissioners of this county be full-time jobs in the county?

3 Optional One-Cent Sales Surtax Referendum County Charter Revision Regarding Setting Consecutive Terms of the Board of County Commissioners One Cent Local Government Surtax Extension Valorem Tax Exemption One-Cent Sales Tax to Fund Reconstruction of Existing Roads, Drainage and Bridges Children s Services Council of Martin County Ad Valorem Reauthorization Hernando County Shall there be levied for ten years, beginning on January 1, 2015, a one-cent sales surtax, with 50% of the proceeds therefrom to be used by the Hernando County School Board for technology enhancements that will benefit teachers and students and for school renovations and upgrades, and with the remaining 50% of the proceeds to be allocated between Hernando County and Brooksville to fund road and transportation improvements, utility improvements, and economic development projects? Lee County County Charter Amendment Section 2.2.A Legislative Branch The County Commission ; limiting the number of consecutive terms of the Board of County Commissioners; amendment would limit County Commissioners to serving no more than three (3) consecutive four-year terms in office excluding terms of service prior to January 1, Leon County To provide for projects designed to improve roads; reduce traffic congestion; protect lakes and water quality; reduce flooding; expand and operate parks and recreational areas; invest in economic development; and other uses authorized under Florida law; and to seem matching funds for these purposes, shall the existing one cent sales tax within Leon County be extended until December 31, 2039, with project expenditures subject to annual independent audit and review by a citizens advisory committee? Levy County Shall the Board of County Commissioners of Levy County be full-time jobs in Levy County? Martin County Shall Martin County levy an additional one-cent sales tax for four (4) years to fund reconstruction of existing roads, drainage and bridges to meet required codes and standards? Each county expenditure shall be approved at an advertised public hearing. Martin County s city and towns will receive funds, as provided by law, from the one-cent sales tax for infrastructure projects. The Children s Services Council of Martin County is an independent special taxing district approved by Martin County s voters in 1988 to enhance the health and well being of Martin County s children. Shall the Children s Services Council of Martin County be reauthorized to fund services for Martin s County s children unless dissolved by the County Commission or special act of the Legislature as provided by law?

4 Funding Court Projects Through Issuance of General Obligation Bonds Reauthorization (or Retention) of the Children s Services Council of Okeechobee County Valorem Tax Exemption Regarding Elections on Citizens Petition Initiatives Regarding Limiting the Power to Amend the Charter Miami Dade County Shall the County fund emergency repairs to the 1928 Courthouse and the acquisition and construction of new court facilities by issuing, in one or more series, General Obligation Bonds paid or secured by taxes derived from the assessed value of property in the County (ad valorem taxes), potentially increasing property taxes, in a principal amount up to 393 million dollars, bearing interest not exceeding maximum legal rate, and maturing within 30 years from issuance? Okeechobee County The Children s Services Council is an independent special district approved by voters in 1990 and since that time has provided preventative, development, treatment, rehabilitative and other services for children in Okeechobee County. Shall the Children s Services Council of Okeechobee County be reauthorized to continue in existence, unless it is dissolved by either the Board of County Commissioners or by a special act of the Florida Legislature? The Valorem Tax Exemption would allow Okeechobee County to encourage the establishment of new businesses by granting an exemption from County property taxes of up to one-hundred (100%) of only the assessed value of improvements and personal property for those businesses. Any exemption would only remain in effect for up to ten (10) years and would not apply to school, city, or voter approved taxes. Shall the Board of County Commissioners of Okeechobee County, Florida, be full-time jobs in Okeechobee County? Orange County For the purpose of allowing citizens more time to consider referendum elections on initiative petitions and to provide certainty regarding petition submittal requirements, shall the Orange County Charter be amended to require petition initiatives to have sufficient signatures verified at least 150 days prior to the next primary, general or special election? Shall the Orange County Charter be amended to limit the amendment of the Charter by initiative, and the enactment, amendment or repeal of ordinances by initiative, where the initiative concerns the regulation of employer wages, benefits, or hours of work; or the encumbrance or allocation of tax revenues not authorized by law or conditioned upon a prospective change in law; and to impose prohibitions on the Board of County Commissioners?

