1 Videoconferencing System Welcome to the Equipment Cart tutorial. A Telehealth videoconferencing system may include these components: Video Teleconferencing CODEC Video Camera Video Monitor (1 or 2) Microphone Speakers Peripheral devices Computer Accessories Technology in group clinics may include: document cameras, presenter software, interactive whiteboards, gaming systems, projectors, and large monitors.
2 Terminology Become familiar with the following Telehealth technology terms and definitions. CODEC: Acronym for coder-decoder. This is the videoconferencing device that compresses the video signal and sends it over a telecommunications link to the receiving site. Consumables: Hygiene guards and other disposable items used with peripheral devices during a Telehealth encounter Dermascope: A video device used to display close-up images of skin abnormalities Document camera: A video camera used to display written or typed information, photographs, graphics, and in some cases x-rays Exam camera: A video camera typically used to examine the general condition of the patient IP: Acronym for Internet Protocol. This is the protocol by which data is sent from one computer or CODEC to another via the VA Intranet or the Internet. Each video endpoint has an individual IP address used for calling. Peripheral devices: Any device that is attached to a computer externally. In Telehealth this includes clinical devices, such as exam cameras, stethoscopes, and otoscopes, in addition to typical peripherals such as keyboards, mouse pointers, and printers.
3 Telehealth Tools The provider who sees a patient in person is able to see and hear whatever is relevant to making an accurate diagnosis. The introduction of distance between the physician and the patient requires specialized imaging tools for the telepresenter to use to help the physician to make an informed decision. Main Camera: The video camera is specially designed for videoconferencing applications. It may offer pan (side to side) and tilt (up and down) controls so that the user can easily point the camera at the appropriate subject. It also may provide control of the zoom for a wider or tighter angle of view. (Pan/Tilt/Zoom is often abbreviated P/T/Z.) The focus is automatically adjusted by the camera. Most systems provide connections for two or more video cameras, facilitating the addition of specialized diagnostic cameras. Exam Camera: The exam camera is a multipurpose camera that can be used to show the provider close up images of the body. Video Otoscope: The video otoscope is based on the standard otoscope and is typically used to view the ears, nose, and throat. It connects to a light box through a fiber optic cable to provide an external light source. The light box also has a connection for the otoscope camera.
4 Digital Stethoscope: The digital stethoscope is used to listen to heart, airway, and lung sounds (auscultation). It works like a normal stethoscope, but the sounds can be transmitted over a distance to the provider. The stethoscope is a basic tool for the provider and its use is a basic clinical skill. Document Camera: Not typically on a video cart, but can be used as a general purpose device to share documents, electrocardiograms (ECGs), x- rays, graphs, and other information materials. The document camera must be equipped with an integrated light box for x-rays and other film-based information. Headphones: The headphones are used by the telepresenter to listen to the digital stethoscope.
5 Peripherals and Accessories Remote controls and other equipment accessories should also be placed for easy access during the Telehealth exam. Peripheral Devices: Peripheral devices, such as the exam camera, otoscope, and dermascope, should be connected to the video cart and ready for use at any time. Reset the white balance on the devices as necessary. To do this, point the camera at a blank white paper and press the white balance button for several seconds. Remote Controls: Keep track of remote controls and secure them when not in use. Keep spare batteries on hand. There is a remote for the videoconferencing equipment and there may be another for an external monitor. Velcro and labels may help keep remotes where they belong. Wire baskets: Wire baskets may be included with primary/specialty care carts. These baskets can be used to store manuals, How to checklists, or peripheral devices. Wire and Cords: Use cable ties to secure loose wires together and to the equipment cart. A single multi-outlet surge protector means that only one cord is required to plug the equipment cart into the power outlet. Use tape or take other precautions to ensure the power cord and network cable(s) do not become a tripping hazard.
