1 ANSWERS TO FREQUENTLY ASKED QUESTIONS DENTISTS RENEWALS When will I renew my license? Renewal notices will automatically be mailed to your credential address in November prior to renewal. You will be required to renew your license prior to December 31 st every odd numbered year. If you do not receive your renewal notice by November 30 th, please contact the Dental Board office to request a duplicate renewal form. How many credit hours do I need to renew? Forty (40) hours of Board approved continuing education credits. CE LOG What types of courses are acceptable for continuing education? Any courses provided by a Board approved Permanent or Biennial Sponsor, Academy of General Dentistry PACE, or the American Dental Association CERP program that include, but are not limited to, subjects that address any of the following: (1) Competency in treating patients who are medically compromised or who experience medical emergencies during the course of dental treatment; (2) Knowledge of pharmaceutical products and the protocol of the proper use of medications; (3) Competence to diagnose oral pathology; (4) Awareness of currently accepted methods of infection control; (5) Basic medical and scientific subjects including, but not limited to, biology, physiology, pathology, biochemistry, and pharmacology; (6) Clinical and technological subjects including, but not limited to, clinical techniques and procedures, materials, and equipment; (7) Subjects pertinent to health and safety. How many credit hours may be obtained by home study programs? Currently all of them as long as there is a graded testing mechanism provided at the end of the program.

General Anesthesia Permit Application Conscious Sedation Permit Application What continuing education do I need to renew my Anesthesia/Conscious Sedation Permit? 1.Basic Life Support; and 2. 3 Two (2) [See OAC (D3)] PATIENT RECORDS How long do I have to keep patient records (ex. X rays, charts, etc.)? It is suggested that a physician/dentist maintain patient records for at least seven (7) years, and although impractical the best advice would be to keep the records indefinitely. Who owns patient records and do I have to provide copies of the records? A dentist must release a copy of the patient record upon request of the patient or the patient s parent/legal guardian. Failure to do so could be a potential violation of the standard of care for the profession, which is grounds for discipline. A patient record includes any document or combination of documents that pertains to a patient s medical/dental history, diagnosis, prognosis, or medical/dental condition, and that is generated and maintained in the process of the patient s dental treatment. Patient records include: Medical/dental history Written progress notes Radiographs Models Billing information Insurance claims A patient record should not include: Care related to another patient Peer review/quality assurance information/documents Correspondence/notes from attorneys Aberrant/deviant statements Ownership: Patient records belong to the owner dentist or treating practitioner, but the patient has an absolute right to a copy of their records. Copies: A dentist has the ethical obligation on request of the patient, parent/legal guardian, subsequent treating dentist with patient release, to furnish copies of the patient s records. Costs: This obligation to provide a copy of the records exists whether or not the patient s account is paid in full. Further, a dentist cannot demand payment for copies up front. The dentist may add a fee for copies to the unpaid balance and include this in any claim for reimbursement, but records cannot be held hostage for payment.

6 Yes, you must be currently certified by the American Red Cross (ARC), American Heart Association or American Safety Health Institute (ASHI), at the time of licensure renewal. PERMISSIBLE DUTIES Questions: What do I have to do in order to practice dental hygiene without the dentist being present? A dental hygienist may provide, for not more than fifteen consecutive business days, dental hygiene services to a patient when the supervising dentist is not physically present at the location where the services are provided if all the following are met, as of March 23, 2015: GENERAL QUESTIONS The dental hygienist has at least one year and a minimum of one thousand five hundred hours of experience in the practice of dental hygiene. The dental hygienist has successfully completed a course approved by the state dental board in the identification and prevention of potential medical emergencies. The supervising dentist completed and evaluated a medical and dental history of the patient not more than one year prior to the date the dental hygienist provides dental hygiene services to the patient and, except when the dental hygiene services are provided in a health care facility, the supervising dentist determines that the patient is in medically stable condition. 9 GENERAL QUESTIONS How long do I have after I finish the initial training course before I can take the radiology examination? You have sixty (60) days within which to complete the clinical portion (actually taking x rays) of the course (you may complete this portion before the allotted time). After that time, however, you must cease taking x rays until such time as you become certified as a Dental Radiographer. Once you have completed both portions of the course you may take the radiology examination. Is there a time limit on when you can take the radiology examination? No. Once you have completed both portions of the initial training course, lecture and clinical, you can take the course immediately or wait a while. Remember though, if you do not hot hold a current Radiographer Certification, you cannot take x rays after the sixty (60) days allowed for completion of the clinical portion. ADDRESS/STATUS CHANGES How/When do I need to notify the Board of a change of address? You need to send the change of address in writing either by fax, mail, or to the Dental Board office and include your name, license number, and new address, within ten (10) days of such change. Change Form How do I retire my certification? There is no allowance in the Dental Practice Act for retiring a Dental Assistant Radiographer Certification. You must either renew it as scheduled or allow it to lapse for failure to renew; in which case you would need to request a reinstatement form from The Dental Board if you decided to renew later. Is a Radiographer required to have CPR? No. Only the dental hygienist is required to be certified in CPR at the time of renewal.