5 Moving Countywide Charter Office Elections and Making All Charter Office Elections Partisan Providing for Term Limits and Non-Partisan Elections for County Constitutional Officers Reauthorization of the Children s Services Council of Palm Beach County Levy of Countywide One Percent Sales Surtax to Fund Greenlight Pinellas Plan for Public Transit 1% Sales Surtax for MyRide/MyRoad Public Transportation, Roadways, and Bus System Sales Tax for Courthouse Reauthorization of the Children s Services Council of St. Lucie County Shall the Orange County Charter be amended to move elections for all Charter offices elected countywide to 2016 and every four years thereafter, abbreviate the term of any office as necessary to comply with this provision, change all Charter office elections from nonpartisan to partisan elections and eliminate procedures required for nonpartisan elections? For the purpose of establishing term limits and non-partisan elections for the Orange County Clerk of the Circuit Court, Comptroller, Property Appraiser, Sheriff, Supervisor of Elections and Tax Collector, this amendment provides for county constitutional officers to be elected on a non-partisan basis and subject to term limits of four consecutive full 4-year terms. Palm Beach County Shall the Children s Services Council of Palm Beach County, which provides early learning and reading skills, development, treatment, preventative and other children s services in Palm Beach County, be authorized to continue in existence as a district with voterapproved taxing authority, with independent oversight and accountability as required by law, unless and until it is dissolved as specified in s (4)(a), Florida Statutes? Pinellas County Shall the improvement, construction, operation, maintenance and financing of public transit benefitting Pinellas County, including an expanded bus system with bus rapid transit, increased frequency and extended hours, local passenger rail and regional connections be funded by levying a one percent sales surtax from January 1, 2016 until repealed, with the proceeds deposited in a dedicated trust fund? Polk County Shall Polk County fund the MyRide/MyRoad initiative to provide for comprehensive transportation and bus networks, including express bus service to airports, area attractions, and veteran s hospital, transportation for night shift workers, maintenance and improvements to the County s roadways, and construction of new roadways, by levying a one percent sales surtax from January 1, 2015, until December 31, 2029; to be deposited into trust fund and half shared with Polk Transit Authority? Santa Rosa County Shall a one-cent sales tax be levied to fund the construction of a new Courthouse at a location to be determined by referendum? The tax will be levied for a period of five (5) years? St. Lucie County The Children s Services Council is an independent special district approved by voters in 1990 and since that time has provided preventative, development, treatment, rehabilitation and other services for children in St. Lucie County. Shall the Children s Services Council of St. Lucie County be reauthorized to continue in existence, unless it is dissolved by either the Board of County Commissioners or by a special act of the Florida Legislature?

Wakulla County Shall Wakulla County ordinances and be readopted to provide local control and protections for Wakulla County s wetlands, and surface waters by regulating development and other activities that could degrade, destroy, or negatively impact these waters and the 75-feet of upland around these waters and providing that these ordinances can only be modified or repealed by referendum vote or unanimous vote of all five County Commissioners, to take effect upon passage? Shall the charter of Wakulla County, Florida, be amended to require that County Commissioners, the Clerk of Court, the Property Appraiser, the Sheriff, The Supervisor of Elections, and the Tax Collector be nonpartisan offices chosen by vote of the electors in a nonpartisan election instead of one indicating a political party affiliation? Shall the Charter of Wakulla County, Florida be amended to require persons elected to the county commission provide a notarized declaration and oath stating their intent to reside in the district in which elected and provide their voter registration card, verifying their physical address in the district, within 30 days after the election? Shall the Charter of Wakulla County be amended to require the County Commission to adopt and adhere to a fund balance policy regulating management and reservation of fund balances; integrating the policy with budget and financial policies to maintain fiscal strength and credit of the county; requiring minimum policy elements addressing spending order of fund balances, reserve fund target balances, replacement of fund deficits, use of reserve funds, annual review of fund balances and reserves? Shall the Charter of Wakulla County be amended to require the county commission to adopt and adhere to a debt policy regulating the issuance and management of debt; requiring integration with other fiscal policies including the fund balance policy to maintain fiscal strength and credit quality of county; requiring the policy to address specific purpose of debt, limitations on debt, credit objectives, features of the debt including interest rate, and placement procedures for debt? Washington County Shall the half-cent sales surtax be levied for ten (10) years in Washington County to provide for improvements and construction of school capital outlay projects, including construction additions, renovations and replacements of school facilities as well as to finance the educational technology needs of the district?
