1 Quarterly Quality Report Calendar Year rd Quarter (July-September ) Health & Rehab is a not-for-profit organization that operates three facilities in the metro Atlanta area providing a range of care including therapy and rehabilitation services, shortterm recovery and long-term care. Our staff of serves a total resident capacity of more than. We know that high quality health care is critical to residents, families and others who look to as a trusted leader in senior care. This quarterly quality report highlights statistics for some of the most important factors considered when evaluating a skilled nursing or rehabilitation facility. These statistics reflect calendar year third quarter, July-September. They include comparisons among Health & Rehab s three facilities, more than facilities in and more than, facilities nationwide. 2 The following charts reflect July-September data from the American Health Care Association s Long-term Care Trend Tracker. Overall star ratings has the highest star quality ratings possible. homes have -star quality ratings, which is the highest possible rating. Our homes exceed the state average of and nationwide average of.8 star quality ratings. Nursing home ratings come from health inspections, staffing and quality measures. Star Ratings Staff hours per patient day has high total staff hours per patient day, which allows staff to provide more personalized care. Staff hours per patient day is the total number of staff providing patient care compared to the number of patients. At, we have total staff hours per patient day of.8. The statewide average is.8 and the nationwide average is.. 7 Staff hours per patient day Health & Rehab

3 Fall incidents takes significant measures to mitigate the risks associated with falling, and we have a lower fall incident rate than statewide and nationwide averages. A fall refers to unintentionally coming to a rest on the ground, floor or other lower level, but not as a result of an overwhelming external force. An episode where a resident loses his or her balance, and would have fallen if not for staff intervention, is considered a fall. While most falls are benign and result in no injuries, takes significant measures to mitigate the risks associated with falling. has a lower fall incident rate than the statewide and nationwide averages with.9 percent of residents with fall incidents compared to. percent in and. percent nationwide. 8 % of residents with incidents of falls Antipsychotic medications has a lower-than-average percentage of residents who take antipsychotic medications. In March, CMS launched a nursing facility quality initiative that included a goal to decrease the off-label use of antipsychotic medications by percent by December. A.G. Rhodes continues to be successful in this effort, which reflects a low percentage, 7.8 percent, of residents who take antipsychotic medications. 4 Acquired catheters Only. percent of residents at acquired urinary catheters this quarter, compared to.7 percent statewide and.9 percent nationwide % of residents with acquired urinary catheters Physical restraints takes many steps to reduce or eliminate the need for restraints, which contributes to its low percentage of residents who require restraints. Physical restraints are items used to restrict, restrain or prevent movement of a person. Restraints are used as a last resort and only after less restrictive alternatives have been tried. takes many steps to reduce or eliminate the need for restraints, which contributes to its low percentage,.78 percent, of residents who required restraints from July-September. This is compared to. percent statewide and.98 percent nationwide % of residents requiring physical restraints Health & Rehab

5 Unexpected weight gain or loss has a low percentage of residents with unexpected weight fluctuations. Weight can be a useful indicator of nutritional status when evaluated within the context of the individual s personal history and overall condition. We monitor residents weight to ensure significant, unintended changes are addressed immediately. had a low percentage of residents,.9 percent, with unexpected weight fluctuations July-September. This is compared to percent statewide and.8 percent nationwide. % of residents with unexpected weight gain/loss Pressure ulcers takes significant steps to prevent pressure ulcers, which contributes to its very low percentage of residents who experience them. A pressure ulcer is any lesion caused by unrelieved pressure that results in damage to the underlying tissue. takes significant steps to prevent pressure ulcers, however in rare instances they are unavoidable.
